Abstract
目的
分析生物学衰老标志物表型年龄和表型年龄加速与心脏瓣膜病的关联性。
方法
从英国生物库2006—2010年纳入的人群中筛选符合纳排标准的研究对象。计算表型年龄和表型年龄加速,运用Cox多因素分析探究表型年龄和表型年龄加速是否是心脏瓣膜病的独立危险因素,并采用剔除缺失值和亚组分析的方法进行敏感性分析。通过受试者操作特征(ROC)曲线比较表型年龄、表型年龄加速对心脏瓣膜病的预测准确性,并绘制基于逻辑回归的临床决策曲线。
结果
本研究共纳入411 687名参与者,其中患有心脏瓣膜病14 258例(中位随访时间12.80年),包括非风湿性主动脉瓣疾病5238例(中位随访时间12.82年)、非风湿性二尖瓣疾病4558例(中位随访时间12.83年)、非风湿性三尖瓣疾病411例(中位随访时间12.84年)等。经人口统计学因素(性别、种族、教育、汤森剥夺指数)、人体测量因素(体重指数)、生活方式因素(吸烟、饮酒、防止高血压的饮食方法评分)、高血压和高脂血症矫正后,表型年龄和表型年龄加速是心脏瓣膜病及其包括的非风湿性主动脉瓣疾病、非风湿性二尖瓣疾病和非风湿性三尖瓣疾病的独立危险因素(表型年龄:校正后风险比均为1.04,P<0.01;表型年龄加速:校正后风险比均为1.03,P<0.01)。敏感性分析也证实了这一点。ROC曲线和临床决策曲线分析结果显示,相比表型年龄加速,表型年龄预测心脏瓣膜病的准确性更高(ROC曲线下面积分别为0.721和0.599)、净获益更高;并且相比单一指标,两种指标联合预测心脏瓣膜病具有更高的准确性(ROC曲线下面积达0.725)和更高的净获益。
结论
生物学衰老标志物表型年龄和表型年龄加速均是心脏瓣膜病的独立危险因素。相比表型年龄加速,表型年龄对于预测心脏瓣膜病更具优势;相比单一指标,两种指标联合可进一步提高预测的准确性和临床价值。

Abstract
Objective
To analyze the association between biological aging markers (phenotypic age and phenotypic age acceleration) and valvular heart diseases.
Methods
Research subjects who met the inclusion and exclusion criteria were selected from the UK Biobank from 2006 to 2010. The phenotypic age and phenotypic age acceleration were calculated. Cox multivariate analysis was used to examine the relationship between the aging markers and valvular heart diseases. Sensitivity analysis was conducted by removing missing values and subgroup analysis. The predictive accuracy of phenotypic age and phenotypic age acceleration for valvular heart diseases was analyzed using receiver operating characteristic (ROC) curves, and a clinical decision curve was generated based on logistic regression.
Results
A total of 411 687 subjects were included in the study, among whom there were 14 258 patients with valvular heart diseases. The overall median follow-up time was 12.80 years, the median follow-up time for patients with non-rheumatic aortic valve diseases (n=5238), non-rheumatic mitral valve diseases (n=4558), and non-rheumatic tricuspid valve diseases (n=411) were 12.82 years, 12.83 years and 12.84 years, respectively. After adjusting for demographic factors (gender, race, education, Townsend deprivation index), anthropometric factors (body mass index), lifestyle factors (smoking, alcohol consumption, Dietary Approaches to Stop Hypertension score), hypertension and hyperlipidemia, Cox multivariate analysis showed phenotypic age and phenotypic age acceleration were independent risk factors for valvular heart diseases, including non-rheumatic aortic valve diseases, non-rheumatic mitral valve diseases, and non-rheumatic tricuspid valve diseases (phenotypic age: corrected HR=1.04, P<0.01; phenotypic age acceleration: corrected HR=1.03, P<0.01), which was also confirmed by sensitivity analysis. ROC curves and clinical decision curves demonstrated that compared with the phenotypic age acceleration, phenotypic age had higher accuracy (the areas and the curves were 0.721 and 0.599) and higher net benefit in predicting valvular heart diseases. Moreover, compared with a single indicator, the combination of the two indicators had higher accuracy (the area under the curve was 0.725) and higher net benefit.
Conclusions
Phenotypic age and phenotypic age acceleration,as markers of biological aging, are independent risk factors for valvular heart diseases. Compared with phenotypic age acceleration, phenotypic age has a greater advantage in predicting valvular heart diseases. Overall, the combination of the two indicators offers a more effective approach for predicting valvular heart diseases.

随着人口老龄化进程的加剧,心脏瓣膜病的全球负担逐渐加重[1-4]。数据显示,心脏瓣膜病影响着5%~10%的65~74岁人群和10%~20%的75岁以上人群,且其患病率仍迅速上升,特别是在高收入国家[1]。医院质量监测系统数据显示,中国2022年出院诊断包含心脏瓣膜病的患者有188.2万人次[4]。心脏瓣膜病是导致心脏功能减退、心力衰竭、心律失常、反复住院和早期死亡的重要原因[5]。考虑到心脏瓣膜病晚期预后不良且现有筛查手段存在漏诊风险,亟须探索更有效的早期筛查策略[5-7]。
衰老作为多种疾病的重要危险因素,其分子机制研究已成为当前热点,探索生物学衰老的标志物更是受到关注[8-9]。如基于九项临床指标和实足年龄构建的表型年龄,对疾病发病率和病死率具有预测价值[10-11]。研究表明,衰老是心脏瓣膜病的重要发病机制[12-14],但目前较少用生物学衰老标志物预测心脏瓣膜病发生,且主要关注主动脉瓣狭窄,如运用双向孟德尔随机化方法探究表观遗传时钟(基于DNA甲基化水平的生物学年龄估算工具)与主动脉瓣狭窄的因果关系[15-16]。生物学衰老标志物能否用于心脏瓣膜病的早期筛查值得探讨。本文利用英国生物样本库资料,分析生物学衰老标志物表型年龄和表型年龄加速对心脏瓣膜病(尤其是非风湿性心脏瓣膜病)的预测能力。
1. 资料与方法
1.1. 资料来源
英国生物样本库(UK Biobank)是英国规模最大的有关致病或预防疾病的基因和环境因子的信息资源库,纳入了2006—2010年约50万名基线年龄为40~69岁参与者的资料[17]。本研究纳入18岁及以上患风湿性或非风湿性主动脉瓣、二尖瓣、三尖瓣、肺动脉瓣和多瓣膜疾病中的任意一种疾病的心脏瓣膜病患者(表1前八种疾病)和未患心脏瓣膜病人群,同时排除:①先天性心脏瓣膜病患者(表1后两种疾病);②计算表型年龄所需信息缺失者;③失访;④入组当天及之前已有心脏瓣膜病者[18-19]。

结果纳入了411 687名参与者的资料,其中14 258例患心脏瓣膜病(中位随访时间12.80年),包括非风湿性主动脉瓣疾病5238例(中位随访时间12.82年)、非风湿性二尖瓣疾病4558例(中位随访时间12.83年)、非风湿性三尖瓣疾病411例(中位随访时间12.84年)、风湿性心脏瓣膜病1579例、其他心脏瓣膜病5601例。由于同一参与者可能同时患多种心脏瓣膜病,因此各种心脏瓣膜病例数之和大于总患病人数。411 687名研究对象的基线特征见表2。

表型年龄加速定义为表型年龄与实足年龄的差值,正值提示生物衰老速度快于预期,负值则提示生物衰老速度慢于预期[11]。
1.3. 协变量选取
本研究的协变量包括人口统计学因素(性别、种族、教育、汤森剥夺指数)、人体测量因素(体重指数)、生活方式因素(吸烟、饮酒、DASH评分)、高血压和高脂血症。其中,种族分为白人、非白人和未知三类,教育分为大学或学院学位、其他和未知三类,吸烟和饮酒均分为以前、当下、从不和未知四类。高血压定义为已确诊原发性或继发性高血压、服用降压药物或者基线收缩压140 mmHg(1 mmHg=0.133 kPa)及以上或舒张压90 mmHg及以上。高脂血症定义为服用降胆固醇药物、三酰甘油1.7 mmol/L及以上、胆固醇5.2 mmol/L及以上或低密度脂蛋白胆固醇3.4 mmol/L及以上。
1.4. 统计学方法
本研究使用R 4.4.1进行数据分析。正态分布的计量资料用均数±标准差( ±s)表示,数据缺失时采用中位数,组间比较采用t检验;非正态分布的计量资料用中位数(上下四分位数)[M(Q 1,Q 3)]表示,数据缺失时用中位数,组间比较采用秩和检验;分类变量采用例数(百分比)[n(%)]表示,缺失数据用“未知”填充,组间比较采用χ 2检验。运用Cox多因素分析探究表型年龄和表型年龄加速是否是心脏瓣膜病的独立危险因素,计算HR值及95%CI,检验水准α=0.0015(0.05/34,Bonferroni校正,进行了34次COX回归)。使用rms包绘制非限制性立方样条图探究表型年龄、表型年龄加速与心脏瓣膜病之间是否存在非线性关系;采用ROC曲线比较表型年龄、表型年龄加速对心脏瓣膜病的预测准确性,并用rmda包绘制基于逻辑回归的临床决策曲线,比较使用表型年龄、表型年龄加速对心脏瓣膜病预测的临床实用性。
本研究使用两种方法进行敏感性分析。①在上述纳排标准基础上,删除协变量数据有缺失值的个体,剩余358 257名参与者,运用Cox多因素分析探究表型年龄和表型年龄加速是否是心脏瓣膜病的独立危险因素。②在411 687名参与者中,按照性别、吸烟状况、有无高血压、有无高脂血症分组,使用Cox多因素分析探究表型年龄和表型年龄加速是否是心脏瓣膜病的独立危险因素,协变量为1.3中除了分组指标以外的协变量。
2. 结果
2.1. 表型年龄和表型年龄加速与心脏瓣膜病发生相关
COX回归分析结果显示,表型年龄和表型年龄加速是心脏瓣膜病及其包括的非风湿性主动脉瓣疾病、非风湿性二尖瓣疾病和非风湿性三尖瓣疾病的独立危险因素(表型年龄:校正后HR均为1.04,P<0.01;表型年龄加速:校正后HR均为1.03,P<0.01),即表型年龄和表型年龄加速每增加1岁,各类心脏瓣膜病发生的风险分别约增加4%和3%,见表3。限制性立方样条图显示,表型年龄大于55岁或表型年龄加速大于0岁时,心脏瓣膜病风险进一步增加(图1)。结果提示,表型年龄和表型年龄加速与心脏瓣膜病的发生相关。

2.2. 表型年龄、表型年龄加速对心脏瓣膜病发生预测的敏感性分析结果
在原有纳排标准基础上,删除协变量数据有缺失值的个体后,剩余358 257名参与者,其中有12 225例患心脏瓣膜病,包括非风湿性主动脉瓣疾病4444例、非风湿性二尖瓣疾病3912例、非风湿性三尖瓣疾病350例。对这部分人群进行分析结果显示,表型年龄和表型年龄加速依然是心脏瓣膜病及其包括的非风湿性主动脉瓣疾病、非风湿性二尖瓣疾病和非风湿性三尖瓣疾病的独立危险因素(表型年龄:校正后HR均为1.04,P<0.01;表型年龄加速:校正后HR均为1.03,P<0.01),见表4。

按照性别、吸烟状况、有无高血压、有无高脂血症分组,在所有亚组中,表型年龄和表型年龄加速均是心脏瓣膜病的独立危险因素。并且,相较于表型年龄加速,表型年龄对心脏瓣膜病发生的预测有微弱的优势(表5)。通过删除缺失值、亚组分析,进一步证明表型年龄和表型年龄加速是心脏瓣膜病的独立危险因素。

2.3. 表型年龄、表型年龄加速对心脏瓣膜病的预测效果
表型年龄和表型年龄加速均有助于预测心脏瓣膜病,AUC分别为0.721和0.599,最佳分类阈值分别为58岁和-0.8岁,其中表型年龄的预测效能更好(P<0.01);两种指标联合可进一步提升预测效能(AUC为0.725)。见图2。临床决策曲线图显示,相比表型年龄加速,表型年龄的净获益更高,即真阳性带来的效益和假阳性导致损失的差值更大,临床实用性更强;两种指标联合可进一步增强临床实用性。见图3。

结果提示,相比表型年龄加速,表型年龄预测心脏瓣膜病的准确性更高、临床实用性更强;相比单一指标,两种指标联合预测心脏瓣膜病具有更高的准确性和更强的临床实用性。
3. 讨论
本研究运用多因素COX回归模型,研究了表型年龄和表型年龄加速与心脏瓣膜病发生的关联性。由于风湿性瓣膜病存在外界感染的影响,单独用生物学年龄这一生物学衰老标志物来预测风湿性瓣膜病无法得到满意的结果,遂对非风湿性瓣膜病中病例数较多的三大类非风湿性主动脉瓣疾病、非风湿性二尖瓣疾病、非风湿性三尖瓣疾病进行亚组分析。结果显示,表型年龄和表型年龄加速是心脏瓣膜病的独立危险因素,该结论在非风湿性主动脉瓣、二尖瓣、三尖瓣疾病中得到了证实。并且,相比表型年龄加速,表型年龄预测心脏瓣膜病的准确性更高、临床实用性更强。相比单一指标,两种指标联合预测心脏瓣膜病具有更高的准确性和更强的临床实用性。
有学者提出衰老的12个特征,包括表观遗传改变、细胞衰老等[23]。在表观遗传方面,DNA甲基化、非编码调控RNA、组蛋白修饰和染色质重塑等均可影响主动脉瓣疾病[24-25]。在细胞衰老方面,随着衰老,机械应力增加诱导内皮剥脱,同时内皮祖细胞再生能力减弱,导致病理性衰老内皮细胞积聚在瓣膜附近[12-13]。衰老内皮细胞又分泌物质影响细胞外基质、瓣叶硬度等,从而导致心脏瓣膜病的发生[12-13]。本文资料显示,表型年龄和表型年龄加速每增加1岁,心脏瓣膜病发生风险分别增加约4%和3%,其中表型年龄预测心脏瓣膜病的AUC达到0.721,决策曲线也提示其预测心脏瓣膜病的净获益更高,临床实用性更强。表型年龄的计算包括C反应蛋白、白细胞计数等炎症指标,而已有研究表明,机械或生化损伤会导致慢性炎症,从而使瓣膜间质细胞激活为炎性肌成纤维细胞-成骨-瓣膜间质细胞,促进瓣膜的纤维化和钙化[26]。此外,表型年龄的计算还应考虑血肌酐指标,研究发现肾功能障碍可以导致炎症细胞和血浆中高度浓缩的分子如氧化低密度脂蛋白进入主动脉瓣,从而导致瓣膜钙化[27]。可见基于表型年龄或表型年龄加速开发心脏瓣膜病预测或早期筛查的工具是可行的。
年轻人和老年人可能有相同的表型年龄,而两者的生物学衰老速度是不同的。一般来说,表型年龄加速(生物学年龄与实足年龄的差值)可以更直观地展示生物学衰老速度。但在本研究中,相比表型年龄加速,表型年龄对心脏瓣膜病发生的预测有微弱优势,具体原因还需要进一步探索。本研究也发现表型年龄、表型年龄加速指标联合对心脏瓣膜病的预测效能、临床实用性优于单独指标,这提示联合运用表型年龄、表型年龄加速两种指标对心脏瓣膜病进行预测更合适。
值得注意的是,衰老是多种疾病的重要危险因素[9],单一使用表型年龄或表型年龄加速预测心脏瓣膜病的针对性不强。在表型年龄或表型年龄加速基础上,联用一种与心脏瓣膜病直接关联度更强的检查方法来预测心脏瓣膜病具有一定的可行性,如结合其他生化指标、多模态影像如CT血管造影和超声心动图的瓣口面积、血液流速等。此外,心脏瓣膜病的发生还可能与遗传学因素有关,所以预测心脏瓣膜病发生还需要考虑基因等其他因素[28]。因此,开发一种心脏瓣膜病发生的预测方法需要综合考虑与心脏瓣膜病发生相关的各种因素,以用于心脏瓣膜病的早期筛查、疾病风险分层,以及指导心脏瓣膜病的预防。
本研究仍存在一些局限性。首先,研究对象可能存在局限性,如研究对象年龄普遍较大、白种人占比大、患有高血压或高脂血症的人数较多。而且,英国生物样本库里的人群不能代表一般人群,在人口统计学、生活方式等方面缺乏普遍性[29-30]。之后需要在来自不同种族、地域的人群中进一步验证结论。其次,本研究考虑的混杂因素可能不完整,如未纳入与心脏瓣膜病密切相关的脂蛋白A[31-32]。最后,本研究仅关注基线的生物学衰老指标,忽略了从基线到最终发生心脏瓣膜病过程中生物学衰老速度的动态变化。
综上,本研究证明了表型年龄和表型年龄加速是心脏瓣膜病的独立危险因素。表型年龄和表型年龄加速作为生物学衰老相关指标,综合考虑了炎症、肾功能、血糖等各方面指标,且易获得计算指标,因此具有临床实用性。未来,将表型年龄和表型年龄加速与传统危险因素如吸烟和高血压、影像学如超声心动图等相结合,可能可以进一步改善心脏瓣膜病的早期筛查和预测。
