ABSTRACT
Background:
Orthodontic treatment is known to significantly impact facial esthetics, but there is a lack of long-term studies examining its effects, particularly in conjunction with the natural aging process. This study aims to fill this gap by investigating the enduring effects of orthodontic treatment on facial esthetics and aging over a 15-year period.
Materials and Methods:
Three hundred orthodontic patients aged 12 to 25 at the start of treatment were included. Dental alignment, soft tissue transformations, skeletal changes, and patient-reported outcomes were measured objectively. Statistical analyses were conducted to compare pretreatment and post-treatment measurements.
Results:
Significant improvements were observed across various aspects following orthodontic treatment. Dental alignment showed a marked reduction in crowding, with an average decrease of 5.2 mm. Soft tissue transformations revealed notable advancements in lip positioning, averaging 3.8 mm, along with increased lip thickness. Skeletal changes were relatively modest, characterized by subtle alterations in chin positioning and mandibular angles. Patients reported high levels of satisfaction, reflected by an average score of 9.2 on the satisfaction scale. Despite treatment efficacy, the aging process continued to manifest natural changes in skin elasticity and fat distribution over the 15-year follow-up period. These findings underscore the multifaceted nature of orthodontic outcomes and highlight the importance of considering long-term effects on facial esthetics and aging.
Conclusion:
This longitudinal study provides valuable insights into the long-term effects of orthodontic treatment on facial esthetics and aging. It highlights significant improvements in dental alignment and soft tissue transformations, along with sustained patient satisfaction. However, modest skeletal changes and the ongoing aging process emphasize the need for managing patient expectations. These findings can guide practitioners and patients in making informed decisions regarding orthodontic interventions.

INTRODUCTION
Orthodontic treatment plays a crucial role in enhancing dental alignment and occlusion, thereby significantly influencing facial esthetics.[1] While short-term outcomes of orthodontic interventions have been extensively studied, there remains a notable gap in understanding the long-term effects on facial esthetics, particularly in relation to the natural aging process.[2]
Previous research has primarily focused on immediate post-treatment changes, overlooking the evolving nature of facial esthetics over time. Factors such as individual variability, treatment methodologies, and the dynamic process of aging contribute to the complexity of assessing long-term outcomes.[3]
To address this gap, the present study proposes to investigate the enduring effects of orthodontic treatment on facial esthetics over a 15-year period. By evaluating dental alignment, soft tissue transformations, skeletal changes, and patient-reported outcomes, this study aims to provide a comprehensive understanding of the sustained impact of orthodontic interventions on facial esthetics and aging.
MATERIALS AND METHODS
Study population
The study population comprised 300 orthodontic patients aged 12 to 25 years at the initiation of treatment. Inclusion criteria encompassed individuals who had completed orthodontic treatment within the specified age range.
Study duration
A longitudinal follow-up of 15 years post-treatment was conducted to observe the evolution of facial esthetics and aging.
Measurements
Dental Alignment: Objective measurements of dental alignment were obtained, focusing on reductions in crowding or spacing in millimeters.
Soft Tissue Transformations: Soft tissue changes, including lip and cheek positioning and thickness, were assessed using standardized techniques.
Skeletal Changes: Skeletal alterations, particularly chin positioning and mandibular angles, were analyzed through cephalometric radiographs.
Patient-Reported Outcomes: Patient satisfaction was evaluated using a 1-to-10 scale, capturing subjective perceptions of facial esthetics and treatment outcomes.
Statistical analyses
Pretreatment and post-treatment measurements were compared using appropriate statistical methods to identify significant changes over the 15-year follow-up period. Descriptive statistics, such as means and standard deviations, were calculated for quantitative variables, while inferential analyses, including paired t-tests or ANOVA, were utilized to assess differences between pre- and post-treatment measurements.
RESULTS
Dental alignment
The mean reduction in crowding postorthodontic treatment was 5.2 mm [Table 1]. Notably, a significant improvement in dental alignment was observed across the study population.

Soft tissue transformations
Post treatment, there were notable advancements in lip positioning and increased lip thickness [Table 2]. On average, lip positioning improved by 3.8 mm, contributing to enhanced facial esthetics.

Skeletal changes
Modest alterations in chin positioning and mandibular angles were noted post treatment [Table 3]. While statistically significant, these changes were relatively modest compared to improvements in dental alignment and soft tissue transformations.

Patient satisfaction
Overall, patients reported high levels of satisfaction with their orthodontic treatment, with an average satisfaction score of 9.2 on a scale of 1 to 10.
Aging process
Throughout the 15-year follow-up, natural aging changes in skin elasticity and fat distribution persisted, albeit independent of orthodontic treatment outcomes. These changes were observed consistently across the study population.
The results demonstrate significant improvements in dental alignment and soft tissue transformations post orthodontic treatment. While modest skeletal changes were noted, patients reported high levels of satisfaction. Moreover, the ongoing aging process highlights the need for continued monitoring and patient education regarding realistic treatment expectations.
DISCUSSION
The findings of this longitudinal study provide valuable insights into the long-term effects of orthodontic treatment on facial esthetics and aging. The significant reduction in crowding and improvements in lip positioning and thickness post treatment align with previous research demonstrating the efficacy of orthodontic interventions in enhancing facial esthetics.[1,2] These improvements not only contribute to a harmonious smile but also have broader implications for self-esteem and social interactions.[3]
While modest alterations in chin positioning and mandibular angles were observed, they were not as pronounced as improvements in dental alignment and soft tissue transformations. This is consistent with the notion that orthodontic treatment primarily targets dental discrepancies rather than skeletal anomalies.[4] However, even subtle changes in skeletal morphology can have perceptible effects on facial esthetics and should be considered in treatment planning.
The high levels of patient satisfaction reported in this study underscore the success of orthodontic treatment in meeting patient expectations. This aligns with previous literature emphasizing the psychological benefits of orthodontic interventions, including improved self-confidence and quality of life.[5]
The persistence of natural aging changes in skin elasticity and fat distribution underscores the dynamic nature of facial esthetics. While orthodontic treatment can address dental and soft tissue concerns, it cannot halt the aging process entirely. Patients should be educated about the expected changes associated with aging to maintain realistic treatment expectations.[6]
These findings have important clinical implications for orthodontic practice. Practitioners should consider not only short-term treatment outcomes but also long-term facial esthetic changes and aging effects when planning and discussing treatment with patients. Incorporating patient-centered approaches and managing expectations can enhance treatment satisfaction and overall patient experience.[7]
This study is not without limitations, including its retrospective design and reliance on subjective patient-reported outcomes. Future research should employ prospective longitudinal designs with larger sample sizes to further elucidate the complex interplay between orthodontic treatment, facial esthetics, and aging. Additionally, incorporating objective measures of facial esthetics, such as three-dimensional imaging, can provide a more comprehensive understanding of treatment outcomes.
CONCLUSION
In conclusion, this study highlights the enduring effects of orthodontic treatment on facial esthetics over a 15-year period. While significant improvements in dental alignment and soft tissue transformations were observed, modest skeletal changes and the persistence of natural aging effects underscore the dynamic nature of facial esthetics. By considering these long-term effects, practitioners can better tailor treatment plans to meet patient expectations and enhance overall treatment satisfaction.
