In the January 2026 issue of Baylor University Medical Center Proceedings, Tisekar et al provide valuable insight into the rare pathology of leiomyosarcoma of the prostate (LMS).1 Previous articles have reported cases and brief overviews of the pathology2–4 but have not provided an adequate understanding of the pathophysiology nor options for treatment. This paper pools data from numerous case reports in an attempt to help remediate the lack of knowledge of LMS of the prostate.
As the authors noted, most cases of LMS of the prostate are not recognized until after the pathology diagnosis. Most patients present with typical benign prostate hyperplasia symptoms, and as such are treated for that disease or for possible prostate cancer evaluation. Furthermore, the ideal treatment is not clear due to the lack of cases, and thus various therapies have been administered, including surgery, radiation, and chemotherapy individually and in combination.
This analysis provides further understanding into this rare tumor. The authors note the inverse relationship between prostate size and the aggressive nature of LMS, similar to that of prostate cancer. This work presents additional prognostic features that will surely encourage future discussion and guidance into LMS treatment going forward.
