ABSTRACT
Systemic lupus erythematosus (SLE) is a complex and multifaceted autoimmune disease that poses a significant diagnostic challenge for healthcare providers due to its diverse clinical manifestations, lack of specific testing, and potential to mimic other conditions. Delayed diagnosis and misdiagnosis of SLE can have serious consequences for patients, impacting their physical and psychological well-being. The physical consequences of delayed diagnosis and misdiagnosis of SLE are significant. Studies have shown that delays in diagnosis can lead to increased organ damage, poorer health outcomes, and increased morbidity and mortality. With a delayed diagnosis or misdiagnosis, patients may also experience increased psychological symptoms, such as depression, anxiety, and lack of trust in clinicians. The median delay from the first symptom to diagnosis in SLE is 47 months, highlighting the need for early recognition and management. Delayed diagnosis can lead to increased disease progression, which can result in irreversible organ damage and long-term complications. This paper investigates the multifaceted consequences of delayed diagnosis and misdiagnosis of SLE, encompassing both physical and psychological impacts. It delves into the underlying reasons behind diagnostic delays and offers comprehensive strategies for healthcare providers to enhance early recognition and management. Through a thorough analysis of medical literature and statistical data, this paper aims to provide physicians with actionable insights to improve diagnostic accuracy and patient outcomes in SLE.

Introduction
Systemic lupus erythematosus (SLE) is a complex autoimmune disease characterized by the production of autoantibodies against self-antigens, resulting in widespread inflammation and tissue damage.[1] It affects multiple organs and systems, leading to a diverse array of symptoms that can vary in severity and presentation. Despite its relatively low prevalence compared to other autoimmune diseases, SLE has a profound impact on patient health and quality of life.[2] Timely diagnosis is essential for initiating appropriate treatment and preventing long-term complications.[3] However, the heterogeneous nature of SLE poses significant challenges in accurate diagnosis, often resulting in delayed diagnosis or misdiagnosis.[4,5] At this time, the average time from onset of symptoms to diagnosis is around 3–5 years.[6,7] In a UK study involving 121 SLE patients, 70% reported receiving an initial misdiagnosis, with an average of 10 consultations with three different doctors needed before an SLE diagnosis was confirmed.[8] The delayed diagnosis of SLE is influenced by various factors, such as the nonspecific nature of symptoms, overlap with other diseases, diagnostic criteria complexity, testing challenges, underestimation of symptoms, lack of awareness, delayed referral patterns, and psychosocial barriers, resulting in prolonged suffering, exacerbation of symptoms, increased risk of organ damage, complications, and poorer prognosis for affected individuals, underscoring the critical need for improved early recognition and management strategies.[7,8]
Methods
A comprehensive literature review was conducted to examine the physical and psychological effects of delayed diagnosis or misdiagnosis of SLE, reasons behind this issue, and proposed improvements in healthcare diagnostics. Sources were identified using PubMed, Consensus, and Perplexity, focusing on articles published within the last 10–20 years to ensure relevance. Keywords included “Systemic Lupus Erythematosus,” “SLE,” “delayed diagnosis,” “misdiagnosis,” “physical effects,” “psychological effects,” and “improving diagnosis.” Inclusion criteria encompassed peer-reviewed articles in English discussing SLE’s diagnostic challenges and strategies for improvement. EndNote software managed citations and sources.
Physical effects of delayed diagnosis and misdiagnosis
Delayed diagnosis and misdiagnosis of SLE can lead to a cascade of detrimental effects on the body, resulting in irreversible organ damage and increased morbidity and mortality rates.[8,9] The systemic nature of SLE, coupled with its potential to affect multiple organs and systems, emphasizes the urgency of timely diagnosis and intervention to mitigate disease progression.[10]
One of the most serious consequences of delayed diagnosis in SLE is renal involvement, which can lead to lupus nephritis and ultimately progress to end-stage renal disease (ESRD).[11] Lupus nephritis is characterized by inflammation of the kidneys, leading to impaired renal function and proteinuria. Studies have shown that delayed diagnosis is associated with a higher risk of renal complications in SLE patients.[8] For example, a study published in 2023 found that patients with delayed diagnosis had a significantly higher prevalence of lupus nephritis compared to those diagnosed promptly.[9] The consequences of untreated lupus nephritis can be severe, including chronic kidney disease, hypertension, and renal failure, necessitating dialysis or kidney transplantation.[11,12]
Delayed diagnosis and misdiagnosis of SLE can also contribute to cardiovascular complications, which are a leading cause of mortality in patients with SLE.[4] The chronic inflammation associated with SLE increases the risk of accelerated atherosclerosis, myocardial infarction, and stroke. Additionally, the presence of antiphospholipid antibodies (aPL) in some patients with SLE can further predispose them to thrombotic events, such as deep vein thrombosis and pulmonary embolism.[9] Studies have shown that delayed diagnosis is associated with a higher incidence of cardiovascular events in SLE patients, highlighting the importance of early recognition and aggressive management of cardiovascular risk factors in this population.[12]
Musculoskeletal symptoms are common in SLE and can range from arthralgia and myalgia to arthritis and avascular necrosis.[4] Delayed diagnosis and misdiagnosis can lead to delays in the initiation of appropriate treatment, resulting in prolonged inflammation and joint damage. Chronic inflammation of the synovium can lead to erosive arthritis, cartilage destruction, and joint deformities, significantly impairing mobility and quality of life.[9] Furthermore, delayed diagnosis may result in delays in addressing secondary musculoskeletal complications, such as osteoporosis and osteonecrosis, which can further exacerbate morbidity and functional impairment in SLE patients.[8]
Skin involvement is common in SLE and can manifest as a wide range of dermatological manifestations, including malar rash, discoid lupus erythematosus (DLE), photosensitivity, and vasculitis.[5] Delayed diagnosis and misdiagnosis can lead to delays in addressing cutaneous manifestations, resulting in prolonged discomfort and cosmetic concerns for patients. Furthermore, untreated cutaneous lupus can progress to more severe forms, such as subacute cutaneous lupus erythematosus (SCLE) or SLE with skin involvement, which may require more aggressive treatment modalities.[8]
Gastrointestinal and pulmonary manifestations are less common but can occur in patients with SLE, leading to significant morbidity and mortality.[4] As with the other systems, delayed diagnosis and misdiagnosis may result in delays in recognizing and addressing these complications, leading to prolonged symptoms and organ damage. Gastrointestinal involvement in SLE can manifest as abdominal pain, nausea, vomiting, and diarrhea, and may be mistaken for other gastrointestinal disorders, such as inflammatory bowel disease or peptic ulcer disease.[9] Pulmonary involvement in SLE can present as pleuritis, pneumonitis, or pulmonary hypertension, and may be misdiagnosed as pneumonia or asthma, leading to delays in appropriate treatment and potential respiratory compromise.[5]
As described, delayed diagnosis and misdiagnosis of SLE can have profound physical consequences for patients, leading to irreversible organ damage and increased morbidity and mortality rates. Renal complications, cardiovascular manifestations, musculoskeletal complications, cutaneous manifestations, and gastrointestinal and pulmonary involvement are among the many potential consequences of delayed diagnosis in SLE patients. Early recognition and aggressive management of SLE are essential to mitigate these complications and improve outcomes for patients.
Psychological effects of delayed diagnosis and misdiagnosis
The psychological impact of delayed diagnosis and misdiagnosis of SLE cannot be underestimated. Patients often experience feelings of frustration, anxiety, and uncertainty as they navigate the diagnostic process and struggle to obtain an accurate diagnosis.[4] Research has shown that patients with delayed diagnosis of SLE are at increased risk of psychological distress and diminished health-related quality of life compared to those diagnosed promptly. A study published in Lupus Science and Medicine found that patients with delayed diagnosis reported higher levels of anxiety and depression, as well as lower overall satisfaction with life.[13]
In addition to the emotional toll on patients, delayed diagnosis and misdiagnosis can also strain patient-provider relationships and erode trust in the healthcare system.[2,6] Many patients report feeling misunderstood or dismissed by healthcare providers during their diagnostic journey, leading to feelings of frustration and alienation.[13] A survey conducted by the Lupus Foundation of America found that a significant proportion of patients had to consult multiple healthcare providers before receiving an accurate diagnosis of SLE, highlighting the challenges in the diagnostic process.[14]
Reasons for delayed diagnosis and misdiagnosis of SLE
Diagnosing systemic diseases poses a significant challenge for healthcare providers due to their diverse symptoms, overlap with other conditions, gradual onset, nonspecific manifestations, and the need for specialized diagnostic tests.[15] With symptoms affecting multiple organs or systems and often presenting gradually over time, systemic diseases can easily be mistaken for more common ailments. Moreover, some symptoms may not immediately suggest a single underlying cause, complicating the diagnostic process. Identifying these diseases requires a comprehensive approach, involving careful consideration of medical history, thorough physical examination, and sometimes invasive or expensive tests. Collaboration among healthcare specialists may also be necessary to ensure accurate diagnosis and effective management of systemic diseases.[16]
Several factors contribute to delayed diagnosis and misdiagnosis of SLE, including the nonspecific nature of its symptoms, lack of awareness among healthcare providers, diagnostic challenges, and patient-related factors. SLE can present with a wide range of symptoms, including fatigue, joint pain, fever, skin rash, and photosensitivity, which overlap with other autoimmune diseases such as rheumatoid arthritis, and non-immune conditions, such as hypertension, generalized anxiety disorder, fibromyalgia, and other more common conditions.[3] As a result, healthcare providers may attribute these symptoms to other causes, leading to delays in diagnosis and misdiagnosis.[17]
Furthermore, the lack of specific diagnostic tests for SLE poses challenges for healthcare providers in distinguishing it from other autoimmune diseases with similar clinical features. The American College of Rheumatology criteria for the classification of SLE require the presence of specific clinical and laboratory findings, but these criteria may not capture early or atypical presentations of the disease, leading to underdiagnosis or misdiagnosis.[15]
Patient-related factors, such as delayed healthcare-seeking behavior, fear of diagnosis, and socioeconomic barriers to healthcare access, also contribute to delayed diagnosis and misdiagnosis of SLE.[8] Many patients with SLE experience symptoms for months or even years before seeking medical attention, either due to the intermittent nature of their symptoms or fear of receiving a diagnosis of a chronic and potentially life-threatening disease.[16] Moreover, socioeconomic factors, such as lack of health insurance, limited access to healthcare services, and language barriers, can impede timely diagnosis and access to appropriate care for patients with SLE.[2,14]
Strategies for combating delayed diagnosis and misdiagnosis
Addressing delayed diagnosis and misdiagnosis of SLE requires a multifaceted approach, involving healthcare providers, patients, advocacy organizations, and policymakers.[5] Healthcare providers should receive comprehensive education and training on the diverse clinical manifestations of SLE and the latest diagnostic criteria and guidelines to improve recognition and diagnosis. Continuing medical education programs and interdisciplinary workshops, involving rheumatologists, dermatologists, nephrologists, and other specialists, can facilitate knowledge sharing and collaboration among healthcare providers.[16]
Implementing standardized screening protocols and clinical decision support tools for patients presenting with suggestive symptoms of SLE can aid in early detection and prompt referral to specialists.[6] The Lupus Foundation of America recommends using validated screening instruments, such as the Lupus Severity Index, to assess the likelihood of SLE in patients with compatible symptoms.[14] Moreover, leveraging electronic health records and telemedicine platforms can streamline the diagnostic process and facilitate timely communication and coordination among healthcare providers, especially in underserved areas with limited access to rheumatology specialists.[13,16]
Empowering patients with knowledge about SLE symptoms, diagnostic tests, and treatment options is crucial for promoting early recognition of the disease and facilitating informed healthcare decision-making.[3] Patient education materials, support groups, and online forums provided by advocacy organizations, such as the Lupus Foundation of America and the Lupus Research Alliance, can serve as valuable resources for patients and their families.[14] Encouraging patients to advocate for themselves and actively participate in shared decision-making with their healthcare providers can help bridge the gap between symptom onset and diagnosis, ultimately improving outcomes for patients with SLE.[13]
In addition to healthcare provider education, there is a critical need to improve patient education and awareness of SLE and autoimmune conditions. Many patients with SLE are unaware of the symptoms of the disease and may delay seeking medical attention until their condition has progressed to a more severe stage. By educating patients about the early signs and symptoms of SLE, as well as the importance of timely diagnosis and treatment, healthcare providers can empower patients to take an active role in their healthcare journey and advocate for their needs.
Advocacy organizations play a vital role in raising awareness about SLE, campaigning for policy changes to improve access to healthcare services, and funding research to advance the understanding and treatment of the disease.[10] By collaborating with government agencies, healthcare systems, and pharmaceutical companies, advocacy organizations can amplify the voices of patients with SLE and drive positive change in the healthcare landscape. Policymakers also have a responsibility to address systemic barriers to timely diagnosis and treatment of SLE, such as disparities in access to care, insurance coverage, and research funding. By prioritizing investments in preventive care, health education, and healthcare infrastructure, policymakers can help reduce the burden of SLE on individuals, families, and society as a whole.[15]
Patient need-to-know information about SLE
Patients play a crucial role in the early detection and management of SLE. Understanding the common symptoms, timeframe of the disease, diagnosing process, and testing can empower patients to seek timely medical attention and advocate for their healthcare needs.
Common symptoms
SLE can present with a wide range of symptoms that vary in severity and frequency. Common symptoms include[5,8]:
Fatigue
Joint pain and stiffness
Fever
Skin rashes, particularly a butterfly-shaped rash across the cheeks and nose
Photosensitivity (sensitivity to sunlight)
Oral ulcers
Raynaud’s phenomenon (color changes in the fingers or toes in response to cold or stress)
Chest pain
Shortness of breath
Hair loss
Kidney problems, such as protein or blood in the urine
Neurological symptoms, such as headaches, seizures, or cognitive dysfunction.
It is important for patients to recognize these symptoms and seek medical attention if they experience any concerning changes in their health.
Timeframe of the disease
SLE is a chronic autoimmune disease characterized by periods of flares and remissions. The course of the disease can vary greatly among individuals, with some experiencing mild symptoms that come and go, while others may have more severe and persistent symptoms.[4] SLE can affect people of all ages, but it most commonly occurs in women of childbearing age. Early diagnosis and appropriate management are crucial for preventing long-term complications and improving outcomes.[5]
Diagnosing process
Diagnosing SLE can be challenging due to its diverse clinical manifestations and the absence of specific diagnostic tests. Healthcare providers typically use a combination of medical history, physical examination, laboratory tests, and imaging studies to diagnose SLE. The American College of Rheumatology has established classification criteria for SLE, which include specific clinical and laboratory findings. These criteria help guide healthcare providers in making a diagnosis of SLE.[17]
During the diagnostic process, patients may undergo blood tests to check for[5]:
Antinuclear antibodies (ANA), which are present in the majority of patients with SLE
Antibodies to specific nuclear antigens, such as anti-double-stranded DNA (dsDNA) antibodies and anti-Smith antibodies
Complete blood count (CBC) to assess for anemia, leukopenia, or thrombocytopenia
Kidney function tests, including urine analysis for protein and blood
Complement levels, such as C3 and C4.
In some cases, imaging studies, such as X-rays, ultrasound, or MRI, may be performed to evaluate organ involvement. Other tests, such as skin biopsy for evaluation of skin rashes or kidney biopsy for assessment of kidney involvement, may also be indicated based on the patient’s symptoms and clinical presentation.
Conclusion
Delayed diagnosis and misdiagnosis of SLE have far-reaching consequences for patients and healthcare providers. From irreversible organ damage and increased morbidity and mortality to emotional distress and financial strain, the toll of delayed diagnosis and misdiagnosis extends beyond physical symptoms to encompass psychological, social, and economic dimensions. Addressing the underlying reasons behind delayed diagnosis and misdiagnosis of SLE and implementing strategies to combat these issues are essential steps in improving outcomes for patients with SLE. By enhancing provider education, promoting multidisciplinary collaboration, empowering patients through education and support, and advocating for policy changes to improve access to healthcare services, we can strive toward earlier detection and effective management of SLE, ultimately improving the quality of life for individuals living with this challenging autoimmune disease. As we continue to learn more about the pathogenesis and clinical manifestations of SLE and explore novel diagnostic and therapeutic approaches, it is imperative that we work together as a global community to raise awareness, reduce stigma, and improve outcomes for patients with SLE, today and in the future.
