A 73-year-old woman presented with a progressively enlarging translucent iris lesion in her left eye 1 year after uneventful phacoemulsification. Visual acuity was 0.0 logMAR. Slitlamp biomicroscopy revealed a well-circumscribed, trilobulated cyst in the inferior iris quadrant (Figure 1, top left). Anterior segment optical coherence tomography demonstrated a cystic structure (8.1 × 4.9 × 3.5 mm) with hyperreflective walls compressing the corneal endothelium (Figure 1, right). The cyst was surgically excised by dissecting it from the iris through an inferior cleavage plane and removing it with revolution scissors and forceps. Histopathologic examination confirmed a conjunctival epithelial inclusion cyst (Figure 1, bottom left).1,2
