On page 8, second column, paragraph 4 on the Polish centers, MUL CKD Hospital in Lodz/Poland was credited with “I. Cygankiewicz, P. Ptaszyński”. Instead it should have been: “I. Cygankiewicz, P. Ptaszyński, K. Kaczmarek, I. Poddebska”.
