基于FAERS数据库的8种免疫检查点抑制剂心血管不良事件分析

Analysis of cardiovascular adverse events associated with 8 immune checkpoint inhibitors based on the FAERS database

Abstract

Objective

Immune checkpoint inhibitors (ICIs) are increasingly used in cancer therapy; however, systematic epidemiological studies of ICI-associated cardiovascular adverse events in China remain limited. This study aims to analyze cardiovascular adverse event signals associated with ICIs approved by the Food and Drug Administration (FDA), thereby providing evidence to support safer clinical use.

Methods

Data were obtained from the Food and Drug Administration Adverse Event Reporting System (FAERS) database from the first quarter of 2011 to the fourth quarter of 2024 (56 quarters over 14 years). Adverse event reports were screened using “primary suspected drugs” as the inclusion criterion. Eight ICIs were included: atezolizumab, avelumab, durvalumab, nivolumab, pembrolizumab, cemiplimab, ipilimumab, and tremelimumab. Adverse events were first classified according to system organ class (SOC), and reports involving cardiac and vascular disorders were extracted. Signal detection was subsequently performed at both the standardized MedDRA query (SMQ) level and preferred terms (PT) level. The Bayesian confidence propagation neural network (BCPNN) method was applied to calculate the information component ( IC) and its 95% confidence interval ( CI) for each target drug-adverse event pair. Positive signals of immune-related cardiovascular adverse events (irCVEs) were defined as reports ≥3 and a lower limit of the 95% CI of IC ( IC 025) >0.

Results

A total of 12 890 cardiac adverse event reports and 8 543 vascular adverse event reports were identified. Among all cardiovascular adverse event reports, the top three drugs were nivolumab, pembrolizumab, and atezolizumab. Male patients accounted for a higher proportion than female patients, and patients aged over 65 years represented more than 50.00% of reports. From 2011 to 2024, the number of cardiovascular adverse event reports associated with the 8 ICIs showed a rapid upward trend. The proportions of fatal outcomes were 30.74% for cardiac adverse events and 20.83% for vascular adverse events. At the SMQ level, positive cardiac adverse event signals involved 6 SMQ codes, whereas positive vascular adverse events signals involved 8 SMQ codes. Non-infectious myocarditis/pericarditis was the only strong positive signal shared by all 8 ICIs, followed by thrombophlebitis, vasculitis, and cardiac arrhythmias. At the PT level, 47 positive cardiac adverse event signals and 36 positive vascular adverse event signals were detected. Strong positive cardiovascular adverse event signals included myocarditis, immune-mediated myocarditis, autoimmune myocarditis, pericardial effusion, cardiac tamponade, malignant pericarditis, embolism, and thrombophlebitis migrans, etc.

Conclusion

Cardiovascular adverse events associated with ICIs in the FAERS database commonly include myocarditis, pericarditis, arrhythmias, and vasculitis, with relatively high proportions of severe outcomes and mortality. Therefore, pre-treatment risk screening, close monitoring during therapy, and multidisciplinary management of cardiovascular adverse events are essential to ensure patient safety.

近年来免疫检查点抑制剂(immune checkpoint inhibitors,ICIs),包括程序性细胞死亡蛋白-1(programmed cell death protein-1,PD-1)抑制剂、程序性死亡受体配体1(programmed death-ligand 1,PD-L1)抑制剂和细胞毒性T淋巴细胞相关抗原4(cytotoxic T lymphocyte-associated antigen-4,CTLA-4)抑制剂,已被批准用于多种肿瘤的治疗,部分已作为一线方案,显著改善了肿瘤患者的预后。然而,ICIs可累及多个器官或系统并引起免疫相关不良事件(immune-related adverse events,irAEs),其临床重要性日益受到关注 [ 1- 3]。

免疫相关心血管不良事件(immune-related cardiovascular adverse events,irCVEs)的临床表现多样,包括心肌炎(可表现为暴发性心肌炎合并心源性休克,也可为轻症)、心包炎、心律失常(如心脏传导阻滞)、心力衰竭、血栓栓塞及血管炎等 [ 4- 5]。Andres等 [ 6]开展的单中心队列研究报道,irCVEs的发生率为3.4%,其中ICIs相关心肌炎是最常见的irCVEs(33例,占37%)。另一项真实世界研究 [ 7]显示,高达10%的接受ICIs治疗的患者发生过主要心血管不良事件(major adverse cardiovascular events,MACEs),尤其存在于合并心力衰竭或瓣膜病史的患者中。需要指出的是,MACEs的定义与irCVEs不尽相同,但两者均提示ICIs相关心血管毒性不容忽视。尽管部分irCVEs 相对少见,但可严重影响肿瘤患者的预后及生活质量,具有重要的临床研究价值 [ 6- 8]。目前,国内尚未见ICIs相关irCVEs的流行病学数据报道。国外虽有基于美国食品药品监督管理局不良事件报告系统(Food and Drug Administration Adverse Event Reporting System,FAERS)的研究,但数据仅更新至2020年。随着ICIs在中国临床的广泛应用及新型ICIs不断上市,临床医师(尤其是基层医师)亟需全面了解irCVEs的最新变化趋势。持续追踪ICIs相关irCVEs的流行病学特征具有重要意义。本研究基于FAERS数据库,分析8种ICIs在2011至2024年期间的irCVEs数据,旨在为中国肿瘤患者安全使用ICIs提供参考。

1. 资料与方法

1.1. 数据来源

本研究数据来源于FAERS,时间范围为2011年第1季度至2024年第4季度(共14年,56个季度)。

1.2. 方法

所有报告按照美国食品药品监督管理局(Food and Drug Administration,FDA)推荐的方法进行重复报告的删除:提取报告编号、患者编号和FDA接收报告日期,并依据上述字段进行排序;对于相同患者编号的记录,保留FDA接收报告日期最新的条目;对于相同的患者编号和FDA接收报告日期,我们保留报告编号最大的报告。此外,从2019年第1季度开始,FAERS每季度数据包都包含1份“删除报告列表”,依据该列表中的患者编号删除相应报告,并按照FDA建议的流程进行重复数据的清理。

本研究采用MedDRA(Medical Dictionary for Regulatory Activities)(v27.1)进行不良事件编码。标准化MedDRA查询(standardized MedDRA query,SMQ)采用狭义搜索策略。药物不良事件报告以“首要怀疑药物”为筛选依据,纳入以下8种ICIs:阿替利珠单抗、阿维鲁单抗、度伐利尤单抗、纳武利尤单抗、帕博利珠单抗、西米普利单抗、伊匹木单抗及曲美木单抗。首先,基于器官系统分类(organ system classification,SOC)对不良事件进行归类,筛选出涉及心脏和血管系统的不良事件报告;然后,在SMQ层级和首选术语(preferred terms,PT)层级进行信号检测。严重心血管不良事件(以下简称“严重事件”)包括需要干预或者住院,危及生命,导致残疾、畸形和死亡,以及其他重要的医学事件。数据提取及筛选流程见 图1。

1.3. 统计学处理及信号判定标准

采用SAS 9.4软件提取数据,代入贝叶斯置信区间递进神经网络法(Bayesian confidence propagation neural network,BCPNN)计算每个“目标药物-目标不良事件”组合的信息成分(information component, IC)值及其95% CI,从而量化目标药物与目标不良事件之间的统计关联强度。计算公式见 式(1)和 式(2)。

式中, a为目标药物目标不良事件报告数, b为目标药物其他不良事件报告数, c为其他药物目标不良事件报告数, d为其他药物其他不良事件报告数, EIC为 IC的期望值, VIC为 IC的方差值。

依据下列标准,筛选各ICIs的irCVEs阳性信号:目标药物-目标不良事件组合的报告数≥3,且 IC的95% CI下限(即 IC 025)>0。

2. 结 果

2.1. ICIs心血管不良事件报告整体特征

从2011年第1季度至2024年第4季度,以8种ICIs为首要怀疑药物,共挖掘出心脏不良事件报告12 890例次,血管不良事件报告8 543例次。心血管不良事件占所有ICIs不良事件的4.79%(21 433/447 560)。在心血管不良事件报告总例次中,排名前3的药物依次为纳武利尤单抗、帕博利珠单抗、阿替利珠单抗( 图1)。

在8种ICIs心血管不良事件报告中,男性患者多于女性( 图2A);45岁以上患者的占比为76.75%,65岁以上患者占比大于50.00%( 图2B);北美洲(32.78%)、亚洲(32.51%)和欧洲(30.35%)的患者占比明显高于非洲、大洋洲等( 图2C);上报者中医师、药师及其他健康专业人员三者占比超过80.00%( 图2D);需要住院治疗的心血管不良事件报告为10 475例次,死亡4 771例。2011至2024年间,8种ICIs心血管不良事件报告数量呈快速增长趋势( 图3)。

2.2. 基于 SOC的 ICIs心血管不良事件报告分析结果

心脏不良事件报告和血管不良事件报告中的患者中位年龄均大于65岁。8种ICIs的心脏严重事件占比(93.75%~99.59%)均高于血管严重事件占比(83.33%~97.00%)。心脏不良事件、血管不良事件的总体死亡结局占比分别为30.74%、20.83%,8种ICIs心脏不良事件的死亡结局占比(24.29%~62.50%)均高于血管不良事件的死亡结局占比(13.56%~25.07%)。除伊匹木单抗、度伐利尤单抗外,其他6种ICIs的心脏不良事件中位用药天数大于血管不良事件中位用药天数。在SOC层面,阿替利珠单抗、阿维鲁单抗心脏不良事件及血管不良事件信号皆为阳性;伊匹木单抗、曲美木单抗心脏不良事件及血管不良事件信号皆为阴性;纳武利尤单抗、帕博利珠单抗、西米普利单抗、度伐利尤单抗心脏不良事件信号皆为阳性( 表1)。

2.3. 基于 SMQ的 ICIs心血管不良事件报告分析结果

心脏不良事件阳性信号涉及6个SMQ编码,血管不良事件阳性信号涉及8个SMQ编码(图 4、5)。心血管不良事件阳性信号数量最多的是PD-L1抑制剂:阿替利珠单抗(12个)、阿维鲁单抗(10个)、度伐利尤单抗(8个)。其次为PD-1抑制剂:纳武利尤单抗(8个)、帕博利珠单抗(7个)、西米普利单抗(4个)。CTLA-4抑制剂信号最少:伊匹木单抗(7个)、曲美木单抗(1个)。非感染性心肌炎/心包炎是8种ICIs唯一共有的强烈阳性信号,其次为血栓性静脉炎、血管炎及心律失常等,不同的ICIs信号程度不一。

2.4. 基于 PT的 ICIs心血管不良事件报告分析结果

心脏不良事件阳性信号涉及47个PT,血管不良事件阳性信号涉及36个PT。PD-1抑制剂帕博利珠单抗和纳武利尤单抗涉及心血管不良事件阳性PT数最多,其次为PD-L1抑制剂阿替利珠单抗和度伐利尤单抗,CTLA-4抑制剂曲美木单抗和伊匹木单抗阳性信号较少(表 2、 3)。

心肌不良事件阳性信号比较强的PT有心肌炎、免疫介导性心肌炎及自身免疫性心肌炎等;心包不良事件阳性信号比较强的PT有心包积液、心脏压塞及恶性心包炎等;其他心脏不良事件阳性信号比较强的PT有完全性房室传导阻滞、心脏炎及心功能失调等。血管不良事件阳性信号较强的PT有栓塞、移动性血栓静脉炎及上腔静脉综合征等(表 2、 3)。

3. 讨 论

从上报特征看,2011至2024年间,随着各ICIs相继上市,其引发的irCVEs报告数量呈逐年上升趋势。一方面,该类药物新品种不断获批上市,阿替利珠单抗、阿维鲁单抗、度伐利尤单抗、纳武利尤单抗、帕博利珠单抗、西米普利单抗、伊匹木单抗及曲美木单抗分别于2016年、2017年、2017年、2014年、2014年、2018年、2011年及2022年上市,其中曲美木单抗上市时间最晚,报告数最少;另一方面,其适应证不断增加,应用范围逐渐扩大。超过80.0%的报告由专业人员上报,表明本研究数据具有较高的可靠性。来自北美洲、亚洲和欧洲的报告占比超过95%,提示ICIs在这些地区应用广泛。男性报告例数多于女性。在不良事件潜伏期方面,ICIs引发的irCVEs大多发生在治疗早期(14~42 d),说明在这个高风险窗口期间密切监测的重要性。8种ICIs心血管不良事件患者的中位年龄均为65岁以上,这可能与年龄是癌症和心血管疾病的共同重要风险因素有关 [ 9- 10]。另一项基于国际药物警戒数据库VigiBase的ICIs研究 [ 11]结果显示,发生irAEs的患者中男性占比(61.7%)较高,年龄为(64.3±12.6)岁,与本研究结果基本一致。

心血管不良事件占所有ICIs不良事件的4.79%。从PT层级看,PD-1抑制剂的心血管不良事件阳性信号最多,PD-L1抑制剂次之,CTLA-4抑制剂最少。尽管在SOC层级,CTLA-4抑制剂心血管不良事件信号均为阴性,但在SMQ层级及其下属的PT层级仍出现一定数量的阳性信号,这与MedDRA不同层级所覆盖的疾病数量差异有关。本研究结果显示,ICIs导致的心血管不良事件在心脏方面主要累及心肌、心包、脉管及传导系统,以心肌炎、心包炎、心律失常、心力衰竭为主,临床表现可从轻微的胸痛、突发无力、心悸、呼吸困难、头晕或晕厥,进展至心源性休克、危及生命的心律失常及传导障碍等。在血管方面则以高血压、各种栓塞、缺血或者闭塞、血管炎、动脉炎、血管功能障碍、出血及渗出等为主。本研究基于上市后的真实世界数据验证了ICIs心血管不良事件阳性信号结果,其信号谱与目前的研究 [ 12]基本吻合。

ICIs导致心血管不良事件的病理生理机制可能如下:首先,老年患者的心血管系统存在不同程度的功能退化,血管更容易遭受炎症攻击 [ 10];其次,ICIs在激活抗肿瘤性T细胞的同时,也激活了自身免疫反应性T细胞,其对免疫稳态的干扰导致T细胞对心肌或血管内皮中的自身抗原产生异常反应,炎症细胞因子如肿瘤坏死因子-α、γ干扰素、白细胞介素(interleukin,IL)-1β、IL-6和IL-17上调,进而引起心脏功能障碍和血管内皮功能障碍,增加心血管风险。Hardy等 [ 13]报道,纳武利尤单抗联合伊匹木单抗治疗后,患者出现三度房室传导阻滞及急性致死性心肌炎,组织病理学检查结果显示,CD3 + T细胞(CD4 +多于CD8 +)和CD68 +巨噬细胞占主导地位,偶见CD20 + B细胞。帕博利珠单抗导致的自身免疫性心肌炎伴急性心力衰竭患者的心肌活检组织学结果显示,淋巴细胞浸润,CD8 + T细胞占优势,FOXP3阳性,调节T细胞减少 [ 14]。另有临床研究 [ 15]显示:组织保护性PD1/PD-L1检查点的抑制可诱导血管发生炎性免疫反应,产生效应T细胞及多种细胞因子(如γ干扰素、IL-17和IL-21),CD4 + T细胞和巨噬细胞在受累动脉壁形成机化的肉芽肿性病变,并通过内膜快速增生和管腔闭塞引起器官缺血性损伤,从而导致血管炎症、栓塞、中膜损伤、渗漏,以及出血和血压升高等。本研究结果显示,在心脏毒性中,所有的ICIs均与心肌炎相关,5种ICIs与临床急症心脏压塞相关;在血管毒性中,6种ICIs与栓塞相关,巨细胞动脉炎是常见的动脉炎,严重时可导致中风或失明,阿替利珠单抗、纳武利尤单抗、帕博利珠单抗和伊匹木单抗均与巨细胞动脉炎相关。

本研究显示:不管在SMQ层级还是PT层级,不同的ICIs引发心血管不良事件的阳性信号有差异。PD-L1抑制剂和PD-1抑制剂的心血管不良事件阳性信号强于、多于CTLA-4抑制剂。除药物剂量导致的数据偏倚之外,可能与其作用机制和作用靶点有关:CTLA-4抑制剂在免疫应答的启动阶段发挥作用,通过耗竭肿瘤部位的调节性T细胞,最终以增加肿瘤内效应T细胞/调节T细胞的比例而发挥作用,作用相对温和;而PD-L1抑制剂和PD-1抑制剂直接作用于免疫应答的效应阶段,直接活化效应T细胞,作用相对强而快。

性别、高龄、既往的抗肿瘤治疗、心血管基础疾病、已知的自身免疫性疾病、患者自身遗传因素及社会因素等均是ICIs心血管不良事件的风险因素 [ 16- 17]。根据国际心脏肿瘤学会的建议,有必要加强临床医师的医学教育,明确ICIs治疗期间可能遇到的各种心血管不良事件的相关表现;治疗前根据肌钙蛋白、心肌激酶同工酶、脑钠肽,以及基线心电图、超声心动图、胸部CT和心脏MRI等进行危险因素分层;治疗期间进行长期的随访,结合其他炎症反应标志物(CRP、ESR)、血清补体检查及时鉴别发现、早期干预管理ICIs心血管不良事件;逐步建立ICIs心血管不良事件的诊断标准、开发风险评估工具、识别需要采取预防措施的高危患者,这些措施有望改善患者的生活质量和医疗体验 [ 17- 19]。更重要的是,当患者疑似出现ICIs心血管不良事件时,心脏科和肿瘤科需要积极评估是否存在其他心血管不良事件,并共同商讨是否继续或停止ICIs及其他抗肿瘤治疗,因为存在多个ICIs心血管不良事件时,发生合并症及死亡的风险更高。

本研究的创新点在于首次从更长时间跨度(14年)分析FAERS中ICIs心血管不良事件,采用国际通用的BCPNN筛选阳性信号,得到的结果可能更客观、更符合实际情况,有利于中国医师对肿瘤患者使用ICIs过程中可能引发的心血管不良事件给予更多的关注与思考,促进合理使用ICIs。同时,本研究也存在一定的局限性。第一,FAERS是一个全球自发报告系统,并非所有不良事件都被收集,因此无法估算ICIs的真实发生率。第二,通过统计学处理得到的阳性信号可能存在一定的数据偏倚,比例失衡法计算得到的ICIs心血管不良事件信号只能证明统计学上的关联,而非生物学上的因果关系。此外,部分单抗(如曲美木单抗和阿维鲁单抗)上市时间晚,报告数较少,用药时间信息缺失,可能导致中位用药时间的统计结果出现数据偏倚,需要上市后的前瞻性研究来进一步验证。第三,FAERS自身信息有限,无法精确评判因果关系,也无法评估其他心血管不良事件或损害(如甲状腺炎间接引发的心血管不良事件)。第四,本研究以“首要怀疑药物”为筛选依据,未能充分考虑合并用药的影响。期待中国心脏肿瘤学者未来开展前瞻性研究,对比抗肿瘤靶向药物或化疗药物与ICIs联合使用时心血管不良事件的发生率,或进行不同联合治疗方案下心血管不良事件发生率的亚组对比分析。
