US, ultrasonography; MRCP, MR cholangiopancreatography; MRA, MR angiography; ERCP, endoscopic retrograde cholangiopancreatography; PTC, percutaneous transhepatic cholangiography. *Where magnetic resonance imaging/MRCP is not possible, patients should have contrast enhanced spiral/helical computed tomography. **Fine needle biopsy or biopsy is ideally avoided until resectability has been assessed by a specialist surgeon.
