A 64-year-old woman presents with a 4-mm nodule on her right helix. The nodule is so tender to light touch that it substantially disrupts her sleep. She has had no recent trauma to the area, is currently taking no medications, and her medical history is unremarkable.

The most likely diagnosis is

  1. Basal cell carcinoma

  2. Squamous cell carcinoma

  3. Gouty tophi

  4. Chondrodermatitis nodularis chronica helicis

  5. Keratoacanthoma
