Abstract
Objective: To determine whether utilization of iron from infant cereal and pureed meat was sufficient to prevent iron depletion and/or anaemia in infants 6 to 12 months old fed whole cow milk (WCM) as their primary milk source.
Design: Six-month-old infants were randomized into a treatment group (n=43) receiving iron-fortified infant cereal (10.2 mg iron), pureed meat (0.75–1.7 mg iron) and WCM for six months or a control group (n=54) receiving no dietary intervention. Haemoglobin < 110 g/L or ferritin < 10 μg/L (measured bi-monthly), confirmed in a second blood sample, defined end-points.
Results: Proportion reaching end-point was similar between the treatment (3/43) and control infants (5/54) (p=0.66). Infants not complying with the protocol were at greater risk of reaching end-point (p=0.0002). Change in haemoglobin and ferritin across age was similar in both groups.
Conclusions: Iron deficiency is not a concern in WCM-fed infants after six months of age if iron-containing complementary foods are concurrently ingested.
Résumé
Objectif: Déterminer si l’utilisation du fer provenant des céréales pour enfants et de la viande en purée était suffisante pour prévenir l’appauvrissement en fer ou l’anémie chez les enfants de 6 à 12 mois dont la principale source de lait est du lait entier de vache.
Méthodologie: Des enfants de six mois ont été placés, de manière aléatoire, dans le groupe traitement (n=43) où ils ont reçus des céréales pour bébés enrichies en fer (10,2 mg de fer), de la viande en purée (0,75–1,7 mg de fer) et du lait entier de vache pendant six mois, ou dans le groupe contrôle (n=54) qui n’a fait l’objet d’aucune intervention alimentaire. L’hémoglobine < 110 g/L ou la ferritine < 10 μg/L (mesurée deux fois par mois), confirmée à l’aide d’une deuxième prise de sang, a permis de déterminer les limites.
Résultats: La proportion de sujets qui arrivaient au point limite était semblable chez les enfants du groupe traitement (3/43) et ceux du groupe contrôle (5/54) (p=0,66). Les bébés pour lesquels on n’a pas respecté le protocole risquent d’avantage d’arriver au point limite (p=0,0002). Le changement dans l’hémoglobine et la ferritine était semblable dans les deux groupes, quel que soit l’âge des sujets.
Conclusions: Le manque de fer ne constitue pas un problème chez les bébés de plus de six mois nourris au lait entier de vache s’ils consomment, en parallèle, des aliments complémentaires qui contiennent du fer.
