CVD cases correspond to any first occurrence of MCE, IS (ICD10: I63, I69.3), ICH (I61, I69.1), or SAH (I60, I69.0) till the censoring date of 1 Jan 2018. MCE refers to nonfatal myocardial infarction (I21-23) or fatal IHD (I20-25) here.
