ADVANCE CARE PLANNING EDUCATIONAL RESOURCES AND DECISION AIDS IN SUPPORTING THE ACP PROCESS

Abstract

Objective: The goal of this project is to identify and evaluate Advanced Care Planning (ACP) decision aids so that they can be appropriately used by patients and health care providers to support the (ACP) process. Methods: A systematic evaluation of ACP decision aids was completed. ACP decision aids were evaluated for cost, languages available, Flesch-Kincaid reading ease and grade level, PEMAT Score, and categories of content including Advance directive/Health care proxy, POLST, Living will, values, preferences and goals. Results: Twenty decision aids were identified. The decision aids included workbooks, interactive websites, and activities/conversation guides. Conclusion: Our analysis has identified the need to continue to refine these decision aids so that they can reach a broad population with varying literacy and language needs. Practice Implications: The analysis presented can provide health care providers the confidence to use these decision aids in practice.
