Mystery membrane: A diagnostic dilemma

A 69-year-old female presented with nonresolving corneal edema post an uncomplicated small incision cataract surgery, a diagnosis of Descemet membrane detachment Figure 1 was made, for which the patient underwent SF6 injection.[1,2]

Clinical picture 1 week postoperatively

On repeat clinical examination, a refractile membrane attached to the posterior surface of the cornea was seen, with the anterior-segment optical-coherence-tomography revealing an intact Descemet membrane Figure 2 and a separate membrane extending from the posterior surface of the cornea overlying the iris Figure 3. Following this, the patient underwent membranectomy, with a remarkable improvement in visual acuity from the hand movement close to face preoperatively to best-corrected visual acuity of 20/40. On the basis of histopathological examination, a retained anterior capsule Figure 4 of the lens was diagnosed.
