Local tumour destruction in renal cell carcinoma : An inpatient population-based study

Abstract

Introduction and objectives :

Local tumour destruction (LTD) is a recommended therapy alternative for localized T1 renal cell carcinoma (RCC) for patients unfit for surgery. We examined patterns of use and complication rates of LTD in a large population-based cohort.

Methods :

Overall, 5285 patients undergoing LTD for RCC were extracted from the Nationwide Inpatient Sample database from 2006 to 2010. We assessed patient and hospital characteristics, as well as postoperative complications using International Classification of Diseases, 9^th Revision codes. The impact of patient and hospital characteristics on peri-interventional complications (overall or specific) was tested using univariable or multivariable logistic regression models.

Results :

Most patients were male (61.2%), aged 70 years or older (50.3%) and most had two or more comorbidities (57.1%). Most LTDs were performed at urban (93.5%), teaching (57.7%) and low-volume (75.7%) hospitals. Overall complications were recorded in 15.4%. In multivariable analyses adjusted for clustering, overall complications occurred more frequently in older, sicker patients who were treated at low-volume hospitals (all p<0.05). Similar results were recorded when each complication category was addressed individually.

Conclusions :

LTD is recommended in older and sicker individuals. These recommendations are reflected in LTD use patterns recorded in the current study. Complications of LTD occur in one out of six patients and are more frequent in individuals with advanced age and/or multiple comorbidities, especially if LTDs are performed at low volume hospitals.

Stem cell therapy for stress urinary incontinence: comparison of bone marrow and adipose derived stem cells in animal model

Abstract

Introduction and objectives :

Bone marrow was the original MSCs source and BMSCs have been widely used to restore functional periurethral tissue SUI model in multiple preclinical and small clinical studies. However, donor site morbidity and cell number decreasing with age are of concern. ADSCs can be easily isolated and cultured, show similar immunophenotype as BMSCs and their transplantation resulted into significant improvement of the function and the histology of the urethral sphincter in SUI animal model. To evaluate the application of adipose derived stem cells in the treatment of stress urinary incontinence and compare the results with those using bone marrow stem cells.

Methods :

Adipose derived stem cells (ADSCs) and bone marrow stem cells (BMSCs) from adult rats were isolated, cultured and characterized. 30 Sprague–Dawley female rats were used as a model for stress urinary incontinence by bilateral complete section of the pudendal nerve and were subsequently subdivided into 3 groups: 6 rats were controls, 12 rats received peri-urethral injection BMSCs and 12 rats received peri-urethral injection of ADSCs. 12 weeks after the injection, we performed urodynamic testing and histological evaluation of the urethra.

Résultats :

Rat ADSCs and BMSCs were successfully isolated and cultured. In ADSCs, as in BMSCs, Cell surface markers were positive for CD29 and CD44 and negative for CD31 and CD11b. In vitro, ADSCs and BMSCs underwent adipogenic and osteogenic and smooth muscle cells differentiations. Abdominal leak point pressure (ALPP) was improved in animals injected with both ADSCs and BMSCs (mean ALPP 25.32 cmH₂O (SD: 3.14) and 24.28 (SD: 1.47) cmH₂O respectively) compared to SUI animal model (16.21 cmH₂O, SD: 1.26). On Histological evaluation, urethral muscle fibers with both ADSCs and BMSCs were thicker and more organized compared to SUI controls.

Conclusions :

ADSCs are capable of restoring anatomical and functional features of the urethral sphincter and the results are equivalent to those with BMSCs. They represent a potential alternative for stem cell therapy of SUI.

Are Canadian urology residency programs fulfilling the Royal College expectation? A survey of graduated chef residents

Abstract

Introduction and objectives :

To assess outgoing Canadian urology chief resident’s well being throughout their residency program as well as their satisfaction with different aspects of surgical training and proficiency in surgical procedures.

Methods :

An anonymous survey was sent by email to all 29 graduated urology chief residents from all provinces across Canada in 2012. The survey included a list of all urologic surgical procedures listed by the Royal College. According to the A/B/C classification used to assess competence in these procedures (A most competent, C least competent), we asked chief residents to self classify their competence with regards to each procedure, comparing the final results to the current Royal College classification.

Results :

The overall response rate amongst chief residents surveyed was 97%. An overwhelming majority (96.4%) of residents agreed that the residency program has affected their overall well-being as well as their relationships with their families and/or partners (67.8%). Overall, 85.7% agreed that research was an integral part of the residency program and 78.6% have enrolled in a fellowship program post graduation. The chief residents surveyed believed that they have received the least adequate training in robotic surgery (89.3%), followed by female urology (67.8%), andrology/sexual medicine/infertility (67.8%), and reconstructive urology (61.4%). Interestingly, in several of the 42 surgical procedures classified as category A by the Royal College, a significant percentage of residents felt that their proficiency was not category A, including repair of urinary fistulae (82.1%), pediatric indirect hernia repair and meatal repair for glanular hypospadias (67.9%), open pyeloplasty (64.3%), anterior pelvic exenteration (61.6%), open varicocelectomy (60.7%) and radical cystoprostatectomy (33.3%). Furthermore, 100% of respondents believed they were deficient in at least one of the 42 category A procedures, while 53.6 % believed they were deficient in at least 10.

Conclusions :

Most residents agree that their residency program has affected their overall wellbeing as well as their relationships with their families and/or partners. There is also a clear deficiency in what outgoing residents achieve and what the Royal College mandates. Future work should concentrate on addressing this discrepancy to assure that training is more aligned with Royal College expectations.

Manifestations urologiques du syndrôme de Down : Est-ce que le dépistage echographique est obligatoire ?

Abstract

Introduction et objectifs :

Dans l’évaluation de routine des patients diagnostiqués avec une syndrôme de Down (SD), une échographie abdominale précoce est recommandée. Toutefois, le dépistage prénatal a drastiquement changé le prise en charge initiale de cette population étant donné que la plupart des anomalies reliées au SD sont maintenant identifiés avant la naissance et un nombre décroissant d’enfants avec ce syndrôme sont menés à terme. Nous avons répertorié les manifestations urologiques évaluables par échographie dans une cohorte contemporaine de patients avec le syndrôme de Down.

Matériels et méthodes :

Les dossiers de tous les 234 patients qui se sont présentés dans notre centre avec un diagnostic de SD entre 2002 et 2011 ont été révisés ainsi que celui de leurs mères lorsque ceux-ci était disponibles. Les patients qui ont eu une échographie abdominale pendant les deux premières années de leur vie ont été inclus. L’histoire clinique de manifestations urologiques, l’évaluation prénatale et les rapports radiologiques ont été révisés.

Résultats :

Des 229 patients avec un diagnostic de trisomie 21 vu dans notre centre, 61 ont un une échographie abdominale (28 filles et 33 garçons) fait dans les deux premières années de leur vie. Les anomalies répertoriées sont les suivantes : 5 hydronéphroses (4 gauche de grade 1 ou 2, 1 bilatéral grade 1 et 3), 1 duplication, 1 rein ectopique pelvien, 7 reins hyperéchogénique bilatéralement, 1 kyste rénal, 2 bassinets épaissis, 1 vessie épaissie et 1 mégavessie. Il y avait un patient avec du reflux vési-courétéral (grade 3 bilatéralement). Un autre patient avait des infections urinaires récidivantes sans anomalies qui ont nécessité une prophylaxie antibiotique. Trente-quatre (34) patients avaient un rapport d’échographie anténatale disponible; toutefois, seulement 2 patients avaient des anomalies urologiques (i.e. hydronéphrose) au travers d’autres anomalies suggestives du syndrome. Aucune correction chirurgicale n’a été indiquée pour l’ensemble des patients en lien avec l’investigation échographique. Cinquante-six (56) patients ont eu un diagnostic de trisomie 21 tardivement, après la naissance.

Conclusions :

Les anomalies urologiques cliniquement significatives chez les patients avec le syndrôme de Down sont rares dans notre cohorte contemporaine de patient. Le dépistage échographique pour les anomalies rénales dans cette population pourrait être optionnel avec un faible risque de manquer une anomalie significative, surtout après un dépistage prénatal.

Cost analysis comparing sequential BCG and electromotive mitomycin versus BCG alone for patients with high-risk superficial bladder cancer in a universal health care system

Abstract

Introduction and objectives :

Sequential BCG and electromotive mitomycin (sequential therapy) has been shown in a randomized prospective trial to be superior to BCG alone (BCG therapy) in patients with high-risk superficial bladder cancer. We aimed to compare these two treatment strategies in a universal health care system by performing a 5-year cost-analysis study.

Methods :

Estimates for yearly progression and death rates and treatment efficacy were obtained via an extensive literature review and using the data from the single randomized trial comparing both treatment strategies. Calculated costs included (1) medical costs (physician fees), (2) drug costs (cost of drug, its preparation, and instillation), and (3) hospital costs (procedure and admission fee and cost of tests and procedures done during surveillance). Medical and hospital costs were estimated from the Regie de l’assurance maladie du Quebec (RAMQ) and Ministere de la santé et des services sociaux (MSSS) lists. Drug costs were obtained from pharmacy records. Patients were allowed a second course of induction therapy. Patients that then failed therapy from both groups and were still alive were assumed to go to radical cystectomy. A Markov model will be constructed to assess the incremental cost-effectiveness ratio.

Results :

The yearly risk of progression for patients on BCG therapy was 7.6%, 13.3%, 15.8%, 11.4% and 11.1% respectively. The yearly risk of progression for patients on sequential therapy was 0%, 3.1%, 1.1%, 0%, 3.9%. The cost of induction therapy for BCG patients was $770.88. The cost of induction treatment for patients on sequential therapy was $4757.88. Furthermore, the cost of completing 1-year of maintenance therapy for patients that remain on BCG therapy, including 4 cycles of reevaluation (cystoscopy + urine cytology) is an additional $3305.64. On the other hand, the additional cost of 1-year maintenance therapy including reevaluation for patients that receive sequential treatment is $10380.28. These yearly probabilities of progression rates and costs will be included in our Markov model that is currently being developed for the cost-effective analysis.

Conclusions :

Sequential BCG and electromotive mitomycin is associated with a higher initial material cost for induction and maintenance therapy. A Markov model will need to be developed to evaluate the cost-effectiveness of this management approach.

Efficacité comparée de la prostatectomie radicale vs. radiothérapie pour le cancer de la prostate localement avancé

Abstract

Introduction et objectifs :

À l’heure actuelle, il demeure une controverse quant à l’efficacité de la prostatectomie radicale (PR) et de la radiothérapie (RT) pour traiter le cancer de la prostate localement avancé. Cette question est d’autant plus importante avec l’adoption croissante de la surveillance active pour le cancer de la prostate à bas risque. Dans cette analyse comparative, nous comparons la mortalité spécifique et toutes causes de ces deux traitements.

Matériels et méthodes :

Nous avons identifié 1577 hommes avec cancer de la prostate localement avancé (≥cT3) traités avec PR ou RT entre 1994 et 2005 dans la base de données Surveillance, Epidemiology, and End Results (SEER)–Medicare. Nous avons utilisé une approche instrumentale pour réduire l’effet de variables confondantes non mesurées. Des modèles de probabilités concurrentes ont été calculés pour comparer l’effet des deux traitements sur la mortalité spécifique et autres causes (cardiovasculaire et non-cardiovasculaire).

Résultats :

À 10 ans, la mortalité toutes causes non-ajustée et la mortalité spécifique était de 41.6% et 13.9% pour PR vs. 58.2% et 20.4% pour RT, respectivement (p<0.001). Après ajustement, la mortalité toutes causes était comparable entre les deux groupes (HR pour PR vs. RT): 0.813, 95% CI: 0.236–2.808, P=0.744). Lorsque les analyses sont restreintes aux hommes n’ayant pas reçu de thérapie adjuvante ou de sauvetage, nos observations sont semblables (HR pour PR vs. RT): 1.428, 95% CI: 0.165–12.373, P=0.747). Dans des analyses secondaires avec des modèles de probabilités concurrentes, la différence entre RP et RT demeure non significative (HR: 0.246, 95% CI: 0.026–2.360, P=0.220).

Conclusions :

Chez des hommes âgés avec un cancer de la prostate localement avancé, PR et RT sont associés avec une mortalité spécifique et toutes causes semblables, après ajustement pour variables confondantes mesurables et non-mesurables.

Ultrasonography-guided shock wave lithotripsy for the treatment of radioluscent urolithiasis : A retrospective study

Abstract

Introduction and objectives :

Ultrasonography-guided shock wave lithotripsy is one of the non-invasive treatment strategies for radioluscent intra-renal and ureteral stones. Currently, there are few urologists in Quebec who have an expertise in this technique. Our objective was to determine the efficacy of ultrasonography-guided shock wave lithotripsy in our tertiary care centre, the CHUM (Centre Hospitalier de l’Université de Montréal), depending on the size and site of calculi and the complications associated.

Methods :

One hundred forty-three ultrasonography-guided shock wave lithotripsies were performed in our centre between January 2007 and March 2013. Twenty-one patients were excluded from our study because of missing data. We therefore retrospectively reviewed the operative procedures, imaging and follow-up of 122 patients. Analysis of the efficiency and complications of ultrasonography-guided shock wave lithotripsy was performed. Success rates were cumulated depending on the size and location of the calculi. Success was defined as no need of a further intervention. Patients were stratified in three size groups (<10mm, 10–15mm and >15 mm) and seven location groups (superior calyx, medial calyx, inferior calyx, renal pelvis, superior ureter, medial ureter and inferior ureter).

Results :

In our study, 56.5% of patients treated by ultrasound-guided shock wave lithotripsy had renal pelvis stones. Fifteen percent of the patients had stones in the medial calyx, 10.7% in the superior calyx and 9% in the inferior calyx. Nine percent of the patients had ureteral calculi. First treatment success rates for renal pelvis stones were 60% for the <10mm stone group, 70% for the 10–15 mm stone group and 21.4% for the >15 mm group. In the superior calyx group first treatment success rates were between 0% and 45.4% with a 45.4% success rate in the <10 mm study group and 0% in the 10–15 mm group. No patients were treated for >15 mm calculi. In the medial calyx group success rates were between 25% and 100% : 88.9% in the <10mm group, 25% in the 10–15mm group and 100% in the >15 mm group. In the inferior calyx group success rates were between 71.4% and 75%: 71.4% in the <10 mm group, 75% in the 10–15mm group. No patients were treated in the >15 mm group. Success rates between 40% and 100% were estimated for ureteral stones. There were few complications in our study. Five patients presented slight macroscopic hematuria, one Steinstrasse event was reported and one urinary tract infection without sepsis.

Conclusions :

Ultrasound-guided shock wave lithotripsy is an efficient treatment option for radioluscent renal or ureteral calculi associated with few complications. Succes rates vary according to size and emplacement of the stones with better results for intra-renal urolithiasis less than 15 mm.

Quality indicators and postoperative mortality of radical cystectomy in Quebec : A population-based analysis during the years 2000–2009

Abstract

Introduction and objectives :

Radical Cystectomy (RC) is a very complex urologic procedure and despite improvements in practice, technique and process of care, RC is still associated with significant complications, including death, with reported mortality rates ranging from 3–6%. Our aim was to examine quality of surgical care indicators and document the recent mortality rates at 30, 60 and 90 days after RC across Quebec.

Methods:

Within the RAMQ administrative database, we used procedure codes to identify patients who underwent RC for bladder cancer in Quebec over 10 years (between 2000 and 2009), as well as RC outcomes and dates of death. Data obtained were retrospectively analyzed in relation to multiple parameters. The outcomes analyzed included: postoperative complications and mortality rates.

Results :

A total of 2778 RC were performed in 46 hospital facilities by 122 urologists across Quebec. Among them, 851 (30.6%) patients had at least one postoperative complication and 511 (18.4%) patients had more than one complication. The Overall mortality rates at 30, 60 and 90 days were 2.8%, 5.3% and 7.5% respectively. From all, 1175 (42.3%) RC were performed in 5 academic hospitals with a mortality rates at 30, 60 and 90 days approaching 2.5%, 4.6% and 6.9% respectively. The other 41 hospitals that practiced in a community base had performed 1603(57.7%) RC with mortality rates at 30, 60 and 90 days that reached 3%, 5.7% and 7.9% respectively. Postoperative mortality rates varied based on University affiliation and region. These mortality rates ranged from 30, 60 and 90 days rates of 1.9%, 4.4% and 5.6% respectively, to rates at 30, 60 and 90 days of 3.4%, 6.2% and 8.4%, respectively.

Conclusions :

Our study demonstrates that postoperative mortality after RC in Quebec varies based on several parameters. In addition, 30-day postoperative mortality after RC in Quebec appears acceptable. However, 90-day postoperative mortality rates remain significantly elevated in some areas and require addressing.

Royal College objectives of urologic training : A survey of faculty members from Canadian training programs

Abstract

Introduction and objectives :

According to the Royal College objectives of training in urology and based on the need for independent proficiency, urologic surgical procedures are categorized as level A, B and C. Our aim was to survey the Canadian urology training faculty on the level of proficiency required for Royal College accreditation.

Methods:

We conducted a survey that was sent electronically to all Canadian urology training faculty using SurveyMonkey®. Questions focused on: faculty member’s type and years of practice, geographic location, subspecialty, access to robotic surgery, contact with any level resident and R5 resident, opinion on the level of proficiency required from a list of 54 surgical procedures and whether their most recent graduates attained level A proficiency in these procedures.

Results :

The response rate was 43.7% (95/217). Among participants 92.6% were full timers, 21.1% practiced urology for less than 5 years and 3.2% for more than 30 years. From across Canada, responses from Quebec and Ontario formed 69.4% (34.7% each). Uro-oncologists formed 37.9% of the surveyed while 75.8% had access to robotic surgery. Sixty percent of faculty operate with R5 residents between 2 to 5 days per month. When analyzing the opinion of the faculty of what should be a level A procedure, only 8 basic procedures received 100% agreement. Also, results varied significantly when analyzed on a sub-specialty base. For example, almost 50% or more of uro-oncologists believed that radical cystectomy, anterior pelvic exenteration and extended pelvic lymphadenectomy should not be categorized as level A. The following procedures had significant disagreement suggesting the need for re-classification: glanular hypospadias repair, boari flap, entero-vesical and vesico-vaginal fistulae repair. Overall, more than 80% of faculty agreed that all of their recent graduating residents achieved level A proficiency, in a consensus subset of procedures. However, more than 50% of all faculties either disagreed or were ambivalent that all of their graduating residents were Level A proficient in a subset of procedures.

Conclusions :

There exists sufficient disagreement between Canadian urology faculty to suggest revision of the current Royal College list of category A procedures.

Longer family physician to urologist referral delays and its impact on survival among women who underwent radical cystectomy (RC) for bladder cancer in Quebec

Abstract

Introduction and objectives :

We reported in 2012 that women with bladder cancer (BC) have higher median delays between their 1st general practitioner (GP) visit and their 1st urologist visit (median of 56 days versus 23 days for men). This longer delay may be caused by the fact that women presenting symptoms of BC are probably referred to a gynaecologist before being referred to an urologist. This study was undertaken to determine if this indirect referral impacts survival of women after RC.

Methods:

We analysed the data from a cohort of women having undergone RC in Quebec during the period 2000–2009 and who had their 1st urologist visit after having visited a GP or emergency physician (EP) for BC symptoms. The cohort was obtained with the linkage of two administrative databases: the RAMQ and the ISQ database. To be included in the study patients must have undergone a RC in Quebec, and also have medical services data available for the two year period before RC. Women were divided into 2 groups: those with direct referral (patients who had their 1^st urologist visit immediately after having their 1st GP/EP visit) and women with indirect referral. The effect of GP/EP to urologist referral delay on survival was assessed by Cox proportional hazards models.

Results :

Among the 347 women included in this study, 49% were indirectly referred; 70% were seen more than once by a GP or a gynaecologist before being referred to an urologist and 10% were seen by a gynaecologist before being referred to an urologist. Women who were directly referred to an urologist after seeing a GP/EP had a 30% higher survival rate after RC (HR= 0.70, 95% CI: 0.5–1.01). Women who had a gynaecologist visit before seeing an urologist had 70% more chance of mortality after RC (HR= 1.71, 95% CI: 1.04–2.80).

Conclusions :

Our results indicate that delaying referral to an urologist is associated with a poorer survival among women requiring RC for BC.

Metastatic castration-resistant prostate cancer: management and associated cost in Canada

Abstract

Introduction and objectives :

Prostate cancer (PCa) is the most common cancer and the 3rd leading cause of cancer mortality in Canadian men. Men dying of PCa do so after failing castration. The management of the metastatic castration-resistant prostate cancer (mCRPC) is complex and the associated drug treatments are costly. The objective of this study was to estimate the cost of drug treatments of mCRPC in the context of the latest evidence-based approach.

Methods :

Two Markov models with Monte-Carlo microsimulations were developed in order to simulate the management of the disease and to estimate the cost of drug treatments in mCRPC, as per Quebec’s public healthcare system, and the latest drug developments. The models include additional lines of treatment after docetaxel (i.e. abiraterone or cabazitaxel). The drug exposure and survival were based on clinical trial results and clinical practice guidelines found in literature review. All costs were assigned in Canadian dollars ($). Only direct drug costs were estimated.

Results :

The mean cost of mCRPC drug treatments over an average period of 25 months was estimated at $41,136 (95% Confidence Interval: $40,561 to $41,712) per patient. Over the mCRPC period, the luteinizing hormone releasing hormone agonists (LHRHa) prescribed to maintain castrate testosterone accounted for 22% of the total medication cost, whereas denosumab prescribed to decrease bone-related events accounted for 32.5%, respectively. When patients receive cabazitaxel in sequence after abiraterone and docetaxel, the mCRPC medications cost per patient per month increases by 66%.

Conclusions :

Our study estimates the direct drug costs associated with mCRPC treatments in the Canadian health system. The total cost of medications for the treatment of each annual cohort of 4,100 mCRPC patients was estimated at $ 168.7 million. Other emerging therapies may become part of the spectrum of mCRPC treatment in the near future, and potentially increase the cost. Perhaps decreasing the use of LHRHa therapies during mCRPC phase can result in potential savings to assist in covering the costs of life-prolonging novel systemic treatments.

Does the end-fire probe improve the prostate cancer detection rate in patients undergoing MRI-targeted prostate biopsies using cognitive fusion

Abstract

Introduction and objectives :

To assess the impact of the type of ultrasound probe (end-fire versus side-fire) on the PCa detection rate during MRI-targeted prostate biopsy using cognitive fusion.

Methods:

Inclusion criteria were: consecutive patients undergoing prostate biopsies after multiparametric MRI; elevated PSA and/or abnormal DRE; no PSA above 10 ng/ml; no clinical bulking disease; MRI areas suspicious for malignancy. Overall, 91 patients were included. A standard 10 TRUS-guided extended biopsy protocol plus 2 targeted biopsies at any MRI lesion was used. Patient’s characteristics, MRI findings, and pathology evaluations were compared between the two groups: end-fire versus side-fire cohorts.

Results :

Mean patient age and PSA were 63 years and 5.95 ng/ml, respectively. The length of sampled prostate tissue per core was longer in the end-fire cohort (14.1 versus 12.4 mm, p=0.038). Two thirds of MRI lesions were graded as score 4–5. The median number of MRI lesions was 2 and the mean volume of the index lesion was 0.64 cc. MRI findings were comparable between the two groups. The overall PCa detection rate was 58.2%. The MRI scoring system was significantly predictive for PCa detection and aggressiveness (p<0.001). There was a not statistically significant trend towards greater PCa detection rate (+23%) in the end-fire compared with the side-fire cohort (p=0.235). In the side-fire group, 70.8% of detected cancers were graded 7–10 as compared to 42.9% in the end-fire group (p=0.043). The PCa detection rate was significantly improved by 1.7-fold when MRI areas suspicious for malignancy were graded 4 or 5 in the end-fire cohort (p=0.031) compared with a not significant improvement of 1.4-fold in the side-fire group (p=0.250). The improvement of the PCa detection rate by the end-fire probe was predominantly reported in case of anterior and of apical peripheral MRI lesions.

Conclusions :

Our study does not lead to definitive recommendation favouring the use of an end-fire probe in case of MRI-targeted prostate biopsies. Nevertheless, there is a not statistically significant trend towards greater PCa detection rate in the end-fire compared with the side-fire cohort, especially in case of anterior and apical peripheral lesions.

Abstract

Introduction et objectifs :

La cystoprostatectomie radicale l’étalon-or oncologique pour une tumeur urothéliale de la vessie envahissante de haut-grade. Cependant, de nos jours, il y a une tendance à vouloir minimiser l’impact de cette approche chirurgicale sur la qualité de vie, en tentant de préserver l’organe sans compromettre le résultat oncologique. La cystectomie radicale épargnant la prostate repose sur les principes suivants : Épargner la capsule prostatique, les vésicules séminales et le paquet neurovasculaire lors de la résection de l’adénome prostatique et la reconstruction du tractus urinaire bas. Chez des hommes bien sélectionnés et pour qui l’érection et la continence sont importantes, ceci peut être une technique bien importante.

Matériels et méthodes :

Dans ce vidéo, nous démontrons la cystectomie radicale épargnant la prostate, assistée au robot ainsi que la dérivation urinaire continente néo-vessie. Il s’agit d’un homme âgé de 64 ans avec un carcinome in situ de la vessie réfractaire au BCG. Son APS était de 0.6 ng/ml et avait un TR normal. Une biopsie de la prostate a fortement été recommandée au patient, mais il l’a refusé.

Résultats :

Le temps opératoire total était de 7 heures et 21 minutes. Les pertes sanguines ont été estimées à 150 cc. Une diète a été débutée au 2^ème jour postopératoire. Les simples J et le drain ont été retirés au 4^ème jour postopératoire. Il a eu congé de l’hôpital au 5^ème jour postopératoire sans aucune complication per hospitalisation. La pathologie finale a démontré une carcinome in situ limité à la vessie et les ganglions étaient négatifs. Toutes les marges chirurgicales étaient négatives. Au dernier suivi le plus récent (8 semaines après la chirurgie), le patient avait déjà des érections partielles spontanées sans l’utilisation d’inhibiteurs de la PDE5. Son contrôle urinaire était excellent : il nie toute fuite urinaire de jour et utilise uniquement une serviette protectrice pour une énurésie nocturne.

Conclusions :

Nous avons démontré la sécurité et la faisabilité de la cystectomie radicale épargnant la prostate, assistée au robot chez des hommes sélectionnés pour qui l’érection et la continence sont importantes. La qualité de vie et le résultat oncologique à long terme vont définir davantage le rôle de cette procédure dans le traitement des hommes avec un cancer de la vessie envahissant.

Robotic partial nephrectomy : Single institution experience

Introduction and objectives :

To assess the safety and outcomes of robotic partial nephrectomy for renal tumours in a single institution robotic program.

Methods :

Between January 2011 and February 2013, 65 consecutive patients underwent a robotic partial nephrectomy by two surgeons. Preoperative characteristics including the RENAL score, operative parameters and postoperative course were assessed from a retrospective database.

Results :

Mean age was 60.2 years. Mean tumour size was 3.9 cm. The RENAL score classified tumour as intermediate risk in 51% of cases and high risk in 18.5% of cases. Mean warm ischemia time (WIT) was 23.4 minutes. Factors associated with WIT were RENAL score, tumour size, complications’ rate and surgeon’s experience (>20 cases). Mean hospital stay was 3 days and the overall Clavien 3 grade complication rate was 9.2% with a re-admission rate of 7.7%. Mean change in eGFR was 6.7 ml/min.

Conclusions :

Robotic partial nephrectomy is a safe procedure. The warm ischemia time depends on tumour characteristics and may be improved by surgeon’s experience. New cases are included in a prospective database in order to confirm these initial findings and to assess the oncological safety of the procedure.

HMGB1 as a marker for radiation response in muscle invasive bladder cancer

Introduction and objectives :

Radical cystectomy remains the “gold standard” treatment for invasive Bladder Cancer (BC). Radiation therapy is an attractive alternative as it preserves the bladder allowing for better quality of life; however, the lack of local control of the disease and significant toxicity associated with it remain problematic. Our objective is to evaluate the role of HMGB1 as a predictive marker for radiation response in BC. We hypothesise that HMGB1 can be an excellent predictor for radiation response as it has been found to play a major role in processes like chronic inflammation and DNA damage repair, which are the two main radio-resistance pathways in cancer cells.

Method :

Expression of HMGB1 and selected proteins involved in radio-resistance pathways was characterized in a panel of eight BC cell lines via RT-PCR and Western Blotting. Correlation of HMGB1 expression and radiation response of cells was done using clonogenic assays. Expression level of HMGB1 was evaluated post radiation in a time dependent manner via Western Blotting (with protein extractions done 3, 6, 12, 24, and 48 hours after radiation).

Results :

All cell lines expressed different levels of HMGB1. Out of these, BV and UC3 were chosen for further analysis as they had the lowest and the highest expression of HMGB1 simultaneously. Clonogenic assay results showed that UC3 was more resistant to radiation (with 60% cell survival post 4 GY of radiation) while BV was quite sensitive (with only 20% cell survival post 4 GY of radiation). Evaluation of HMGB1 expression in these two cell lines post radiation showed an increase during a period usually associated with DNA damage repair.

Conclusions :

Preliminary data shows that high levels of HMGB1 protein lead to a more radio-resistant outcome. Evaluation of HMGB1 expression levels post radiation in a dose dependent manner and manipulation of expression via lentiviral expression system is underway.This analysis will verify if HMGB1 could potentially be used as a radiation response marker in invasive BC.

The impact of multiple biopsy sessions on erectile and voiding function after radical prostatectomy : A population-based assessment

Introduction and objectives :

In the context of increased consideration for deferred treatment following prostate cancer (PCa) diagnosis, the number of men undergoing multiple prostate biopsy sessions (PBx) is rising. To evaluate the rates of adverse functional outcomes according to the number of PBx amongst men treated with radical prostatectomy (RP) for clinically localized PCa.

Methods:

Overall, 10,743 men who underwent RP for non-metastatic clinical stage T1–2, low-grade PCa within the U.S. Surveillance, Epidemiology, and End Results Medicare-linked database were abstracted (1995–2005). Number of PBx were dichotomized as 1 vs. ≥2, prior to RP. Postoperative functional outcomes consisted of incontinence and erectile dysfunction (ED) diagnosis/procedure. Univariable and multivariable logistic regression analyses were performed.

Results :

Overall, 7739 (72%) and 3004 (28%) patients had 1 or ≥2 PBx prior to RP, respectively. Patients who underwent ≥2 PBx had higher rates of ED diagnosis (37 vs. 31%) and procedure (6 vs. 5%), as well as urinary incontinence diagnosis (31 vs. 25%) and procedure (25 vs. 19%, all P≤0.04). After accounting for confounders, patients with ≥2 PBx were respectively 32 and 25% more likely to experience an ED diagnosis and/or procedure postoperatively (P<0.001 and P=0.027, respectively), and 27 and 31% more likely to experience a postoperative urinary incontinence diagnosis and/or procedure, (both P<0.001).

Conclusions :

Patients who underwent multiple PBx prior to RP may be at higher risk of ED and urinary incontinence postoperatively. This finding warrants consideration at the time of therapy decision making especially when active surveillance is compared to definite therapy.

Short-term mortality after external beam radiation therapy and brachytherapy in patients with clinically-localized prostate cancer: a population-based analysis

Introduction and objectives :

External beam radiation therapy (EBRT) represents a very safe treatment option in patients with clinically-localized prostate cancer, with respect to treatment-related mortality. However, anaesthesia and surgical manipulation in patients treated with brachytherapy (BT) may predispose them to higher rates of treatment-related mortality relative to EBRT. We tested this hypothesis within the SEER-Medicare database.

Methods :

We identified 43,576 patients with clinically-localized prostate cancer treated with either EBRT (n= 30,188; 69.3%) or BT (n=13,388; 30.7%). Treatment-related mortality was defined as 30-, 60-, and 90-day mortality after EBRT or BT. We focused on univariable and multivariable logistic regression analyses for prediction of 30-, 60- and 90-day mortality, according to treatment type (EBRT vs. BT).

Results :

EBRT patients were older, sicker, of low SES and more frequently single, relative to BT patients (all P<0.001). Similarly, more had poorly differentiated grade, advanced clinical stage and unknown lymph node and distant metastases status (all P<0.001). 30-, 60- and 90-day mortality rates were, respectively, 0.1, 0.3 and 0.6% and 0.03, 0,2 and 0.3% for EBRT and BT. After adjustment for all disparities between EBRT and BT, no statistically significant differences were recorded for 30-, 60- and 90-day mortality.

Conclusions :

Regardless of the exact cause of the recorded advantages for BT, we can conclude that BT is safe when 30-, 60- and 90-day mortality represent the endpoints of interests.

Factorial analysis of disease progression in prostate cancer patients under active surveillance

Introduction and objectives :

Active Surveillance (AS) is a treatment option for low risk Prostate Cancer (PCa). The aim of our study was to evaluate the effect of clinical and pathological factors in predicting disease progression.

Methods:

Our cohort includes 300 patients diagnosed with PCa between 1992 and 2012 elected to be managed by AS or refused treatment. Of these, 155 patients with at least 1 repeated biopsy and histological findings compatible with insignificant disease at the time of the diagnosis were included in this analysis. Patients were followed every 3 to 6 months with prostate-specific antigen (PSA) and physical examination and were offered repeat biopsy annually or if there were any changes on physical examination or in PSA. We defined disease progression as ≥ 1 of the following criteria; ≥ 3 positive cores, >50% of cancer in at any core involved, or a Gleason score (GS) ≥ 4+3.

Results :

The mean age ± SD at the time of diagnosis of these 155 patients was 68 ± 7 years with a median follow up of 5.4 (IQ range 3.6–9.5) years. Of these, 79, 44, 23, 7, and 2 patients had 1, 2, 3, 4 and 5 repeated biopsies, respectively. Eleven (7%) patients had a GS of 3+4 while 144 (93%) patients had a score ≤ 6 at diagnosis. Fifty (32%) patients had disease progression on rebiopsy. The 5-year overall and cancer-specific survival rate was 100%. On univariate analysis, Positive first repeated biopsy, ≥2 positive cores, PSA density of > 0.15, GS ≥6 at the time of the diagnosis, short PSAST (<2 years) have higher risk of disease progression (p<0.05). On the other hand, using 5 α- reductase inhibitor (5ARI) and having > 1 repeated biopsy have a lower risk. On multivariable analysis, positive first repeat biopsy has a significant higher risk, while the use of 5ARI and having >1 repeated biopsy have significant lower risk of disease progression.

Conclusions :

In men with low-risk prostate cancer on active surveillance, the positive first repeat biopsy, the use of 5ARI and having >1 repeated biopsy were independent predictors of disease progression.

Co-distribution analysis of p65 and RelB NF-kappaB subunits in prostate cancer patients: a large-scale immunofluorescent study

Introduction and objectives :

The implication of both classical and alternative NF-kappa B (NF-kB) signaling in prostate cancer (PCa) is now widely accepted. The classical p65 subunit is clearly involved in PCa progression and associated with poor patient outcomes, while the role of RelB alternative protein is not as well defined. Previous studies explored each NF-kB pathways individually in patient specimens and cellular models without exploring the impact of their simultaneous expression or activation. We propose to study both NF-kB pathways, not only in the same patient specimen, but more importantly, at the individual cellular level to define the biological relevance of the concomitant activation of both NF-kB pathways in PCa.

Methods:

Radical prostatectomy formalin fixed and paraffin embedded samples (n=300) were collected and used to construct tissue microarrays (TMAs). Immunofluorescence was employed to concomitantly explore the nuclear localization of p65 and RelB. Immunofluorescent signals were analyzed from scanned images of each TMA with VisiomorphDP Software (Visiopharm).

Results :

The epithelium (mask 1) was defined by an orange stained with Alexa 546 and Cy3 dyes using the cytokeratin 18, cytokeratin 19 and PSA markers. With these markers we obtained an optimal coverage of the epithelium in all tissue samples. Selection of nuclei from mask 1 was accomplished using DAPI staining thus defining epithelial nuclei (mask 2). The analysis protocol was completed with the evaluation of the nuclear staining of p65 (red Cy5) and RelB (green Alexa 488) in mask 2, according to the mean value of the nuclear signal. Using a serie of algorithms, we generated data including the proportion (% and absolute values) of negative, simple positive (either red or green) and double positive nuclei. We also harvested the mean of global and nuclear intensity that could be used to define the score of each core on TMAs (0 = negative; 1 = green; red = 2; 3 =double positive).

Conclusions :

We developed a protocol of quadruple immunofluorescence staining optimized for PCa TMAs in order to proceed to an automated semi-quantitative analysis. The use of four different filters allowed the scoring of two nuclear markers, RelB and p65, specifically in epithelial cells and perform a unique co-distribution analysis of two nuclear markers in the same cells.

L’épidermolyse bulleuse et ses complications urologiques

Abstract

Introduction et objectifs :

L’épidermolyse bulleuse (EB) est une maladie cutanée systémique dévastatrice avec atteinte tissulaire épithéliale bulleuse non-inflammatoire secondaire à des traumas mécaniques de sévérité variable. Cette maladie rare se présente parfois avec des complications urologiques. Lorsque nous faisons face à des cas plus complexes et que notre expérience est limitée, nous cherchons dans la littérature afin d’y trouver des principes pour guider notre conduite. Notre but est donc d’établir les principes directeurs de la prise en charge des complications urologiques des EB en prenant en considération la maladie globale de l’enfant.

Matériels et méthodes :

Nous avons revu les cas d’EB ayant présenté des complications urologiques dans notre centre dans les onze dernières années. Le type de complications et la prise en charge furent répertoriés. La complexité du plus récent cas demanda une revue exhaustive de la littérature et une approche multidisciplinaire impliquant l’équipe traitante de pédiatrie, la néphrologie et l’urologie.

Résultats :

Des trois cas répertoriés (dysurie, hématurie macroscopique et sténose du méat), le cas principal implique un garçon de huit ans présentant une EB avec multiples complications urologiques associées ayant mené à une prise en charge médicale et chirurgicale.

Ce dernier a nécessité de nombreuses chirurgies dont une incision de petites valves de l’urètre postérieure et réimplantation urétérale bilatérale extra-vésicale pour des sténoses des jonctions urétéro-vésicales impliquant des bulles. Il persista une dysurie importante intermittente et un reflux secondaire du côté droit qui fut observé étant donné qu’il était asymptomatique. Toutefois, une néphrectomie droite fut effectuée vu l’hypofonctionnalité et les pyélonéphrites récentes à répétition. Après une normalisation du patron mictionnel, il y eu détérioration de son hydronéphrose gauche. À l’imagerie, il fut noté une pseudo-sténose urétéro-vésicale secondaire à une rétention chronique sur dysurie importante. Le retour de la dysurie et des bulles dans son urètre ont engendré une détérioration de son rein restant. Nous conclûme donc en équipe multidisciplinaire de la nécessité de dériver les urines avec le moins de morbidité possible et fonctionnelle au long terme. Composée d’uniquement 27 cas d’EB avec complications urologiques, la littérature stipule la nécessité d’assurer une précaution anesthésique et de limiter l’instrumentation, les pansements ainsi que les drains. Une appendicovésicostomie de type Mtrofanoff fut effectuée avec succès et avec apprentissage des auto-cathétérisations.

Conclusions :

Considérant le potentiel élevé de complications chirurgicales dans la population atteinte d’EB, une reconnaissance précoce des pathologies urinaires permet une prise en charge multidisciplinaire et un suivi rapproché. Il est important de prioriser les avenues limitant les interventions mécaniques chez ces patients autant que possible. Lorsque les chirurgies sont faites en tenant compte de la pathologie de ces patients et de leur contexte souvent palliatif, les résultats sont généralement acceptables.

Determinants of performance on the transfer task of the basic laparoscopic urologic surgery (BLUS©) curriculum administered at objective structured clinical examinations

Introduction and objectives :

The Objective Structured Clinical Examination (OSCE) adapted by the American Board of Urology and the Royal College of Physicians and Surgeons of Canada assess only clinical cognitive skills while the American Board of Surgery requires candidates to pass the Fundamentals of Laparoscopy program which consists of the McGill Inanimate System for Training and Evaluation of Laparoscopic Skills (MISTELS) in addition to a cognitive skills test. Furthermore, MISTELS has been adapted by AUA for Basic Laparoscopic Urologic Surgery (BLUS^©) skills curriculum. However, these basic laparoscopic skills have not been assessed among urology postgraduate trainees in the setting of OSCEs.Therefore, the aim of the present study was to assess determinants of performance on the Transfer Task of the BLUS^© skills curriculum administered at OSCEs.

Methods:

After obtaining Institutional Review Board approval and informed consent, urology trainees (PGY-3 to PGY-5) from 4 different training programs (A, B, C, D) across the province of Quebec were recruited for the study. One of the rest stations was replaced by the peg transfer task of MISTELS/BLUS during 3 consecutive OSCEs between May 2011 and May 2012. Transfer Task Times (TTTs) were compared and correlated with previous laparoscopic experience, amount of endotrainer practice and scores obtained at practice sessions and other OSCE stations.

Results :

A total of 37 trainees were evaluated, including 16 (43.2%) trainees from program A with a dedicated laparoscopic skills training program. Compared with trainees from programs B, C, and D, trainees from program A had significantly more practice per week (0 vs. 45 min, p=0.001) and significantly lower median TTTs at OSCEs [114 (68– 209) vs. 74(52– 189) sec, p=0.001] despite significantly lower number of laparoscopic cases assisted within the previous 6 months [13 (0–57) vs. 2 (0–35), p=0.001]. For program A trainees, TTTs moderately correlated with median TTTs at practice sessions (r= 0.57, p=0.001) and negatively correlated with amount of practice per week (r= −0.41, p=0.003). Thus, more training resulted in faster times at OSCEs. On multivariate analysis, amount of practice per week was the only significant predictor of TTTs at OSCEs (p=0.028).

Conclusions :

Performance on the transfer task of BLUS^© during OSCEs significantly correlated with the amount of practice rather than the number of laparoscopic cases assisted.

Incidence et coût socioéconomique de la prise en charge intra-hospitalière du cancer de la prostate métastatique

Introduction et objectifs :

Il existe une controverse grandissante sur l’utilité du dépistage précoce du cancer de la prostate. Cependant, les conséquences d’un fardeau augmenté de patients avec un cancer de la prostate métastatique ont été peu discutées. Dans cette étude, nous examinons les tendances américaines d’incidence et de coûts associés à la prise en charge intra-hospitalière du cancer de la prostate métastatique.

Matériels et méthodes :

Les données sur les admissions pour cancer de la prostate métastatique ont été extraites du Nationwide Inpatient Sample (NIS), la plus grande base de données intra-hospitalières américaines, entre le 1^er janvier 1998 et le 31 décembre 2010. Des poids d’échantillonnage ont été appliqués pour estimer des projections nationales. L’incidence a été normalisée en fonction de la population américaine en l’an 1998 et rapporté par 100 000 personnesannées. Les coûts ont été ajusté pour l’inflation en utilisant le U.S. Consumer Price Index 2012 du Bureau of Labor Statistics. Les valeurs manquantes de coûts ont été assignées en utilisant une imputation de type ‘hot-deck’. Les tendances temporelles ont été quantifiées par la variation annuelle en pourcentage (VAP), calculée à partir d’une régression linéaire simple faisant appel à la méthode des moindres carrés à deux degrés.

Résultats :

Entre 1998 et 2010, il y a eu une estimation pondérée de 108 949 hospitalisations pour cancer de la prostate métastatique. Il y a eu une diminution significative du nombre annuel d’hospitalisations pour cancer de la prostate métastatique, de 18.0 à 12.5 /100 000 personnes-années (VAP −2.81%, 95% CI −2.1% à −3.6%). Cette diminution du nombre d’admissions est accompagnée d’une augmentation de +3.8% des coûts d’hospitalisation (95% CI 3.2% à 4.4%).

Conclusions :

Malgré une diminution du nombre absolu d’admissions pour cancer de la prostate métastatique, une augmentation substantielle des coûts a été observée. Notre étude réitère l’impact et le fardeau socioéconomique du cancer de la prostate métastatique.

Comparaison des taux de complication de la prostatectomie par laser et la résection transurétrale de la prostate : une étude populationelle

Abstract

Introduction et objectifs :

La prostatectomie au laser (L-TURP) démontré une meilleure sécurité péripératoire et une efficacité équivalente par rapport à la résection transurétrale de la prostate par électrocoagulation (E-TURP) dans plusieurs essais contrôlés randomisés menés dans des centres à haut volume. L’objectif est de comparer les taux de complications et la durée du séjour de L-TURP vs. E-TURP dans une cohorte populationnelle.

Matériels et méthodes :

S’appuyant sur la Florida Inpatient Datafile entre les années 2006 et 2008, nous avons identifié 8066 hommes avec un diagnostic primaire d’hypertrophie bénigne de la prostate ayant subit une intervention de type L-TURP ou E-TURP. Les taux de complication globaux et spécifiques et la durée de séjour (DS) représentent les issues primaires. Les tests du moindre carré et de Mann-Whitney ont été utilisés pour comparer les caractéristiques de base. Un modèle de régression linéaire généralisée multivariée (MVA) a été utilisé pour examiner l’effet de la L-TURP vs. E-TURP sur les issues primaires.

Résultats :

Aucune différence significative n’a été enregistrée pour l’âge des patients (75 contre 73 ans), le score moyen de comorbidité (0,4 vs. 0,5), la race (78,6 vs. 80,1% blanc) et l’assurance Medicare (83,5 vs. 79,5%) pour les individus subissant L-TURP ou E-TURP respectivement. Le taux global de complications ne diffèrent pas de manière statistiquement significative pour L-TURP par rapport à E-TURP dans l’analyse univariée (8,8 vs. 7,4%, p=0,1) et multivariée (odds ratio [OR] 1,06, intervalle de confiance à 95% [IC]: 0,85 −1.32, p=0,6). Les personnes qui subissaient une E-TURP étaient plus susceptibles de connaître une DS au-delà de 1 jour (46,2 vs. 59,7%, p<0,001). Un risque diminuer de connaître une DS au-delà de 1 jour a été confirmée pour les patients subissant L-TURP après analyse multivariée (OR: 0,37, IC à 95%: 0,23 à 0,58, p<0,001), mais pas pour une DS au-delà de 2 jours (OR: 0,96, IC à 95%: 0,83 à 1,10, p=0,2).

Conclusions :

Les caractéristiques des patients et la sécurité périopératoire étaient similaires pour les deux groupes. Cependant, il y a un bénéfice par rapport à la durée de séjour des patients L-TURP par rapport aux patients E-TURP.

La mesure du point de pression de fuite rétrograde améliore les issues du Male Sling Virtue pour le traitement de l’incontinence urinaire à l’effort post-prostatectomie radicale

Abstract

Introduction et objectifs :

Nous rapportons notre expérience avec le Male Sling Virtue. Les objectifs de l’étude sont d’évaluer rétrospectivement l’efficacité et le taux de complication du Male Sling Virtue pour le traitement de l’incontinence post-prostatectomie avec et sans mesure du point de pression de fuite rétrograde (RLPP) per-opératoire, ainsi qu’évaluer le taux de satisfaction subjectif des patients quant à la qualité de vie (QoL) après le traitement.

Matériels et méthodes :

Étude rétrospective des taux de continence et de complications chez 34 hommes consécutivement traités avec le Male Sling Virtue pour incontinence post-prostatectomie entre mars 2009 et février 2013. Évaluation initiale des patients incluait l’historique médical complet et un examen physique, décompte des pads utilisés par jour, pad test (poids d’urine) des 24 h x 3 jours, la cystoscopie et le bilan urodynamique. Dans les 18 premières implantations, la tension de la bandelette a été ajusté en tirant à la fois les deux bras transobturateurs et les deux bras pré-pubiens pour avoir une coaptation urétral adéquate, confirmé par cystoscopie per-opératoire. Pour les 16 derniers patients, les quatres bras de la bandelette ont été initialement tensionné à la main pour avoir un RLPP à 60 cm H2O, puis les bras ont ensuite été fixés en place avec des pinces hémostatiques. Par la suite, les bras ont été fixés avec des points de sutures à la région périnéale, et la longueur excessive des bras ont été excisés. Le RLPP final a ensuite été re-mesurée et enregistrée. Le suivi post-opératoire était de 2, 6 et 12 mois, puis annuellement. Le Patient Global Impression on Improvement scale (PGI-I) et les pad tests post-opératoires ont été utilisés pour mesurer la satisfaction subjective et les issues cliniques à la dernière visite de suivi.

Résultats :

Les 18 premiers patients de l’étude n’ont pas eu de mesure du RLPP per-opératoire, tandis que les 16 derniers patients l’ont eu. Le temps moyen (intervalle) de suivi était de 15.9 (1.8 à 45) mois. L’âge moyen (intervalle) était de 66 (53–75) ans. Sur 34 patients au total, 11 (32%) patients avaient une incontinence légère (0–2 pads par jour [ppj] ou pad test <100g/24h), 17 (50%) patients avaient une incontinence modérée (3–5 ppj ou pad test 100–400g/24h), et 6 (18%) patients avaient une incontinence sévère (>5 ppj ou pad test >400g/24h). Il n’y avait pas de différences statistiquement significatives entre les deux groupes pour l’âge, la sévérité de l’incontinence, la technique de la prostatectomie (laparoscopique vs. rétropubienne), la radiothérapie, ou des traitements des pathologies du col vésical. Sur les 18 premiers patients qui n’ont pas eu de mesure du RLPP per-opératoire, 11 (61%), 3 (17%) et 4 (22%) patients n’ont pas eu d’amélioration (réduction pads <50%), une amélioration (0–1 ppj ou réduction de pads >50%), et la guérison (0 ppj) de leur incontinence respectivement, comparativement à 2 (12.5%), 2 (12.5%), et 12 (75%) des 16 derniers patients ayant eu le RLPP mesuré per-opératoire. La moyenne du RLPP final (intervalle) était de 41.1 (35–58) cm H2O. Il n’y avait pas de complications per-opératoires. 11 (61%) des 18 patients qui n’ont pas eu, par rapport à 1 (6%) des 16 patients qui ont eu le RLPP mesuré ont eu des traitements chirurgicaux ultérieurs pour une incontinence persistante. Une douleur transitoire est survenue dans 2 cas (11%) des 18 qui n’ont pas eu, par rapport à 10 (62%) des 16 patients qui ont eu le RLPP mesuré. 1 patient du groupe avec mesure du RLPP avait une déhiscence de plaie qui a été débridé et refermé primairement. 1 patient du groupe avec mesure du RLPP a eu une rétention urinaire transitoire nécessitant une sonde urétrale pour deux jours. 5 (28%) des 18 patients qui n’ont pas eu, par rapport à 12 (75%) des 16 patients qui ont eu le RLPP mesuré étaient très satisfaits de leur choix.

Conclusions :

Le Male Sling Virtue est une option de traitement efficace et sécuritaire pour l’incontinence urinaire post-prostatectomie légère à modérée. La mesure du RLPP per-opératoire améliore significativement les taux de guérison et de satisfaction.

Étude des effets de la kétamine sur l’urothélium à l’aide d’équivalents vésicaux issus du génie tissulaire

Abstract

Introduction et objectifs :

La kétamine est un agent anesthésique utilisé tant pour l’induction de l’anesthésie générale que pour soulager les patients avec des douleurs chroniques. Elle est associée avec des effets secondaires du type hallucinations, confusion et impression de dissociation du corps et de l’esprit. Ces effets, agréables pour certains, combinés à son faible coût ont mené à son utilisation comme drogue récréative. Or, au cours des dernières années, des cas de cystites sévères liées à l’utilisation de kétamine furent rapportés. Afin de mieux comprendre les mécanismes impliqués dans cette affection, les effets de la kétamine ont été évalués sur un modèle d’étude tridimensionnel de tissu vésical.

Matériels et méthodes :

Des équivalents vésicaux entièrement humains produits par génie tissulaire en utilisant la méthode d’auto assemblage propre à notre centre de recherche ont été utilisés : des cellules urothéliales on été ensemencées sur un stroma reconstruit par des fibroblastes dermiques et incubées à l’interface air/liquide permettant leur différenciation. Des solutions de kétamine à différentes concentrations furent ensuite déposées sur l’urothélium des équivalents matures pour une période de deux et quatre jours. À l’issue de cette incubation, des coupes histologiques et des marquages par immunofluorescence furent réalisés afin d’évaluer les effets de la kétamine sur l’urothélium.

Résultats :

Un effet très délétère de la kétamine sur l’urothélium fut démontré. En effet, la structure de ce dernier fut notablement affectée par la présence de kétamine. De plus, cet effet avait tendance à augmenter en fonction du temps et aussi de la concentration.

Conclusions :

À l’issu de cette étude, il semble qu’un des mécanismes impliqués dans les symptômes urinaires des consommateurs chroniques de kétamine puisse être une attaque directe de l’urothélium par la kétamine qui est excrétée dans les urines. Ce travail original utilisant un équivalent humain issu du génie tissulaire, ouvre la porte à une étude plus poussée des mécanismes impliqués dans la cystite à kétamine de même qu’à l’étude d’éventuels traitements.

Expression des protéines GLUT-1, GLUT-12 et hexokinase 2 dans l’adénocarciome prostatique

Abstract

Introduction et objectifs :

La positivité au FDG-TEP-Scan a été corrélée à l’expression de transporteurs du glucose dans certains cancers non-urologiques, mais la littérature a été inconsistante à démontrer des relations solides. De plus, aucune étude de bonne puissance n’a été publiée sur l’expression des transporteurs du glucose et des kinases dans le cancer de la prostate primaire (CaP). Nous avons démontré que la captation intraprostatique au FDG-TEP-Scan est associée à des facteurs de mauvais pronostics tels que le score de Gleason. Dans cette étude, nous visions à déterminer le modèle d’expression des GLUT-1, GLUT-12 et Hexokinase 2 (HEX2) dans les glandes prostatiques bénignes et le CaP, ainsi que d’évaluer si ce modèle d’expression pouvait corréler avec la captation intraprostatique de FDG au FDG-TEP-Scan.

Matériels et méthodes :

Des anticorps contre les épitopes GLUT1, HEX2 et GLUT12 ont été utilisés pour effectuer des immunohistochimies (IHC) sur des tissus prostatiques. Les IHC ont été faites sur 2 groupes de patients. Le groupe 1 incluait 48 patients et consistait en du tissu prostatique normal, de l’hyperplasie bénigne de la prostate (HBP), du CaP Gleason 6 (3+3) ou Gleason 7 (4+3/3+4) (n=12 par groupe). Le groupe 2 incluait 29 patients avec des CaP à hauts risques (Gleason ≥ 8 à la biopsie) qui avaient eu un FDG-TEP-Scan avant une prostatectomie radicale avec lymphadénectomie. Neuf de ces patients avaient une captation intraprostatique de FDG augmentée. Des analyses statistiques non-paramétriques ont été effectuées.

Résultats :

Basé sur des scores tumoraux moyens (ST), HEX2 et le GLUT12 étaient constamment surexprimés dans le CaP (ST=1.14 ± 0.41 pour HEX2 et ST=1.34 ± 0.50 pour GLUT12) lorsque comparés à l’HBP (ST=0.54 ± 0.35 pour HEX2 et ST=0,81 ± 0,43 pour GLUT12 ) ou les glandes prostatiques normales (ST=0.85 ± 0.56 pour HEX2 et ST=0,52 ± 0,34 pour GLUT12) (p<0,05). L’expression de HEX2 était augmentée dans les CaP Gl ≥8 comparativement aux Gl 6 ou 7 (p<0,05). Aucun modèle d’expression des marqueurs ne corrélait avec la captation intraprostatique de FDG au TEP-Scan.

Conclusions :

Nous démontrons dans la plus large série publiée pour le moment sur le CaP primaire, que les protéines GLUT1, GLUT12 et HEX2 sont exprimées dans le CaP. Toutefois, même si l’expression des GLUT12 et HEX2 est augmentée dans le CaP, cette expression ne corrèle pas avec la captation de FDG au TEP-Scan.

Modèles et tendances de soins chez les hommes présentant des symptômes du bas appareil urinaire : une analyse socioéconomique des salles d’urgence aux États-Unis

Abstract

Introduction et objectifs :

Les symptômes du bas appareil urinaire (SBAU) sont fréquents chez les hommes plus âgés. Dans cette étude, nous examinons l’incidence des SBAU chez des hommes américains et déterminons les caractéristiques reliées au patient et à l’institution associés avec un risque augmenté d’hospitalisation après une visite dans la salle d’urgence.

Matériels et méthodes :

Les visites dans la salle d’urgence avec un diagnostic primaire de SBAU entre 2006 et 2009 ont été extraites du Nationwide Emergency Department Sample (NEDS). Nous avons calculé l’incidence ajustée pour l’âge des SBAU dans les salles d’urgence, de même que les coûts associées à ces visites. Les coûts ont été ajusté pour l’inflation en utilisant le U.S. Consumer Price Index 2012 du Bureau of Labor Statistics. Les valeurs manquantes de coûts ont été assignées en utilisant une imputation de type ‘hot-deck’. Des analyses uni- et multi-variées ont été faites pour identifier les caractéristiques associées à un séjour intra-hospitalier.

Résultats :

Entre 2006 et 2009, une estimation pondérée de 1 178 423 visites à la salle d’urgence pour SBAU a été identifiée dans le NEDS, ce qui représente une incidence nationale de 197.6 par 100 000 hommes par année. De ces cas, 112 288 patients (9.5%) ont été hospitalisés. Des effets indésirables de l’hypertrophie bénigne de la prostate ont été identifiés dans 121 177 patients (10.3%). Les coûts associés à ces visites en 2009 ont été estimés à $494 981 922. Dans les analyses multivariées, les prédicteurs d’admission comprenaient le Charlson Comorbidity Index (CCI) ≥3 (OR: 5.67, p<0.001), le type d’assurance (Medicaid vs. privée OR: 1.60, p=0.001), la localisation de l’hôpital (rural vs. urbain non-académique OR: 0.32, p<0.001), le revenu moyen de la région (très élevée vs. très bas OR: 0.65, p=0.005), le volume de la salle d’urgence (très élevé vs. très bas OR: 1.61, p=0.004), et des effets indésirables tels que le sepsis (OR: 88.08, p<0.001), l’insuffisance rénale aigue (OR: 44.29, p<0.001), l’hématurie (OR: 5.74, p<0.001) ou l’hydronéphrose (OR: 15.46, p<0.001).

Conclusions :

Malgré l’émergence de traitements pharmacologiques efficaces, les visites à l’urgence pour SBAU sont fréquentes et représentent un fardeau économique imposant aux États-Unis.

Predictors of prolonged OR time during robotic-assisted radical prostatectomy

Abstract

Introduction and objectives :

Time efficiency during robotic radical prostatectomy is of critical importance to improving resource utilization in the operating room. However, little is known about the determinants of robotic OR time after the initial learning curve period. Therefore our objective is to determine predictors of prolonged OR time during robotic radical prostatectomy.

Methods:

We prospectively collected data from 440 consecutive patients who underwent robotic radical prostatectomy performed by a single surgeon at an academic institution from 2006 to 2012. The first 40 cases from the early learning curve were excluded, as previously defined¹. Our primary outcome, prolonged OR, was defined as greater than one standard deviation above the mean OR time. The Student-t test was used to compare continuous data, and Chi-square for categorical data. A multivariate logistic regression was conducted to identify predictors of prolonged OR time, and a multivariate linear regression was used to further characterize their impact.

Results :

Our cohort was made up of men aged 60 ± 7 years, with low or intermediate risk prostate cancer (PSA 7±3, 60% Gleason 6, 40% Gleason 7, and 71 % stage 1 disease). These men had a mean BMI and IIEF scores of 27±3 and 17±8, the majority of which underwent bilateral (62%) or unilateral (21%) nerve sparring prostatectomy with pelvic lymph node dissection (49%). The mean OR time was 187±32 min. Logistic regression revealed 5 independent predictors of prolonged OR time (Table 1). According to the multivariate linear model, OR time was most clearly affected by the presence of a robotic malfunction 32 min per malfunction, blood loss 6.5 minutes/100cc, PSA 10 min/10 unit and gland volume 3 min/10cc, when controlling for all other variables (Table 1).

Conclusions :

Robotic malfunction was the strongest predictor of prolonged OR time. Blood loss, PSA, and gland volume were also associated with prolonged OR time. Knowledge of these predictors may assist in improving resource utilization.

Determination of the accuracy of FDG-PET/CT in the primary staging of biological high risk prostate cancers before local therapies: Increased uptake is associated with highly agressive tumours

Abstract

Introduction and objectives :

Cancer staging with FDG-PET/CT is accurate for many cancers but not for prostate cancers (PCa) based on early studies on heterogeneous cohorts of patients. However, in more recent studies, FDG-PET/CT was shown to be as accurate as 18F-Choline-PET/CT in recurrent and metastatic PCa. Moreover, it was shown that glucose metabolism enzymes were overexpressed in high Gleason sum PCa. We therefore hypothesized that FDG-PET/CT might be useful in the initial staging of biological high risk PCa (Gleason ≥8) before local therapies.

Methods:

We retrospectively collected data on 54 patients with Gleason score 8 or higher at TRUS-biopsy that had FDG-PET/CT and bone scan as initial staging modalities. Management of these patients was left to the urologist oncologist based on standards of care. 41 patients underwent radical prostatectomy (RP) with extended pelvic lymph node dissection, while 13 patients received androgen deprivation therapy (ADT) alone or with radiation therapy. Increased FDG uptake for each organ was defined as our detection rate.

Results :

At biopsy, 73 and 27% of patients had Gleason sum 8 and 9, pre-operative PSA was 16 ng/mL (median = 7.5) and 34, 34, 26 and 6% of patients had clinical stages T1, T2, T3 or T4. Increased FDG uptake was found in the prostate, lymph nodes (LN) and bones of 44, 13 and 6% of patients. Bone scan was positive in the 3 patients that had bone FDG uptake. In the operated patients cohort, using RP pathological specimens, sensitivity, specificity, positive and negative predictive values were 27, 100, 100 and 78% for LN metastasis. On univariate analysis (RP group), higher clinical stage, pathological Gleason sum and pattern and the percentage of intraprostatic cancer were significantly associated with intraprostatic FDG uptake (all p<0.0420). Patients without FDG uptake in the prostate were downstaged to Gleason ≤7 in 73.3% of cases at RP (vs. 18.2% with FDG uptake, p<0.0001). None of the patients with a primary Gleason pattern 3 had FDG uptake in the prostate, while all the patients with a primary Gleason pattern 5 had FDG uptake (p=0.0027). Using two validated nomograms based on clinical and pathological RP data, we found that 5-year progression-free probability was statistically different between patients that had intraprostatic FDG uptake with those that did not (CAPRA-S nomogram: 27 vs. 70% respectively, p=0.0097).

Conclusions :

Our results suggest for the first time that FDG-PET/CT is highly specific for PCa metastasis and may identify patients with higher risk of local treatment biochemical failure and define a very high risk subgroup of patients. Therefore, we demonstrate a possible staging and prognostic role for FDG-PET/CT in high risk prostate cancers imaged before primary therapies.

Retrospective evaluation of efficacy and safety of Argus sling for treatment of male Stress urinary incontinence: the Canadian experience

Abstract

Introduction and objectives :

The Argus transobturator male sling for stress urinary incontinence (SUI) can be performed after radiation and is adjustable postoperatively.We aim to report the efficacy and safety in the first North American series of patients.

Methods:

The records of 96 patients treated at 8 Canadian institutions since 2010 were reviewed. Continence status and complications were recorded.

Results :

Of the 96 patients 25 had mild (1–2 pads), 57 had moderate (3–5 pads) and 14 had severe (>5 pads) incontinence. With a mean follow-up of 8.9 months (range 1.0–25.7), improvement in continence status was noted in 92%, with 57% being completely dry. There was a significant reduction in daily pad use from 3.8 to 0.9 (p0.0001).Twenty-one (22%) men had had previous radiation and their oucomes were similar to those who did not. Adjustment was necessary in 17 (18%) cases - loosening (1/96) or tightening (16/96) at an average of 192. No device erosion was seen. The overall complication rate was 65%, the most common of which was early postoperative perineal discomfort, in 38%. This resolved in most but 7 patients had persistent pain. Other complications included 3 intraoperative urethral/bladder perforations, 8 de novo OAB symptoms, 4 UTIs, 3 wound infections, 19 patients with transient urinary retention and 2 with chronic retention. Nine patients required Argus removal because of sling breakage (3), sling infection (2), incontinence (2), chronic retention (1), or chronic scrotal pain (1).

Conclusions :

The Argus sling was found to be highly effective to treat male SUI, even after radiation therapy. Although mostly minor and self-limited, a non-negligible complication rate was seen, which raises concerns especially about postoperative perineal pain. We plan to continue reporting outcomes after extended follow-up.

Développement d’un système d’imagerie moléculaire basé sur l’activité transcriptionnelle de l’ARN non-codant DD3 pour imager spécifiquement les cellules cancéreuses prostatiques

Introduction et objectifs :

L’imagerie moléculaire joue un rôle important en oncologie pour la stadification des tumeurs. Malheureusement, la spécificité et la sensibilité des techniques actuelles demeurent faibles. Cette étude vise à améliorer le système d’imagerie par amplification transcriptionnelle, nommé le « Two-Step-Transcriptional-Amplification (TSTA) system » afin d’imager avec précision in vivo les cellules cancéreuses prostatiques (CaP) par Tomographie par Émission de Positrons (TEP). Nous étudions le potentiel du promoteur de DD3/PCA3, un gène exprimé spécifiquement dans le CaP, à atteindre cet objectif.

Matériels et méthodes :

Diverses constructions d’adénovirus incorporant le promoteur DD3, le système TSTA et le gène rapporteur de la « Firefly » luciférase ont été générés et leurs spécificités pour les cellules CaP testées en infection transitoire. Par génie moléculaire, nous avons amélioré le système TSTA et généré le 3STA. Les activités luciférases de DD3-TSTA, de DD3-3STA et de PSA-TSTA (Prostate specifique antigen) ont été comparées in vivo par bioluminescence dans des modèles de xénogreffes de souris.

Résultats :

Le promoteur DD3, est à la fois spécifique aux cellules prostatiques et cancéreuses. Lorsque qu’incorporé dans les systèmes d’amplification TSTA et 3STA, le promoteur DD3 demeure spécifique aux cellules CaP et son activité est amplifiée de plus de 300 et plus de 600 fois, respectivement, lorsque comparés à l’activité de base du promoteur de DD3. De plus, les activités de DD3-TSTA et DD3-3STA sont androgéno-indépendantes. In vivo, DD3-3STA présente des niveaux d’activités comparables à ceux obtenus avec le système PSA-TSTA dont l’activité peut être imagé par TEP.

Conclusions :

Le nouveau système DD3-3STA permet une imagerie spécifique et sensible des cellules CaP. Grâce au nouveau système d’amplification 3STA, l’activité de DD3 atteint des niveaux permettant l’imagerie par TEP, une technologie disponible en clinique.

LITHOGOLD versus STORZ SLX

Abstract

Introduction and objectifs

Extracorporeal shock wave lithotripsy (ESWL) revolutionized the treatment of renal calculi disease. Through innovative research newer lithotripters with larger focal zones and lower peak pressures have been developed. These lithotripters have been shown to produce better fragmentation with less acute renal injury. However, clinical data based on these newer lithotripters is scare. The objective of this study is to clinically evaluate two lithotripters with different focal zones and peak pressures. The LithoGold LG-380 lithotripter (LG) has a large focal zone (~20mm) and low peak pressures (~20MPa), whereas the Storz Modulith SLX lithotripter (SLX) has a small focal zone (~3mm) with high peak pressures (~90MPa).

Methods:

155 consecutive patients were treated with the LG lithotripter followed by 200 consecutive patients who were treated with the SLX lithotripter. Shock waves were delivered at a rate of 60 shocks waves/min and included pause and power ramping protocols. Shocks were delivered until stone breakage or until 4000 total shocks were delivered. All pre-operative and postoperative films were reviewed by one dedicated GU radiologist.

Results :

We had 142 and 75 patients who were treated with the LG and SLX lithotripters respectively (Table 1). There was no statistically significant difference in the original stone locations however overall there seemed to be more renal stones and less ureteral stones in the LG group (p=0.06). Additionally, there were more stones greater than 5mm in the LG group (p=0.01) and more shocks were delivered in the SLX group (p≤0.001). The overall stone free rate was not statistically different between the SLX and LG lithotripters (p=0.1) at 55% and 44% respectively (Table 2). Additionally the number of ancillary procedure was also not statistically significantly different between the two groups (p=0.4). Finally, the number of procedures per patient was 1.6 and 1.8 for the SLX and the LG lithotripters respectively.

Conclusions :

Overall there was no statistically significant difference in the fragmentation rate between the LG and the SLX lithotripters. Both lithotripters resulted in below average fragmentation rates. These results could be due to the lack of a dedicated lithotripter technologist and inadequate acoustic coupling.

No increase in Clavien complication rate with simultaneous bilateral percutaneous nephrolithotomy

Abstract

Introduction and objectives :

Percutaneous nephrolithotomy was first reported in 1976. At our institution we have been performing simultaneous bilateral percutaneous nephrolithotomys (bPCNL) since the early 90’s. The main benefits of performing this procedure includes the use of a single anesthetic session, cost and efficiency. Reports have documented a higher rate of complications associated with simultaneous bPCNLs compared to unilateral percutaneous nephrolithotomy (uPCNL). In this study we compare uPCNL and bPCNL using the Clavien grading system for complications.

Methods:

A retrospective single institution analysis of PCNL patients between 2007–2011 was performed. Pre, intra and postoperative variables were compared. In addition all complications were identified and reported according to the Clavien grading system.

Results :

Overall, of 468 patients, 362 had uPCNLs and 106 had simultaneous bPCNLs (Table 1). Total surgery time (p≤0.001), length of stay (p≤0.001) and the need for a secondary procedures (p≤0.001) was more frequent in patients undergoing bPCNL (Table 2). Postoperatively (Table 3), mean creatinine at 24 hours was higher in the bilateral group (p≤0.001). Additionally, transfusions were more frequent in patients undergoing bPCNL (p=0.004) which led to more Clavien II complications in this same group (p=0.009). There was no difference in Clavien class I (p=0.6), IIIa (p=0.2) and IIIb (p=0.1) between the unilateral and bilateral PCNL groups. In addition the overall Clavien complication rate was not different between the uPCNL and bPCNL groups (p=0.1). There were no Clavien IV complications.

Conclusions :

The Clavien complication rate associated with bPCNL is not higher then that associated with uPCNL. It is our belief that at institutions with nephrolithiasis expertise, simultaneous bPCNL should be standard of care and should be offered to all patients with large bilateral renal calculi.

Incidence et évolution du carcinome rénal des reins natifs après transplantation rénale : étude comparative avec une population générale

Introduction et objectifs :

Le carcinome rénal (RCC) représente seulement 2 à 3% des cancers, mais l’incidence chez les greffés rénaux est jusqu’à 15 fois plus élevée que dans la population générale. L’association entre la maladie rénale kystique acquise (ACKD) de l’insuffisance rénale et le développement du RCC contribuerait en partie à cette incidence plus élevée. De plus, la littérature montre une maladie avec des caractéristiques cliniques et pathologiques uniques, ainsi qu’une évolution plus favorable chez le greffé rénal, suggérant des mécanismes oncogéniques spécifiques.

Matériels et méthodes :

Les objectifs de notre étude étaient de décrire les caractéristiques cliniques et pathologiques des greffés rénaux qui développent un RCC des reins natifs, de les comparer avec la population générale et enfin, d’évaluer l’efficacité du dépistage des RCC en vigueur dans notre population de greffés rénaux. Le dépistage consiste en une échographie des reins natifs chaque 2 ans. Entre 1981 et 2010, parmi les 1347 greffés rénaux de l’Hôtel-Dieu-de-Québec, 44 patients ont développé un RCC pour un total de 47 tumeurs (prévalence 3.5%). Nous avons comparé ce groupe de greffés à 334 patients issus de la population générale ayant subi une néphrectomie partielle (n = 178) ou radicale (n = 156) pour RCC dans notre milieu entre 2003 et 2010. Les données ont été collectées de façon rétrospective.

Résultats :

Comparativement à la population générale, les greffés étaient plus jeunes au diagnostic de RCC (55 vs. 60 ans; p<0.001), les hommes étaient plus souvent touchés (86 vs. 62%; p<0.001), leurs tumeurs étaient plus petites (39 vs. 42 mm; p<0.001) et le type histologique papillaire était prédominant (55 vs. 16%; p<0.001). Le stade pathologique et le grade nucléaire de Fuhrman étaient comparables. Nous n’avons pas démontré de différence dans la survie sans récidive et la survie sans décès par RCC. D’autre part, 50% des greffés avec RCC démontraient une ACKD. Parmi le groupe de greffés, 45% ont passé leur échographie aux 2 ans, 23% annuellement, 27% n’ont pas eu de dépistage et le dépistage était inconnu pour 5%. Il n’y avait pas de différence dans la mortalité par RCC peu importe le type de dépistage.

Conclusions :

En accord avec les connaissances actuelles sur les greffés rénaux avec RCC, nous démontrons une prédominance du type histologique papillaire, ainsi qu’une proportion d’ACKD plus grande (50%) que chez les greffés en général, dont la prévalence est estimée à 25%. Enfin, le dépistage par échographie aux 2 ans n’a pas démontré d’avantage de survie dans notre petite population. Le dépistage de routine demeure un sujet controversé dans la littérature.

A multicentre randomized controlled study comparing surgical and pharmacological therapy to treat mixed urinary incontinence

Abstract

Introduction and objectives :

Mixed urinary incontinence (MUI) defined as the combination of stress and urge incontinence accounts for approximately 33% of all cases of incontinence in women. No consensus exist in literature either medical or surgical treatment has to be proposed first and the usual attitude to treat the dominant symptom first is not supported by any evidence and often disappointing. Aims of study are to determine the most effective strategy and sequence of therapy between surgery and anticholinergics in females suffering from mild to severe MUI.

Methods:

66 women were included in a prospective multicentre randomized study with an optimal cross over. In phase 1 of the study patients were randomized into two groups: TVT-O (J&J Women’s Health) procedure or Oxytrol patch (Paladin) 3.9 mg/Q3D for 12 weeks. In phase 2 patients not satisfied with the result of their initial treatment were offered a cross over. Our primary outcome was 24h pad test. Cure was defined by a 24-h pad test of 8 g or less. Improvement was defined by a reduction of pad weight ≥ 25% in comparison to base line and failure less 25% reduction of pad weight. The follow-up was 56 weeks.

Results :

66 patients were randomised and 62 patients completed the study. In phase 1, 31 patients received Oxytrol treatment and 31 patients TVT-O procedure. At 12 weeks, 64% (20/31) of patients having had TVT-O as first line were satisfied (80% (16/20) cured). Only 16% (5/31) patients having had Oxytrol as first line were satisfied (20% (1/5) cured). At 12 weeks, 84% (26/31) of patients who started with Oxytrol crossed over to TVT-O and 61,5% (16/26) were cured at 56 weeks after surgical procedure. Only 35% (11/31) of patients who started with TVT-O crossed over to Oxytrol and 18% (2/11) were cured at 56 weeks.

Conclusions :

Patients with moderate to severe MUI treated surgically in first line have statistically better chance to be cured in comparison to a patient treated pharmacologically. This RCT support initial treatment of MUI by mild urethral sling.

Novel, remotely-controlled, artificial urinary sphincter: A retrocompatible device

Introduction and objectives :

The implantation of an artificial urinary sphincter (AUS) is considered the standard treatment for severe cases of stress urinary incontinence in men. The functioning of this implant is purely hydro-mechanical applying a constant pressure around bulbous urethra. Common reasons for revision of the device are insufficient cuff pressure or at contrary urethral atrophy secondary to this constant pressure. Furthermore its functioning requires some dexterity limiting its use in some patients. Our objective was to develop a novel AUS offering the possibility to remotely control the pressure within the implant rapidly and without complex manipulation.

Methods:

We developed this new sphincter using an AMS 800 to which we replace the pump. The new electronic pump has been designed as small as possible to facilitate its implantation beside the reservoir and to be compatible with the existing AMS 800 tubing. We integrated a minimal number of components to minimize power consumption and reduce failure risk and production cost.

Results :

The device is extremely compact and totally wireless. Cuff pressure can be controlled and modified remotely. The e-pump is fast-reacting and compatible with many Bluetooth remote controllers such as intelligent phones. A prototype has been tested on a fresh pig’s bladder and total continence was proven.

Conclusions :

The new e-pump can be easily implanted in patients with limited dexterity and also in women. It could also replace a manual pump in an already implanted device.

La Biobanque Procure du cancer de la prostate : Résultat d’une fructueuse collaboration au Québec

Abstract

Introduction et objectifs :

Le cancer de la prostate figure au 1^er rang des cancers chez l’homme et au 3^e comme cause de décès par cancer au Canada. On ne peut prédire l’évolution, ni les risques de récidive de ce cancer, d’où l’importance de la recherche dans ce domaine. C’est ce qui a motivé PROCURE, un organisme québécois voué à la lutte contre le cancer de la prostate, à créer une biobanque pan-provinciale afin de fournir de précieux échantillons et données pour la recherche.

Matériels et méthodes :

Entre 2007 et 2012, plus de 2000 patients subissant une prostatectomie suite à un diagnostic de cancer de la prostate ont signé un formulaire de consentement et ont accepté de fournir à la biobanque des spécimens de sang, d’urine et de tissu prostatique; les biospécimens ont été traités et entreposés à chacun des sites selon de rigoureuses procédures normalisées de fonctionnement. Les participants ont aussi rempli un questionnaire sur leurs habitudes de vie et leurs données socio-démographiques. Des analyses biochimiques des acides nucléiques du sang et histopathologiques des tissus congelés ont été réalisées pour vérifier la qualité des biospécimens entreposés.

Résultats :

La compilation des données cliniques et pathologiques de la cohorte de patients participant à la Biobanque PROCURE montre qu’il s’agit d’hommes de 62 ans en moyenne dont l’APS au diagnostic est de 7,8 ng/mL. Le poids de la prostate lors de la chirurgie est de 46,8 g. Les grades Gleason (G) des tumeurs à la biopsie de même qu’à la chirurgie sont de 7. On observe une fréquence deux fois plus grande du patron 3+4 que du patron 4+3. Pour l’ensemble des cas, la distribution des grades Gleason est la suivante : G6 : 23,9% ; G7 : 65,7% ; G8 : 5,4% ; G9 : 4,9% et G10 : 0,1%. Le stade tumoral prédominant est pT2 (62,8%), suivi de pT3a (26,5%) et de pT3b (10,8%). L’extension extraprostatique et l’invasion des vésicules séminales sont observées dans 38% et 10,7% des cas, respectivement. Aucune différence significative n’a été notée d’un site à l’autre en ce qui a trait aux données cliniques, ce qui milite en faveur de l’homogénéité de la cohorte et de l’excellence des procédures suivies. En effet, les analyses effectuées ont démontré la haute qualité de l’ADN et des ARN sanguins et tissulaires. L’examen histopathologique a révélé une excellente préservation des tissus prostatiques congelés au fil du temps, i.e. depuis 2007. Jusqu’à maintenant, 375 cas de récidive, soit un taux de 19%, ont été identifiés. La nature prospective de la Biobanque permet de recueillir de nouveaux échantillons (sang et urine) lors des visites de suivi et de mettre à jour les données cliniques, augmentant ainsi l’unicité et la valeur de la Biobanque.

Conclusions :

La Biobanque PROCURE a atteint une maturité telle qu’elle représente désormais un outil de choix pour la recherche sur le cancer de la prostate. Avec une cohorte de plus de 2000 patients et une projection de 10 années de suivi, la valeur de la Biobanque continuera de s’accroître, en faisant ainsi une ressource inestimable pour les chercheurs œuvrant dans ce domaine.

Creation d’une neovessie ileale par laparoscopie robot-assistee apres cystoprostatectomie totale pour cancer de vessie.

Introduction et objectifs :

La réalisation d’une cystoprostatectomie pour cancer infiltrant de vessie est actuellement réalisable, dans certains centres, par laparoscopie robot-assistée. La plupart des équipes réalisent une laparotomie pour le temps de reconstruction urinaire, notamment pour la création d’une néovessie iléale. L’intéret de réaliser la dérivation urinaire sous laparoscopie est de limiter le saignement et garder les bénéfices d’une chirurgie en milieu clos.

Matériels et méthodes :

La vidéo montre un exemple de réalisation intra-corporelle robotassistée d’une néovessie iléale selon Studer, en gardant les memes principes que ceux de la chirurgie ouverte classique. Il s’agit d’une réalisation étape par étape de la technique chirurgicale qui montre les différents temps clef de l’intervention.

Résultats :

La cystoprostatectomie avec création d’une néovessie selon Studer par laparoscopie robot-assistée est réalisée en 6 heures, un temps equivalent a celui de la chirurgie ouverte dans notre experience.

Conclusions :

La réalisation d’une néovessie iléale selon Studer, apres cystoprostatectomie pour cancer infiltrant de vessie, est réalisable sous laparoscopie robot-assistée dans un temps équivalent a celui de la chirurgie ouverte et en suivant les memes principes chirurgicaux.
