In 2011 the second number of Clinical Cases in Mineral and Bone Metabolism (CCMBM) is presenting a unique collection of articles in the area of orthopaedics and bone metabolism. The skeletal fragility concept is mastered by Paolo Tranquilli Leali and coll. Bone properties and their change in the fragile skeletal are described, along with methods into evaluate bone fragility.
Prisco Piscitelli and coll. are responsible of an article on epidemiology of fragility fractures in Italy. While Giulio Guido and coll. described the hip fractures in the extremely elderly patients, with a deep discussion on implications to clinical management of this condition.
The “domino” behavior in fragility fractures at the vertebral level is presented as our original paper by Alfredo Nardi and coll.
Four papers are dedicated to the definition and surgical treatment of familial hip and knee prostheses.
Kamal K. Pandita and coll. report on sacral stress fractures and their management, an area rich of unanswered questions.
Two interesting clinical cases, one on oncologic osteomalacia and one on Osteogenesis Imperfecta, are described respectively by Giuseppe Guglielmi and coll. and by Patrizia D’Eufemia and coll.

Finally, Francesco Tonelli is offering an overview on novel interpretations of the skeletal disorders within the Medici family, a fascinating story that the modern science is trying to explain.
The aforementioned papers indicate the scientific growth of our Journal, whose mission is to update the knowledge of the important questions in the area of bone and mineral metabolism applied to orthopaedics.
