An otherwise healthy 57-year-old white woman presents with an erythematous, painful nodule over her left dorsal thumb. This began approximately 2 months ago, after sustaining a paper cut at work. Over the past month, she subsequently developed multiple erythematous papules and nodules that tracked up her left forearm. She has been treated with 2 courses of cephalexin without any noticeable improvement.

The most likely diagnosis is

  1. Mycobacterium marinum infection

  2. Leishmania parasite infection

  3. Francisella tularensis infection

  4. Sporothrix schenckii infection
