Review of Chungcheong Region’s Community-based Mid-to-long-term Plans for Chronic Disease: Focusing on the 8th Regional Health Care Plan

Abstract

Long-term strategies are necessary to address the prevention and management of chronic diseases due to the aging population in the Chungcheong region. Upon reviewing the Regional Healthcare Plan by municipality, major chronic diseases were categorized into hypertension, diabetes, cerebrovascular diseases, infectious diseases, mental illnesses, and cancer. It was noted that most of these are interconnected with regional projects. To achieve sustainable operations for public health centers, systematic standardization of regional projects that are feasible for the public health center’s operation, efficient streamlining of overlapping initiatives, and integration of existing programs need to be pursued.

Introduction

In 2019, 14.9% of the total Korean population were aged 65 years or older, and this percentage is projected to increase to 20.3% by 2025 and 46.5% by 2067 [1]. Particularly, the Chungcheong Province, excluding Sejong, is expected to become a super-aged society in 2025 [2]. The rate of population aging in the Chungcheong Province is much higher than in other metropolitan cities and provinces, and the proportion of the older adult population in this province is markedly high as well [3]. Managing chronic diseases in an aging society is a pressing policy issue that needs to be addressed. However, the treatment rate for chronic diseases in the Chungcheong Province, specifically in Chungnam, Chungbuk, and Daejeon, excluding Sejong, is very low, contributing to an increasing disparity in chronic disease treatment within the province [4-6].

As of 2021, 79.6% of deaths in the Republic of Korea (ROK) were caused by chronic diseases, and healthcare expenditure due to chronic diseases in the country accounted for 85.0% (71 trillion KRW) of all healthcare expenditure in 2020 [7]. Managing chronic diseases should focus on a continuous control and management approach, as opposed to a purely treatment-oriented one. The high prevalence, low control rates, and poor treatment continuation of chronic diseases could severely undermine health recovery during an epidemic, as chronic diseases act as determinants of complications and death [8]. In particular, the regulation of blood pressure or blood glucose requires continued contact with a healthcare provider [9].

In 2019, the Ministry of Health and Welfare launched the Chronic Disease Management in Primary Care Pilot Project, a comprehensive chronic disease management model that features the strengths of a chronic disease management fee project and a community-based primary care pilot project. However, the Chronic Disease Management in Primary Care Pilot Project was designed so that healthcare providers can be reimbursed only when they comply with specific criteria, such as the number of services provided. Thus, certain issues have emerged, including patients who fail to adapt to or have resistance to it being lost to follow-up and a low guidance compliance rate, highlighting the need for evidence-based strategies for patient management [10].

A major reason for poor chronic disease management in primary care in the ROK is the lack of self-care or little to no support for the utilization of community resources. Furthermore, the fees designated for counseling and education essential for chronic disease management are extremely low, while the fees for tests and treatment are relatively higher, leading to a neglect of self-care [11]. Currently, there is no comprehensive chronic disease management strategy in the Chungcheong Province. Therefore, given the high disease burden and mortality associated with chronic diseases, it is crucial to assess the current status of chronic disease management and develop strategies to promote it in the Chungcheong Province.

Methods

To identify strategies to promote chronic disease management in the ROK, the agency reviewed the eighth community healthcare plan, including the Hypertension and Diabetes Registry Project, Clinic-level Chronic Disease Management System, Community Primary Care Pilot Project, and Chronic Disease Management in Primary Care Project, in the following 35 si‧gun, and gu regions: Daejeon Metropolitan City, five regional gu regions (Dong-gu, Jung-gu, Seo-gu, Yuseong-gu, and Daedeok-gu), Sejong Special Self-Governing City, 15 si and gun areas in Chungcheongnam-do (Cheonan-si, Gongju-si, Boryeong-si, Asan-si, Seosan-si, Nonsan-si, Gyeryong-si, Dangjin-si, Geumsan-gun, Buyeo-gun, Seocheon-gun, Cheongyang-gun, Hongseong-gun, Yesan-gun, and Taean-gun), and 11 si and gun areas in Chungcheongbuk-do (Cheongju-si, Chungju-si, Jecheon-si, Boeun-gun, Okcheon-gun, Yeongdong-gun, Jincheon-gun, Goesan-gun, Eumseong-gun, Danyang-gun, and Jeungpyeong-gun).

Results

The major chronic disease management projects in each region encompass initiatives for hypertension, diabetes, cardiovascular and cerebrovascular diseases, communicable (respiratory) diseases, mental illnesses, and cancer. In Chungnam, diverse projects aim at preventing chronic diseases, including those focusing on hypertension/diabetes and cardiovascular/cerebrovascular diseases, such as individuals accruing mileage through the walking app “Geotju” and other projects to improve healthcare access among vulnerable populations, including healthy lifestyle, comprehensive health promotion, and smoking cessation projects; mobile healthcare; and patient registration and monitoring using the public health information system. Other projects and support currently underway in the region include a cardiovascular/cerebrovascular disease prevention project, support for complication screening fees, and post-discharge referral service (Table 1).

In Chungnam, all local governments have planned national vaccination promotion as part of their communicable or respiratory disease projects. Depending on the region, there are plans for encouraging education and screening for sexually transmitted infections (HIV infection/AIDS), support for treatment costs, mobile screening for tuberculosis and treatment fee support for close contacts, mobile clinics for Hansen’s disease, and support for medical and living expenses. For mental illnesses, early screening and prevention programs are offered at dementia support centers. For cancer, patients are referred to national cancer screening and medical treatment fee coverage through the National Cancer Center (Table 2).

In Chungbuk, chronic disease prevention and hypertension/diabetes management projects include comprehensive health promotion and home health management projects to promote a healthy lifestyle as well as a hypertension/diabetes registry. Furthermore, the cardiovascular/cerebrovascular prevention and management project involves the provision of consultations to high-risk groups and registered patients, the launch of the Red Circle Campaign in communities, and education promotion projects such as Knowing My Blood Vessel Numbers (Table 3).

In terms of communicable (respiratory) diseases in Chungbuk, a national vaccination campaign and monitoring program as well as tuberculosis screening have been planned in all regions, and there are other projects planned in certain regions, including sexually transmitted disease management and education; early screening, prevention, and management at dementia support centers; the Dementia Awareness Project, national cancer screening at the National Cancer Center; and referral to the cancer treatment fee support project (Table 4).

In Daejeon Metropolitan City and Sejong Special Self- Governing City, chronic disease prevention and hypertension/diabetes projects primarily include a healthy lifestyle, comprehensive health promotion, and smoking cessation projects, and the cities have launched or are planning to implement a hypertension/diabetes registry project. Furthermore, the cities are focusing on the education (e.g., the Red Circle Campaign and Knowing My Blood Vessel Numbers) and promotion of cardiovascular/cerebrovascular disease prevention and management (Table 5). Additionally, the local governments in these cities plan to implement the national vaccination campaign. In particular, most local governments in Daejeon plan to implement a tuberculosis-related project; projects for early screening, prevention, and management of dementia and the Dementia Awareness Project through dementia support centers; and national cancer screening and referral to cancer treatment fee support projects through the National Cancer Center (Table 6).

Discussion

The results of the review of the 8th regional health and medical plan for chronic disease-related projects in the Chungcheong Province (Chungnam, Chungbuk, Daejeon, Sejong) were reviewed at the si‧gun, and gu levels and are summarized as follows. First, communities in this region are promoting the prevention and management of hypertension and diabetes through various healthy lifestyle-facilitating activities as part of comprehensive health promotion projects targeting hypertension and diabetes. Furthermore, they identify and register high-risk groups and vulnerable groups through the public health information system and refer individuals to home health management projects and the hypertension/diabetes registry project at public health centers. Second, the communities promote cardiovascular/cerebrovascular prevention management project and provide education about them and implement systems for emergency department transport and post-discharge management through regional cardiovascular disease, trauma, and emergency medical centers. Third, for respiratory disorders and communicable diseases, the communities implement national vaccination campaigns (e.g., vaccination against influenza and Streptococcus pneumoniae for older adults) as well as sexually transmitted disease management, tuberculosis patient and contact management, and livelihood support for patients with Hansen’s disease through the regional infectious disease management support foundation. Fourth, for mental disorders, the communities provide early screening, prevention, and management of dementia; treatment fee support; and awareness projects through regional dementia centers (dementia support centers). Fifth, for cancer, the communities implement the national cancer screening project, cancer treatment fee support, and comprehensive support for cancer survivors and promote self-help groups for hospice and community-dwelling cancer patients through the National Cancer Center and regional cancer centers. Overall, compared with other regions, the Chungnam region has stronger high-risk patient registration and monitoring for hypertension and diabetes via the use of regional healthcare information systems and mobile healthcare projects. On the other hand, the Chungbuk region focuses on the hypertension/diabetes registry project, and Daejeon and Sejong focus on comprehensive health promotion projects.

For cardiovascular/cerebrovascular diseases, all regions within the Chungcheong area implement education campaigns such as the Red Circle Campaign and Knowing My Blood Vessel Numbers as part of the cardiovascular/cerebrovascular disease prevention and management project. Chungnam provides relatively more complication screening fee support through regional cardiovascular centers, while Chungbuk has more emergency department transport services through regional trauma centers. Regarding communicable diseases, all regions within the Chungcheong area implement the national vaccination campaign (for influenza) and tuberculosis management and screening projects. For mental disorders, all regions within the Chungcheong area implement early dementia screening, prevention, and management through dementia support centers. Chungnam focuses relatively more on caregiver support and the supply of supportive and self-care products, while Chungbuk and Daejeon focus relatively more on the Dementia Awareness Project. With regard to cancer, most regions within the Chungcheong area have implemented the national cancer screening project and cancer treatment fee support project through the National Cancer Center.

Recently, the Chungcheong Regional Centers for Disease Control and Prevention hosted the “2023 Expert Forum for Narrowing the Health Disparities in the Chungcheong Region” and presented the current status and performances of the “Status and Strategy of Health Disparities in Chronic Diseases in the Chungcheong Region” and “Development of Mid- to Long-Term Strategies for the Management of Chronic Diseases in the Chungcheong Region.” Efforts are being made to discuss and address health issues related to chronic diseases by establishing region-specific chronic disease management strategies in the Chungcheong Province. In addition to these efforts, this study reviewed local government-level strategies that have been established according to regional characteristics through links to regional-level chronic disease projects in the Chungcheong Province. For these strategies to be effectively and feasibly implemented by public health centers in each community, systematic standardization based on guidelines for regional-level projects is needed, and it is important to promote sustainable operations of the projects by optimizing overlapping projects and linking to existing projects.

지역사회기반 충청권 만성질환 중장기 전략 고찰: 제8기 지역보건의료계획 중심으로

Abstract

충청권 인구 고령화에 따른 만성질환 예방 및 관리에 대한 중장기적인 전략이 필요하다. 이를 위해 시‧군‧구별 지역보건의료계획을 검토한 결과, 주요 만성질환은 고혈압‧당뇨병, 심뇌혈관질환, 감염성질환, 정신질환, 암질환으로 구분되었고, 대부분 권역 단위 사업과 연계되어 있는 것을 확인하였다. 이는 보건소의 실현 가능한 운영을 위한 권역 단위 사업의 체계적인 표준화, 중복사업의 효율화 및 기존사업의 연계 등을 통해 지속 가능한 운영을 추구해야 한다.

서 론

2019년 65세 이상 고령자는 14.9%에 해당하며, 2025년에는 20.3%, 2067년에는 46.5%까지 증가할 것으로 예상된다[1]. 특히 세종을 제외한 충청권은 2025년을 기점으로 초고령화 사회에 진입할 예정이다[2]. 충청권의 고령화는 타 광역시‧도에 비해 상당히 빠른 수준이며, 현재 충청권 고령인구 비중도 상당히 높은 권역에 속한다[3].

고령화 사회에 따른 만성질환 관리는 우리 사회가 직면한 주요 정책과제이며, 충청권의 세종시를 제외한 충남, 충북, 대전에서는 낮은 만성질환 치료율을 보이며, 충청권 내 만성질환 치료 격차가 심화되는 것으로 나타난다[4-6].

우리나라의 만성질환으로 인한 사망은 2021년 기준 전체 사망의 79.6%를 차지하며, 우리나라 만성질환으로 인한 진료비는 2020년 기준 71조 원으로 전체 진료비의 85.0%를 차지하고 있다[7].

만성질환은 치료의 개념이 아닌 지속적인 관리와 조절의 개념으로 접근해야 하는 질환으로, 높은 유병률과 낮은 조절률, 관리 지속성이 미흡하기에 미래 감염병 유행 시 건강 회복력에 치명적인 부담을 줄 수 있는 요인으로 사망 및 합병증 부담을 결정하게 된다[8]. 특히 혈압이나 혈당의 조절은 지속적인 의료 제공자와의 접촉을 통한 관리가 필요하다[9].

보건복지부는 2019년에 만성질환관리 수가 시범사업과 지역사회 일차의료 시범사업 등의 장점을 반영한 만성질환 통합 관리 모형인 일차의료 만성질환 관리 시범사업을 출범하였다. 하지만 일차의료 만성질환 관리 시범사업의 경우, 사업모형이 횟수 등의 실적을 준수하여야만 수가를 보상받는 방식으로 설계됨에 따라 IT기술에 비적응하거나 거부감이 있는 환자가 중도 탈락하는 일이 발생되었고, 지침의 준수율이 저조한 한계점 또한 노출되어 환자관리전략으로써 더욱 근거기반 전략마련의 필요성이 제기되고 있다[10].

우리나라 일차의료가 만성질환관리에 미흡한 이유로, 가장 큰 원인은 만성질환관리에 꼭 필요한 자기관리 또는 지역사회 자원 활용에 대한 지원이 없거나 매우 낮아 자기관리와 지역사회 자원 활용이 제대로 이루어지지 못하고 있으며 만성질환관리에 꼭 필요한 내용에 대한 상담이나 교육 수가는 현저히 적고, 검사와 치료에 대한 수가는 상대적으로 높아 자기관리가 등한시되고 있기 때문이다[11].

충청권의 종합적인 만성질환 관리 전략은 부재한 상황이며, 만성질환으로 인한 사망과 질병 부담이 높은 상황에서 충청권의 만성질환 관리를 위한 현황 파악 및 전략의 필요성이 제기되고 있다.

방 법

국내 만성질환 관리를 위한 전략을 파악하기 위해 고혈압당뇨병 등록관리사업, 의원급 만성질환관리제, 지역사회 일차의료 시범사업, 일차의료 만성질환 관리사업 등을 포함한 지역사회 기반의 만성질환 관리 계획을 파악하고자 대전광역시를 비롯한 총 5개 지역구(동구, 중구, 서구, 유성구, 대덕구)와 세종특별자치시, 충청남도 포함 15개 시‧군지역(천안시, 공주시, 보령시, 아산시, 서산시, 논산시, 계룡시, 당진시, 금산군, 부여군, 서천군, 청양군, 홍성군, 예산군, 태안군), 충청북도를 포함한 11개 시‧군지역(청주시, 충주시, 제천시, 보은군, 옥천군, 영동군, 진천군, 괴산군, 음성군, 단양군, 증평군) 총 35개 시‧군‧구 지자체의 제8기 지역보건의료계획을 검토 및 정리하였다.

결 과

지역보건의료계획에 따른 지역별 주요 만성질환 관리사업은 고혈압‧당뇨병, 심뇌혈관질환, 감염성(호흡기)질환, 정신질환, 암질환으로 구분되며, 충남의 만성질환 예방 및 고혈압‧당뇨병, 심뇌혈관질환에 대한 사업으로는 걷기APP “걷쥬”를 통한 마일리지 적립이 가능하며, 건강생활실천과 통합건강증진사업, 금연지원사업, 모바일헬스케어, 지역보건의료정보시스템(public health information system, PHIS)을 활용한 환자 등록 및 모니터링, 취약계층 의료 접근성 제고를 위해 노력중이며 심뇌혈관질환예방관리사업과 합병증 검진비 지원, 퇴원환자 사후관리 서비스 연계 등을 추진 중으로 나타났다(표 1).

충남에서는 감염성 혹은 호흡기 질환 사업의 일환으로 모든 지자체에서 국가예방접종 홍보 및 독려를 계획하였고, 지역에 따라 성매개감염병(HIV/AIDS)을 위한 교육 및 검진 독려, 진료비 지원과 결핵 이동검진 및 접촉자치료비 지원, 한센병의 이동진료, 진료비 및 생계비 지원을 계획하였다. 정신질환은 치매안심센터를 통해 조기검진과 예방관리 프로그램 운영을 하고 있으며, 암은 국립암센터를 통한 국가암검진, 암환자 의료비 지원을 대부분 연계하고 있다(표 2).

충북의 만성질환 예방 및 고혈압‧당뇨병과 관련된 사업은 건강생활실천을 위한 통합건강증진사업과 방문건강관리사업, 고혈압‧당뇨병등록관리사업 중심으로 계획되어 있으며, 심뇌혈관질환예방관리사업을 통해 고위험군 및 환자의 등록을 통한 상담과 지역 내 레드서클 캠페인, 자기혈관 숫자 알기 등의 교육 및 홍보를 중심으로 나타났다(표 3).

충북의 감염성(호흡기)질환은 국가예방접종 독려 및 모니터링과 결핵 검진사업을 모든 지역에서 계획되었고, 지역에 따른 성매개감염병관리 및 교육과 치매안심센터의 조기검진 및 예방관리와 지역 내 치매인식 개선사업, 국립암센터의 국가암검진사업, 암환자 의료비 지원사업연계를 주로 계획하고 있다(표 4).

대전광역시 및 세종특별자치시의 만성질환 예방과 고혈압‧당뇨병 관련 사업은 건강생활실천과 통합건강증진사업, 금연지원사업이 대다수였고, 고혈압‧당뇨병등록관리사업을 운영 및 계획하고 있다. 또한 심뇌혈관질환예방관리사업의 교육(레드서클캠페인, 자기혈관 숫자 알기 등) 및 홍보를 주로 계획하고 있다(표 5).

대전광역시와 세종특별자치시의 경우, 모든 지자체에서 국가예방접종사업을 계획하였고 특히 대전의 경우, 결핵 관련 사업과 치매안심센터를 통한 조기검진, 예방관리, 치매인식 개선사업, 국립암센터와 연계된 국가암검진사업과 암환자 의료비 지원사업을 대부분 지자체에서 계획하고 있다(표 6).

논 의

충청권(충남, 충북, 대전, 세종) 시‧도별, 시‧군‧구별제 8차 지역보건의료계획 만성질환 관련 사업을 검토한 결과는 다음과 같다.

첫째, 고혈압‧당뇨병 관련 통합건강증진사업의 다양한 건강생활실천 독려를 통해 고혈압‧당뇨병의 예방‧관리를 실시하고 있으며, 지역보건의료정보시스템(PHIS)을 통한 고위험군 및 취약계층 발굴‧등록과 방문건강관리사업, 보건소 고혈압‧당뇨등록관리사업을 연계하고 있다.

둘째, 심뇌혈관질환 관련 심뇌혈관질환예방관리사업을 통한 교육 및 홍보와 권역심뇌혈관질환센터, 권역외상센터, 권역응급의료센터와 연계된 응급실 이송 및 퇴원환자 사후관리 등을 운영하고 있다.

셋째, 호흡기질환 및 감염성질환 관련 국가예방접종(인플루엔자, 어르신 폐렴구균 등)과 권역감염병관리지원단과 연계된 성매개감염병 관리, 결핵환자 및 접촉자관리, 한센병 생계비 지원 등을 운영하고 있다.

넷째, 정신질환 관련 광역치매센터(치매안심센터)와 연계된 조기검진, 예방관리, 치료비 지원, 인식개선 사업 등을 운영하고 있다.

다섯째, 암 관련 국립암센터 및 권역단위 지역암센터와 연계된 국가암검진사업, 암환자 의료비 지원사업, 암생존자통합지지, 호스피스, 재가암환자 자조모임 등을 운영하고 있다.

지역사회 기반 충청권 만성질환 관리사업 검토의 종합적인 결과는, 고혈압‧당뇨병 관련 충남에서 타권역에 비해 지역보건의료정보시스템과 모바일헬스케어사업을 통한 고위험군 및 환자 등록, 모니터링을 강화하고 있으며, 충북은 고혈압‧당뇨등록관리사업, 대전과 세종은 통합건강증진사업을 집중적으로 운영하고 있다.

심뇌혈관질환 관련 충청권 전권역에서 심뇌혈관질환예방관리사업의 일환으로 레드서클 캠페인, 자기 혈관 숫자 알기 등 교육을 실시하고 있다. 충남은 권역심뇌혈관센터의 합병증 검진비, 충북은 권역외상센터 연계를 통한 응급실 이송을 비교적 많이 운영하고 있다.

감염성 질환 관련하여 충청권 전권역에서 국가예방접종(인플루엔자) 독려와 결핵관리 검진사업을 운영하고 있다.

정신질환 관련하여 충청권 전권역에서 치매안심센터와 연계된 조기검진, 예방관리를 운영하고 있다. 충남은 보호자 및 조호물품 지원, 충북과 대전은 치매인식 개선사업을 비교적 많이 운영하고 있다.

암 관련하여 충청권 전권역에서는 국립암센터의 국가암검진사업과 암환자 의료비 지원사업을 대부분 운영하고 있다.

최근 충청권질병대응센터 ‘2023년 충청권 건강격차 해소방안 마련을 위한 전문가 포럼’을 통하여 「충청권 만성질환 건강격차 현황과 전략」, 「충청권 만성질환 관리를 위한 중장기 전략 개발」 관련 현황 및 성과를 공유하며, 충청권 지역 맞춤형 만성질환 관리 전략 마련을 통한 만성질환 관련 건강문제 해소를 위한 논의를 진행하는 등의 노력을 진행중이다.

이러한 노력과 더불어 본 연구에서는 충청권 만성질환 관련 권역 단위 사업의 연계를 통한 지자체 단위의 지역 특성에 맞게 수립된 전략들을 검토하였는데, 이러한 전략들을 각 지자체 보건소에서 효율적이고 실현 가능하도록 운영하기 위해 권역 단위 사업에 대한 지침 기반의 체계적인 표준화 작업이 선행되어야 하며, 중복사업 효율화 및 기존사업의 연계 등을 통해 지속 가능한 운영을 추구해야 할 것이다.
