Abstract
代谢相关脂肪性肝病(MAFLD)已成为中国最常见的慢性肝病,其自然病程及进展风险因素尚缺乏全面梳理。文章综述整合了中国MAFLD队列研究,评估了其自然史特征与关键风险因素。MAFLD人群在3~6年的肝纤维化进展率为19.2%~38.4%,6~11年的肝细胞癌发生率为0~2.7%,4.6~10年的全因死亡率为2.0%~12.0%;糖尿病和血糖异常是疾病进展的强有力预测因子,此外,超重与肥胖、高血压、年龄亦为风险因素。文章提示中国特定人群中MAFLD可呈快速进展,多个风险因素共同驱动病程。未来需大规模前瞻性研究以明确机制并优化干预策略。

Abstract
Metabolic associated fatty liver disease (MAFLD) has become the most common chronic liver disease in China, yet a comprehensive review of the natural history of disease progression and its risk factors is still lacking. This review evaluates integrated natural history characteristics and key risk factors from Chinese cohort studies of MAFLD. In the MAFLD cohorts, the incidence rate of liver fibrosis progression was 19.2%-38.4% over 3-6 years. The hepatocellular carcinoma incidence rate was 0-2.7% over 6-11 years. The all-cause mortality rate was 2.0%-12.0% over 4.6-10 years. Diabetes and abnormal blood glucose were strong predictors of disease progression, and overweight and obesity, hypertension, and age were other risk factors. The article suggests that MAFLD can progress rapidly in certain Chinese cohorts, with multiple risk factors driving the course of the disease. Future large-scale prospective studies are necessary to clarify the mechanisms and optimize intervention strategies.

代谢相关脂肪性肝病(metabolic associated fatty/steatotic liver disease,MAFLD/MASLD)已成为全球常见的慢性肝病[1]。近年来,随着对其代谢本质的认识不断加深,国际权威组织相继将“非酒精性脂肪性肝病(non-alcoholic fatty liver disease,NAFLD)”更新为“MASLD/MAFLD”[2-4],以更准确反映疾病属性与管理理念的变化。尽管不同诊断标准存在细微差异,但学界内普遍认为不同标准覆盖的人群差异有限,更名的过渡阶段也都可相互通用,本文将统一采用MAFLD描述。MAFLD涵盖从单纯性肝脂肪变性到进展性病变的一系列肝脏疾病,包括代谢相关脂肪性肝炎(metabolic associated steatohepatitis,MASH)、肝纤维化、肝硬化及肝细胞癌(hepatocellular carcinoma,HCC)。最新数据显示,MAFLD影响约30.1%的成年人群,中国MAFLD患病率在过去二十年间快速上升,系统评价显示,中国MAFLD患病率已由2008—2010年的25.4%上升至2015—2018年的32.3%[5]。自1990年以来,随着乙型肝炎防控力度强化,中国MAFLD相关肝硬化负担所占比重显著上升[6]。另一方面,MAFLD的自然病程具有高度异质性:部分患者多年病情稳定,另有部分患者则可快速进展至严重的疾病阶段甚至危及生命。其进展受遗传、代谢、生活方式及环境等多重因素影响。胰岛素抵抗、肥胖、血脂异常及高血压是其主要驱动因素[1]。
目前,多个国际研究探讨了MAFLD自然史及风险因素,但主要基于西方人群。近年来国内虽也有相关临床和流行病学研究,但数据分散,存在定义不一、设计差异及随访不足等问题。因此,亟须整合中国人群证据以探究中国人群中MAFLD疾病负担、进展模式及特异风险因素。基于此,本文采用范围综述,总结了中国MAFLD自然病程相关研究,重点关注疾病进展及关键风险因素。
一、. 文献的收集、整理与筛选
系统检索2012年1月1日至2024年1月17日PubMed、荷兰医学文摘(EMBASE)、考克兰(Cochrane)图书馆、中国知网(CNKI)、万方数据库和维普数据库,目标为中国人群中MAFLD自然病程或风险因素的研究。纳入诊断符合MAFLD(含NAFLD、MASLD)基线特征、随访观察肝纤维化、肝硬化或HCC发生情况及相关风险因素、报告效应量及其置信区间的队列研究。排除文献:重复发表中非最完整或非最新的研究、横断面研究、病例报告、综述、干预性研究、动物实验及体外研究、诊断不明确或未区分MAFLD与其他肝病、未报告关键结局指标(如肝硬化发生率、HCC发病率、全因死亡率)、数据不完整(如未提供随访时间、样本量或统计学方法)或无法获取全文的研究、非中国人群或研究人群地域不明的研究。
共检索到相关文献27 881篇,其中中、英文数据库27 407篇,会议摘要474篇。去重后共16 293篇进入筛选,通过标题和摘要剔除15 274篇无关文献后,对553篇全文进行评估,最终39篇符合纳入标准(图1),其中35篇报道了MAFLD自然病程[7-41],21项风险因素分析[9-13, 15, 17, 20, 23, 27-28, 33, 35-36, 39-45]。

二、. 中国MAFLD人群疾病进展相关研究的分布
图2所示为中国MAFLD人群自然病程进展路径分布,涵盖从早期肝脏代谢异常到严重肝脏疾病及多种临床结局,共涉及18类终点,反映了不同阶段的关注程度与证据强度。

在肝脏病程方面,MASH演变、肝纤维化进展、肝硬化及肝功能失代偿均有研究支持。HCC是报道最多的终点,提示在中国MAFLD相关结局中HCC证据较突出。此外,亦涉及肝衰竭、肝移植及其他肿瘤相关风险。除肝脏结局外,MAFLD还与肝外系统风险密切相关,尤其是心血管事件,进一步强调MAFLD作为全身性代谢疾病的重要性。死亡相关结局同样突出,全因死亡和肝脏相关病死报道较多,心血管疾病病死亦被关注。少数研究采用复合终点(如“死亡或HCC”)衡量疾病进展。总体而言,所研究MAFLD自然病程覆盖了从早期病理变化到终末期并发症的多个阶段,其中肝硬化、HCC和死亡相关证据最为充分(图2)。
三、. 自然病程相关研究的基本特征
纳入的35项自然病程研究共覆盖247 502名受试者[7-41],来自中国四大区域的10个省级行政区,其中17项研究来自华东地区,11项研究来自华南地区,6项研究来自华北地区,1项研究来自西南地区,研究对象涵盖不同临床场景及亚组人群,研究时间跨度为2012—2023年,研究数目总体呈逐年增加趋势,反映出临床对MAFLD关注程度持续上升(表1)。早期研究多为小样本住院队列,通常聚焦于未分类MAFLD患者或合并糖尿病者;近年研究逐步依托临床试验注册数据,或关注特定表型,如合并乙型、丙型肝炎,肥胖、2型糖尿病及MAFLD的不同疾病阶段等。研究对象平均年龄主要取决于其所属临床亚群,通常分布在40~60岁。值得注意的是,纳入研究中,女性占比差异较大,范围为10.4%至76.9%,反映出不同研究的纳入标准及特定人群MAFLD患病率差异。一些大型临床试验注册研究未报告年龄或性别信息,限制了跨研究的基线特征比较。

四、. 一般人群的MAFLD自然病程
表2总结了在中国一般人群(即不区分有无特殊合并症)中MAFLD自然病程的研究情况,涵盖疾病的进展路径、重大肝脏结局、肝外并发症及死亡风险。

在纤维化进展方面,3项研究随访了3至6年,经肝活组织检查或无创指标评估肝纤维化进展率分别为27.45%[7]、38.39%[15]和19.20%[33],提示MAFLD人群不可忽视肝纤维化进展风险。肝活组织检查证实的MASH诊断进展仅有1项研究报道[7],3年发生率为19.61%。肝硬化作为MAFLD重要结局,在5项随访4~11年的研究中,肝硬化发生率差异显著,分别为0[21]、0.06%[30]、1.0%[8]、2.0%[12]和5.25%[20]。7项研究报告了HCC结局:一项大样本及长期随访人群中(10 580例,9.9年),HCC发生率为2.30%[20];Lee等[12]和Zhang等[27]则分别报道为2.73%和2.0%。而在较短随访或小样本研究中[18,21],HCC发生率介于0~0.27%之间。Zhang等[27]在纳入了12 486例患者,随访11年的研究报道了肝功能失代偿相关事件,其中腹水1.1%、肝性脑病0.4%、肝肾综合征0.1%、食管胃静脉曲张出血0.1%、自发性细菌性腹膜炎0.1%,复合终点发生率为3.6%,11年随访中无肝移植发生。Lin等[20]在纳入了10 524例患者,随访9.9年的研究报道了包含以上5项复合终点的发生率为1.56%。另有4项研究报告肝脏相关事件的复合终点发生率自1.0%至10.3%[25,27,35,37]。
多项研究证实,MAFLD患者多伴发较高的心血管系统相关疾病发生率。高血压患病率(随访10~11年)为36.76%~39.66%[21];颈动脉斑块检出率在3项研究中均超过34%[28,36,39];杨小艳等[14](随访4.6年)研究报道,非致死性心肌梗死发生率为9.5%,冠状动脉重建率为15.5%。关于肿瘤结局;2项大规模研究报道了在10年随访期内,肿瘤总体发生率分别为4.28%和4.02%[18,32]。死亡结局方面,3项研究报道了肝脏相关死亡,随访6.6~11年的发生率为0.3%~1.11%[9,27,31];4项研究提及心血管相关死亡,随访4~6.6年的发生率从0.44%至7.0%[8-9,11,14]。其他原因死亡在随访5.9~6.6年发生率为0.22%至3.89%[9,11]。4项研究报道了全因死亡,Lin等[31]报道的较大规模的队列研究(随访10年,30 943例患者)病死率为7.0%,其他研究在随访4.6~6.6年中的发生率为2.0%~12.0%[9,11,14]。
五、. MAFLD进展风险因素相关研究的基本特征
本综述共纳入了21项队列研究(总样本量66 143例)用于MAFLD风险因素的分析[9-13, 15, 17, 20, 23, 27-28, 33, 35-36, 39-45]。其中13项研究来自我国华东地区,4项研究来自华南地区,4项研究来自华北地区,研究样本量差异显著,从少于100人的小型特定亚组,到超过18 000人的大型队列。研究时间跨度超过10年,近期研究更倾向于基于更广泛人群的分析。参与者大多为中年人,平均或中位年龄通常介于40至60岁。不同队列中女性比例差异较大,差异范围在不足15%至占比在75%以上(特别是肌肉减少症型MAFLD队列)。尽管如此,大多数未加限定纳入标准的MAFLD队列中性别分布较为均衡。部分研究按临床指标分层(如MAFLD纤维化评分或骨密度三分位),但由于人群构成和方法学的异质性,不同研究之间难以进行直接比较。
六、. MAFLD疾病进展的代谢性风险因素
一般人群代谢类风险因素的常见结果见表3。在所有风险领域中,代谢是研究提示最频繁且与MAFLD疾病进展一致相关的影响因素。

糖尿病和血糖异常是疾病进展严重程度的影响因素。多项研究表明,糖尿病和餐后血糖水平与肝纤维化进展和肝功能失代偿显著相关。虽然糖尿病与肝硬化、HCC关联度在不同研究中存在差异,但其作为MAFLD患者心血管事件(风险很大程度上可由颈动脉斑块的形成直接体现)的稳定预测因素则得到了持续证实,如高糖化血红蛋白为其独立危险因素。
超重与肥胖[通常以身体质量指数(body mass index,BMI)评估]的结果不尽一致。现有研究未发现体质量相关因素与肝纤维化进展的明确关系。但在心血管结局方面指出,正常或偏低体质量(相较于超重/肥胖)与超重/肥胖均在至少一项研究中被认为是重要风险因素。
高血压与MAFLD结局的关系较为复杂。整体来看,血压与MAFLD进展无显著关联。然而,在分层分析中,正常高血压及确诊高血压均被证实是肝纤维化进展和颈动脉粥样硬化的显著预测因素。高血压与肝硬化、HCC或死亡率之间则未发现显著关系。现有证据未显示血脂异常(以综合指标或单项脂质参数评估)与MAFLD进展或HCC的显著关联。代谢性共病的相互作用也会影响结局。例如,糖尿病与肥胖共存显著增加肝纤维化和心血管疾病的风险。同样,胰岛素抵抗合并外周动脉疾病也与纤维化进展相关。
七、. MAFLD疾病进展的其他风险因素
多项研究显示,年龄与肝纤维化、肝硬化、HCC及心血管事件相关,是MAFLD进展的关键人口学因素。男性性别的作用不一致,仅一项研究提示其与肝硬化和失代偿风险升高相关[20]。生物化学指标关联性总体有限[9,12,28,36]:丙氨酸转氨酶和γ-谷氨酰转移酶并非可靠的预测因子,较高水平的白蛋白在部分研究中与肝硬化、HCC的较低风险相关;总胆红素和尿素在分层分析中显示出对晚期肝病有一定预测价值。无创指标如脂肪肝指数和非酒精性脂肪肝纤维化评分等在预测肝纤维化和死亡方面表现良好[9,17,28,36]。病毒性肝炎相关结果不一,丙型肝炎病毒感染与纤维化进展的关系更为一致[36,40],而乙型肝炎病毒感染在部分研究中显示出对颈动脉粥样硬化具有保护作用[28,40]。肌肉减少症被认为是肝纤维化和心血管事件的重要风险因素[39,45]。其他变量(如血小板计数、外周动脉疾病等)证据仍较有限。
八、. 总结与展望
本综述汇总了中国人群中MAFLD自然病程及风险因素的最新证据。部分MAFLD患者会随着时间推移以较高速度进展至高级别纤维化、肝硬化甚至HCC。既往一项大型队列研究提示MAFLD患者5年内全因死亡率为12.4%,对照组在相似的随访时间中则为7.7%[46],本研究所纳入文献所示全因死亡率与之相近,其中2项研究提供的对照组全因死亡率略低于MAFLD组[11,14],但因样本量较小,难以阐明差异。而肝硬化患者死亡风险更高,在一项包含4 000余例MAFLD合并肝硬化患者的研究中报告了高达17.9/100患者年的全因死亡率[17]。尽管全球证据强调心血管疾病(CVD)是MAFLD患者的主要死因,中国仅有两项小样本研究报告CVD特异性死亡,突显肝外结局研究的不足。
风险因素分析指出,代谢综合征成分是疾病进展的重要预测因子[47]。其中糖尿病相关报道最为频繁,且与肝纤维化进展、肝功能失代偿和心血管事件均显著相关,进一步提示其在疾病演变中的核心作用。尽管体质量相关因素在多项研究中被提及,其对肝脏结局的预测价值却并不一致。其与肝纤维化进展的关系因未达到统计学意义而尚无法被证实,而在心血管结局方面甚至还出现了矛盾的结果。这可能与东亚人群较易发生中心性肥胖这一脂肪分布特点有关。有研究显示,即使BMI较低,中心性脂肪堆积仍显著增加代谢与肝脏风险,而腰臀比等更直接表征中心性肥胖的指标则能为MAFLD的风险分层提供更加精准的信息[48]。但本研究发现,此前国内MAFLD领域研究普遍仅以BMI评估肥胖,缺乏对脂肪分布的分析。这一缺口值得关注,提示未来研究应纳入更能反映中心性肥胖的指标(如腰围、腰臀比、腰高比等)以改进风险评估。高血压和血脂异常也在部分研究中显示出与MAFLD患者的纤维化进展或动脉粥样硬化形成存在关联,再一次印证了MAFLD是全身代谢性疾病的肝脏表现,其疾病进展和结局均与多种代谢性疾病存在关联,尤其是与糖尿病关系最为密切。
总体来看,中国MAFLD自然病程与全球趋势基本一致:即从脂肪变性逐步发展为纤维化、肝硬化,最终可能进展为HCC及肝相关死亡。这一演变过程与国际研究和综述中报道的结果相一致[49]。同时,中国人群中也存在一些特有的疾病进展特征及风险调节因素,这可能与遗传易感性、较低BMI下的较高内脏脂肪负担,以及近年来饮食模式快速向高脂高糖转变等因素有关[47,50]。
本综述系统评估了中国人群中MAFLD自然病程及风险因素,整合了中国证据,并涵盖了社区与医院人群,增强了结果的普适性。同时梳理了疾病进展路径和风险因素,勾勒出中国MAFLD流行趋势的变化图谱。此外,本综述纳入的人群特征、诊断方法和结局定义方面存在异质性,随访时间较短,部分人群信息不完整,且多数研究基于住院人群,可能高估疾病严重程度。
从临床角度看,本综述强调了对MAFLD快速进展人群进行早期识别的重要性。无创性纤维化评估工具(如瞬时弹性成像、纤维化4因子评分)应纳入常规,尤其是对于有2型糖尿病等代谢风险因素的患者及老年个体。BMI正常但有向心性肥胖者亦需警惕。公共卫生层面,应在中国国情下开展生活方式干预及在较低BMI阈值下进行代谢筛查,并推动多学科协作,整合肝病、内分泌与心血管专科资源。
根据中华医学会肝病学分会颁布的2024年版指南[4],临床在诊断和评估脂肪性肝病时,应主动识别混合病因(如乙型肝炎病毒感染和饮酒),避免将患者简单归类为“单纯代谢病因”。指南明确指出:酒精性肝病和其他原因脂肪肝患者有肥胖和/或2型糖尿病、代谢综合征时需考虑合并存在MAFLD;慢性乙型肝炎、慢性丙型肝炎等其他类型慢性肝病患者经常合并MAFLD,我国以慢性乙型肝炎合并MAFLD最常见。当混合病因存在时,其病理机制更为复杂、疾病进展可能更快。一项多中心肝活组织检查研究显示,慢性乙型肝炎患者的MAFLD患病率为19.4%[51];另一大型队列提示慢性乙型肝炎合并MAFLD人群纤维化进展及HCC风险显著增加,HCC风险可提升约7.3倍[52-53]。因此,临床应常规评估HBV感染及饮酒史,以便识别混合病因,优化监护与干预。
总之,中国MAFLD疾病进展特征总体上与全球一致,但在部分人群或阶段存在快速进展。年龄、糖尿病、肥胖等因素共同驱动疾病发展。早期干预和高危人群筛查至关重要。本综述为临床实践和公共卫生政策制定提供了依据,并为中国MAFLD患者个体化管理策略的优化提供了参考。
