As
part of our ongoing efforts to better
understand the global malaria situation, we are surveying major bilateral and
multilateral funders on their funding of malaria related endeavors in the year
2000. The data will be compiled and
analyzed to better inform governments, funders, non-governmental organizations,
and others whose common goal is the global control of malaria.
Please
find the attached survey that asks details of your agency’s malaria control
funding. The survey is divided into
four major parts:
§
General Information on Funding Policies
§
General Malaria Funding Profile
§
Malaria Funding By Activity for Year 2000
§
Analysis by Funding Recipient
You
have two options as to how to fill out the survey:
Option 1: Fill the survey out electronically on
your computer with Microsoft Word using the boxes embedded in the document ,
save it, and reattach it to a response e-mail (note some calculations will
automatically happen in this case.)
OR
Option 2: Print the survey in either HTML or Word format, complete it, and fax it to:

We
realize that this is a detailed survey and may require some effort to fully
complete. We also understand that many programs are not malaria specific, but
rather part of integrated health programs.
However, we ask that you please do your best to complete all possible
information as we believe it will help us better inform all participants in
global malaria control. Please estimate to the best of your
knowledge whenever necessary and indicate you have done so.
Thank you in advance for your help and
effort. We hope to have a completed
report finished by the fall of this year.
Please let us know if you have any questions or require any
clarification.
Sincerely,
Vasant

requires a number of calculations as well as funding details. All responses are coded with a number and
letter e.g. 7A. Please be sure to use
the appropriate values for your answers.

This survey
will require data from your most recent 12 month reporting period. Will you be providing data from?

Another Period – From (month/year) to (month/year)
Please fill out the following chart
based on your agency’s obligations/commitments and disbursements
for the reporting period.

programs. Please consider the values
you have used in 8A and 8B above: Health

Please estimate the
percentage of the values in the categories that you would consider devoted to
malaria or to regions where malaria is a major cause of morbidity and
mortality.

11. Total Malaria Funding Calculation
In order to
calculate your organizations total Malaria funding amount please complete the
following calculation using the values from the previous questions.
Note: If you fill this form out electronically,
this calculation should occur automatically.
If the computer does not calculate the values please manually enter
them.
Total Malaria Commitments:

9A 8A % from 10A
Total Malaria Disbursements
+ (X .) = (11B)
9B 8B % from 10B

Please ensure you feel that the values in item 11 are accurate as they will serve as the basis of the analysis
of your answers to subsequent questions.
If there are other sources of malaria
related funding that are not accounted for in the above calculation please
explain below and provide a revised estimate. If you are satisfied with the
values in item 11A and 11B please re-copy them in boxes 12A and 12B below.

Part IV: Analysis of Funding Recipients
This section of the survey will focus
on specific issues on how your agency supports malaria control programs.
Please estimate wherever an exact value
is not available. We may ask for
specifics of programs in various countries as a follow-up to this initial
evaluation.
This section is divided into two main
areas
Multilateral
&
Multi-Bilateral: Transfer
of funds from an Overseas Development Aid (ODA) Agency to the UN or any of its
specialized agency’s (e.g. WHO, UNICEF, UNDP, World Bank). Including funds channeled to a
specific
program through a UN agency.
Bi-Lateral: Transfer of funds from ODA
agency to a recipient country government.
16. Given your value for 12A in the previous section; please breakdown your agency’s total
obligations for malaria control by the percentage devoted to each type of
funding recipient. Please note 100% = 12A (total malaria commitments). If necessary give actual monetary
values.
