We thank Lansbergen and colleagues for their interest in the recent European Academy of Neurology guidelines on the treatment of cluster headache [1]. The authors narrow in on the issue of the place of occipital nerve stimulation in the treatment of chronic cluster headache, and the recommendations around sphenopalatine ganglion stimulation.
It is European Academy of Neurology policy to use the GRADE system [2, 3], to which we refer interested readers. Table 15 of the cluster headache guidelines sets out the grade profile for occipital nerve stimulation and Table 14 the findings for sphenopalatine ganglion stimulation. Whilst these may not align with the correspondents' views, they have the advantage of being semi‐objective. Certainly, local experience with the implanter will colour assessment of adverse events, so we understand their experience with the device may be more favourable in that domain.
