Acid suppression and gastric atrophy: sifting fact from fiction

(A) Normal gastric corpus mucosa. Glands are tightly packed and there are few inflammatory cells in the subepithelial area and in between the glands. (B) Severe corpus gastritis with probable atrophy. Islands of glands are separated by large numbers of inflammatory cells and fibrous tissue.

In H pylori infected patients who receive proton pump inhibitors for extended periods the pattern of gastritis changes from antral predominant to corpus predominant. The bacterial density tends to decrease in the antrum whereas it remains essentially unchanged in the corpus.
