On page 3704 in the 11 June 2015 issue, there is an error in Table 1. The dosage for cytarabine, found in the Consolidation-1/intensification section in the C10403³⁴ column, reads “500 mg/m² IV/SC (d 1-4, 8-11, 29-32, 36-39).” It should read “75 mg/m² IV/SC (d 1-4, 8-11, 29-32, 36-39).” The error has been corrected in the online version, which now differs from the print version.
