Brief history

A 53-year-old woman with newly diagnosed acute myeloid leukemia (AML) developed a new pruritic, papular eruption approximately 1 week after initiation of induction chemotherapy with daunorubicin and cytarabine. The rash started on her back and progressed to involve the neck, chest, abdomen, and extremities. The patient had persistent neutropenic fevers of unknown etiology and began treatment with cefepime 2 days before the rash developed. She also complained of drenching night sweats. On physical examination, numerous pink edematous papules were scattered across the back, lateral aspects of the neck, chest, abdomen, and extremities (Figs 1 and 2). Punch biopsy of a left thigh lesion was performed (Fig 3).
