Thank you for asking me to referee this paper. This short simple study has more than adequately demonstrated an error of some clinical significance in current ATLS teaching. This is, therefore, an original piece of work with some significance to all ATLS practitioners which must include all those at the current sharp end of anaesthetic and surgical practice.

The simplicity of the study belies its reliability and whereas it may be possible to take issue with the choice of the 50th percentile as the cut-off point it nevertheless clearly explains the relevance of the study. I recommend therefore that this paper be accepted as a short report. There is only one typographical error on page 3, paragraph 1, line 6 where I believe that "failure to non-invasive" should be "failure of non-invasive".
