Importance of Case Studies in Clinical Practice, Clinical Research, and Clinical Education

Case studies are essential to the advancement of clinical knowledge, particularly for rare, rapidly evolving, or diagnostically challenging conditions. They allow clinicians to document atypical presentations and share experiential knowledge. In clinical practice, case reports can enhance pattern recognition, inform differential diagnosis, and guide clinical decision-making when evidence is limited. Well-described case studies can generate hypotheses, expand the understanding of disease mechanisms, and highlight gaps in knowledge, thereby spurring new research.

Case studies are also a cornerstone of clinical education because they allow educators to connect theoretical knowledge with real-world patient presentations. By presenting complex authentic scenarios, case studies can help students develop clinical reasoning skills applicable to both routine patient encounters and rare conditions.

In this issue of Journal of the American Academy of Audiology (JAAA), Behr & Honaker (2025) present a case report that highlights an atypical early presentation of paraneoplastic cerebellar degeneration (PCD), which is a rare autoimmune neurological disorder most commonly associated with anti-Purkinje cell cytoplasmic antibody type-1 (anti-Yo) that leads to irreversible cerebellar damage, and the critical role of vestibular testing in identifying central pathology. Although PCD typically manifests with subacute cerebellar signs such as ataxia and dysarthria, early symptoms may mimic acute vestibular disorders, thereby delaying the diagnosis.

In this case, the authors found that vestibular findings, including down-beating nystagmus and abnormal video head impulse test results, served as important early indicators of PCD. The authors stated the following: “Given the rapid disease progression of PCD, it is crucial for clinicians conducting vestibular testing to understand signs of central nervous system involvement and refer patients promptly for neurological evaluation.”

This case highlights a relatively rare disorder; however, audiologists may play a crucial role in its timely diagnostics. Clinicians should be aware of such cases, and clinical researchers should continue pursuing innovative methods to identify central nervous system issues in patients who present with dizziness.

The report by Behr & Honaker (2025) also serves as the first case study published in JAAA that includes clinical discussion questions. These questions are designed to stimulate critical thinking and encourage discussions and analyses among readers. We hope that these discussion questions can be used in the classroom and clinical settings to help engage readers in analyzing real-world scenarios and considering future strategies and research directions to move the field of audiology forward.
