A 31-year-old gravida 4 with previous 3 abortions presented at 10 weeks with ultrasound diagnosis of missed abortion. USG-guided suction and evacuation was planned. Cervix was gradually dilated and procedure was completed under USG guidance with flexible Karman cannula No. 8. While removing the cannula, slight resistance was felt at internal cervical os and to our surprise, tip of Karman’s cannula was found missing. Repeated attempts were undertaken using hysteroscopy but were unsuccessful due to blunt nature of the tip and tight internal os. Procedure was deferred. She was posted again for hysteroscopic removal of tip of foreign body. Cervix was gradually dilated with hegar dilator till no. 10. Operative hysteroscope with an optical grasper forcep was used to catch hold of the cannula tip under hysteroscopic guidance and ensure its safe retrieval. (Figs. 1, 2(a–f)).
