In the version of this paper originally published online March 27, 2008, a breakpoint (with phenotype; red arrow) was missing at band 5q21.3. Additionally, in Table 1, the clinical features of case 6 should have read “Bilateral coloboma, hypotonia.” The table and figure have been corrected online and in print.
