At the final stage of producing the pages for this article (BMJ 2008;336:1106-10, 17 May, doi: 10.1136/bmj.39500.677199.AE), a capital A managed to insert itself in front of the name of the first author, which should read Holger J Schünemann. This error appeared only in the printed journal and in the pdf version.
