An unusual cause of rectal bleeding

Clinical presentation

A 46 year old woman was referred with a nine month history of lower abdominal pain, altered bowel habit, and per rectal bleed. Her per rectal bleed usually coincided with the onset of her menstrual periods. Her periods were regular but she complained of some intermenstrual bleeding, particularly in the premenstrual days. She had no history of weight loss and her grandfather had carcinoma of the colon. A colonoscopy was performed and the following abnormality (fig 1) was seen in the rectosigmoid area. Histology of this lesion is shown in figs 2 and 3.
