The originally published version of this manuscript contained errors in the graphical abstract, which had prevalence in females to be 13.1% for stages 1-5, and 6.4% for stages 3-5, and for males 13.2% for stages 1-5 and 7.5% for stages 3-5. These numbers were the wrong way round. The graphical abstract has been corrected to read: prevalence in females, 13.2% for stages 1-5, 7.5% for stages 3-5, and for males 13.1% for stages 1-5 and 6.4% for stages 3-5.
