The following change has been made to the original publication of this manuscript. In the section Subjects and methods, beneath subheading Inclusion criteria, the first sentence of the second bulleted paragraph has been emended to read: “If the force exceeded 400 cN after the first or the second week, the expansion screw was turned 1 mm back or until the spring scale read under 400 cN.” instead of: “If the force exceeded 400 cN after the first or the second week, the expansion screw was turned 1 mm or until the spring scale read under 400 cN.”
