Immune-related thyroiditis in a 50-year-old female with lower leg Meckel-cell carcinoma who developed mediastinal metastatic spread as shown on baseline 18F-FDG-PET/CT [coronal PET, CT and merged PET/CT images, # (A)]. A thyroid nodule was also present [arrow, fused PET/CT image (A)]. Pembrolizumab (anti-PD-1) was administered. Follow-up 18F-FDG-PET/CT at 6 months showed disappearance of mediastinal disease and decrease in metabolic activity of the thyroid nodule [arrow, fused PET/CT image (B)]. However, a marked increase in thyroid activity was also evident, consistent with a thyroiditis [arrowheads, fused PET/CT image (B)].
