Response to the Comment by Fabian et al. on Our Report Entitled “Two Uveal Melanomas in One Eye: A Choroidal Nevus Giving Rise to a Melanoma in an Eye with a Separate Large Choroidal Melanoma”

We thank Fabian et al. for their interest in our report entitled “Two Uveal Melanomas in One Eye: A Choroidal Nevus Giving Rise to a Melanoma in an Eye with a Separate Large Choroidal Melanoma” [1] and for highlighting their excellent article entitled “Malignant Transformation of a Choroidal Nevus in an Eye Treated for Choroidal Melanoma” [2]. Their article describes a patient with a choroidal melanoma and a separate nevus that transformed clinically into a melanoma 5 years after diagnosis. Our case, similar to their article, describes a patient with a choroidal melanoma and a separate choroidal mass that clinically appeared to be a nevus. Our case is unique in that we were able to show through histopathologic analysis that the bland appearing nevus tissue had a small focus of melanoma cells arising within it before any clinical clues for malignancy were evident. We were also able to show that both lesions harbored GNAQ mutations and lacked a known predisposition. Both articles emphasize the importance of closely monitoring choroidal nevus for any signs of malignant transformation, especially in eyes that also have a choroidal melanoma.
