Cutting off half of QRS duration can cause overcorrection of QT interval in left bundle branch block

Dear Editor,

We read with great interest the article by Bogossian et al., (2017) entitled “A New Experimentally Validated Formula to Calculate the QT Interval in the Presence of Left Bundle Branch Block Holds True in the Clinical Setting”. March, 2017. The authors demonstrated that in the presence of left bundle branch block (LBBB), the true QT interval could be estimated using the formula [QTm = QTb − 50% × QRSb] (Bogossian et al., 2014). We tested this formula and found it actually overcorrected QT interval in patients with LBBB.

The falsely prolonged QT interval in LBBB is primarily caused by the widened QRS (QRSb) due to delayed depolarization (Bogossian et al., 2014; Das, 1990). The key is to know how much of QRSb added to the QT interval in LBBB. In the formula, the authors simply chopped off 50% of QRSb. Table 1 demonstrates that when we applied this formula to a group of patients (n = 22) with intermittent LBBB, the estimated QT becomes significantly shorter than the baseline level (424 ± 31 vs. 441 ± 34 ms, p = .004). The JT interval did not alter much in LBBB, so what is the cause? We looked at the net increase in QRS duration, and noticed it was only about 40% of QRSb (0.39 ± 0.04). The same is true in the authors’ data (Bogossian et al., 2017). Therefore, we modified the formula by cutting off 40% of QRSb, and by doing so the best estimate of the true QT interval was achieved (442 ± 33 vs. 441 ± 34 ms, p = .969). In other words, replacing 50% with 40% of QRSb in the formula [QTm = QTb − 40% × QRSb] can prevent overcorrection of the QT interval in LBBB. To determine whether such modification is valid, we applied Rautaharju formula designed for LBBB (Rautaharju, Zhang, Prineas, & Heiss, 2004) in our dataset. Table 1 shows that the results are compatible. Nevertheless, Rautaharju formula is more complicated. QTm = QTb − 40% × QRSb, on the other hand, is much easier to remember and apply in clinical practice.

QTc50: QTm = QTb − 50% × QRSb was applied for the QT component of heart rate‐corrected QT interval by cutting off 50% of QRS duration in LBBB.

b

QTc40: QTm = QTb − 40% × QRSb was applied for QTc, respectively.

c

QTcR: Heart rate−corrected QT interval corrected by Rautaharju formula. (Rautaharju et al., 2004).

d

Paired t test between QTc50 and baseline QTc with narrow QRS: p = .004.

e

Paired t test between QTc40 and QTc with narrow QRS: p = .969.

f

Paired t test between QTc50 and QTc with narrow QRS: p = .533.

Based on our testing, the net increase in QRS is about 40% of the widened QRS in LBBB in both our and the authors’ dataset. Therefore, we would suggest adjusting the quotient from 50% to 40% of QRSb in the new formula to prevent overcorrection of QT interval.
