The following changes have been made to the originally published manuscript.
There were errors in Tables 1 and 2.
In Table 1, the heading above “RV sinus myectomy and pulmonary valve repair” is emended to read: “Concomitant procedures” instead of: “Concomitant TV repair”.
In Table 2, the entry for Patient 7 has been emended. “−0.3 (at second RVOh (Concomitant)” is corrected to now read: “−0.3 (at second RVOh [Concomitant])”.
