The injected dose listed on page 4388 ¹ for the ultrasound agent (Definity^™) was incorrectly quoted; it should read 20 μl kg⁻¹ instead of 40 μl kg⁻¹. Figure 7 on page 4393 was published incorrectly; the symbols for both source‐ and CT‐based phase & amplitude corrections were mistakenly reversed from their corresponding phase correction case. The correct version of the figure is given below.
