A number of mistakes were introduced in Table 2 as a result of proof corrections, as follows:

Line (from top of Table 2), column:

1.6, col. 2 “Cases” should be “Cases/controls”.

1.6, col. 5 “Cases” should be “Cases/controls”.

1.11, col. 3 “(−0.269, 1.458)” should be “(−0.269, 21.458)”.

Line (from bottom of Table 2), column:

1.19, col. 2 “Cases” should be “Cases/controls”.

1.19, col. 5 “Cases” should be “Cases/controls”.
