The recent report by Pamplona et al. on racemose neurocysticercosis is very interesting.1 Pamplona et al. concluded that “it is important to recognize neurocysticercosis as a differential diagnosis in intra-cranial cysts, not only intraparenchymal cysts.”1 Indeed, neurocysticercosis can be seen at any part of the neurological system.2 Neurological symptoms due to neurocysticercosis can be serious. However, asymptomatic neurocysticercosis can also be seen. Neuroimaging technology helps detect neurocysticercosis. The interesting concern is on the definitive diagnosis. In the present case,1 the final diagnosis is done by biopsy study. A brain biopsy may be harmful, and there should be an alternative option. The use of high-resolution magnetic resonance (MR) imaging might be useful in identification of cystic content that is useful in the diagnosis of neurocysticercosis.3 Advance techniques including 1H nuclear MR spectroscopy, diffusion-weighted imaging, and MR perfusion imaging can be helpful in making a differential diagnosis.4 Diagnosis by neuroimaging is interesting. According to a recent expert commentary, repeated imaging, following-up of a suspected neurological cyst and antiparasitic treatment is suggested.5
