20 February 2008 New citation required and conclusions have changed Substantive amendment. This review contains results from one new trial (Elder 2006) and further data from a previously included study (Knivsberg 2002).
 
 In addition, we have learned of an ongoing USA‐based Phase I randomized, double‐blind, placebo controlled, parallel assignment, safety/efficacy study, which is now within the references of this review. The study is anticipated to be completed in April 2008.
