利妥昔单抗追加治疗对初次完全缓解的弥漫大B细胞淋巴瘤患者的疗效评估

Efficacy of additional two cycles of rituximab administration for patients with diffuse large B-cell lymphoma in first remission

Abstract

Objective

To analyze the efficacy of additional two cycles of rituximab administration for Chinese patients with diffuse large B-cell lymphoma (DLBCL) in first complete remission (CR) after six cycles of standard 21-day rituximab plus cyclophosphamide, doxorubicin, vincristine and prednisone (R-CHOP21).

Methods

Retrospective analysis was performed in 351 patients with DLBCL diagnosed from March 2003 to March 2012. International Prognosis Index (IPI), Revised (R)-IPI and National Comprehensive Cancer Network (NCCN)-IPI were calculated for each patient. Patients were divided into GCB and non-GCB subtype according to Han's Classification. Progression-free survival (PFS) and overall survival (OS) were analyzed using Kaplan-Meier methods.

Results

282 (80.3%) patients achieved CR and 132 (46.8%) of 282 cases received additional two rituximab therapy. The other 150 (53.2%) patients entered into observation on the intention of the patients. No significant difference was observed in baseline characteristics between the two groups. 3-year estimated PFS for additional rituximab group and observation group were 80.0% and 78.1% (P=0.334), while 3-year estimated OS were 89.7% vs. 86.1% (P=0.452). By subgroup analysis, prolonged PFS were observed in R-IPI low-risk and NCCN-IPI low-risk patients after additional two rituximab cycles.

Conclusion

For patients with DLBCL in first remission after standard six cycles of R-CHOP21 regimen, additional two cycles of rituximab maintenance did not significantly improve the general prognosis, but low-risk subgroups of R-IPI and NCCN-IPI could benefit from this regimen.

弥漫大B细胞淋巴瘤(DLBCL)是非霍奇金淋巴瘤(NHL)中最常见的亚型,占西方国家NHL的30%~40%[1]。R-CHOP21(利妥昔单抗联合环磷酰胺、多柔比星、长春新碱及泼尼松,21 d为1个疗程)方案已成为DLBCL的标准一线治疗方案[2][3]。在标准R-CHOP21方案治疗的基础上,仍有30%~40%患者出现复发[2],[4],增加利妥昔单抗的治疗次数已成为提高疗效的候选方案之一。目前为数不多的研究报道显示,利妥昔单抗维持治疗在DLBCL中的作用及适合人群仍存在争议。并且,国际预后指数(IPI)、修订的国际预后指数(R-IPI)及国立综合癌症网络国际预后指数(NCCN-IPI)等预后模型及Hans分型在接受利妥昔单抗追加治疗患者中的意义尚不明确。我们在本研究中回顾性分析以6个疗程标准R-CHOP21方案作为初始治疗方案的351例DLBCL患者资料,探讨获得初次完全缓解(CR)后继续2次利妥昔单抗追加治疗的临床意义。

病例与方法

1.病例:收集2003年3月至2012年3月在我院确诊的351例DLBCL患者资料。所有患者的诊断均符合世界卫生组织(WHO)2001年关于淋巴组织肿瘤分类标准[5],并进行IPI、R-IPI、NCCN-IPI评分,以及Hans分型区分生发中心来源(GCB)和非生发中心来源(non-GCB)。

2.治疗与疗效评估:入选患者初治时均接受6个疗程R-CHOP21方案治疗,评估手段包括PET-CT检查或颈、胸、腹、盆增强CT,并依照国际工作组标准[6]评定疗效。282例(80.3%)患者获得CR,根据患者意愿,132例(46.8%)继续接受2个疗程利妥昔单抗追加治疗(追加组),间隔21 d,治疗剂量均按标准浓度根据体表面积计算;150例(53.2%)患者治疗结束后进入随访(对照组)。

3.随访及预后:随访方式主要为电话随访和住院或门诊复查,末次随访时间为2015年1月1日。无进展生存(PFS)时间定义为明确诊断时间至疾病进展、复发或死亡时间。总生存(OS)时间定义为明确诊断时间至死亡时间或末次随访时间。

4.统计学处理:采用SPSS 19.0软件进行统计学分析。采用Kaplan-Meier方法描绘生存曲线,单因素分析采用Log-rank检验,各分类变量之间采用χ2检验,双侧P<0.05为差异有统计学意义。

结果

1.一般资料:患者的临床特征见表1。总中位发病年龄为55 (15~89)岁,追加组和对照组患者中位发病年龄分别为55 (16~82)岁和55.5 (16~79)岁,两组在性别、年龄、Ann Arbor分期、美国东部肿瘤协作组(ECOG)体力状态评分、结外累及器官数、LDH水平、B症状及IPI、R-IPI、NCCN-IPI和Hans分型分布的差异均无统计学意义(P值均>0.05)。

2.生存情况:截至随访结束,中位随访时间为40.4个月。351例患者CR率为80.3%,复发或进展患者114例(32.5%),死亡患者76例(21.7%);282例CR患者中追加组和对照组分别复发或进展27例和34例,死亡19例和22例。351例患者总体3年PFS和OS率分别为68.4%和79.5%;追加组和对照组患者的3年PFS率分别为80.0%和78.1%(P=0.334),3年OS率分别为89.7%和为86.1%(P=0.452),差异均无统计学意义(图1)。

在入组的351例患者中,IPI、R-IPI、NCCN-IPI预后模型均能明显区分患者危险程度,3年PFS、OS率差异均有统计学意义(P值均<0.05);经标准治疗后获得初次CR的DLBCL患者,也能较好地被上述3种预后模型区分危险程度,3年PFS、OS率差异均有统计学意义(P值均<0.05)(表2)。

3.亚组分析:对比性别、年龄、ECOG评分、Ann Arbor分期、LDH水平、B症状亚组中追加组与对照组患者的PFS和OS率,结果显示差异均无统计学意义(P值均>0.05)。

对比IPI、R-IPI、NCCN-IPI各危险分层中追加组与对照组患者的PFS和OS率,结果显示追加组与对照组R-IPI分层低危患者的3年PFS率分别为100.0%和87.5%(P=0.017),NCCN-IPI分层低危患者的3年PFS率分别为100.0%和87.1%(P=0.017),差异均有统计学意义;3年OS率差异均无统计学意义(P值分别为0.284和0.292)(图2)。比较GCB和non-GCB患者中追加组与对照组的PFS和OS,non-GCB分型患者3年PFS率分别为81.4%和75.0%(P= 0.116),3年OS率分别为95.7%和84.6%(P=0.081)。

讨论

DLBCL是一组高度异质性的疾病,根据形态学、分子生物学、临床特征等方面可将其分为不同亚型。既往年轻低危患者(MInT试验[7])R-CHOP方案一线治疗的缓解率为86%,老年患者(LNH-98.5试验[2])为76%。本研究包含全年龄段患者,缓解率为80.3%,总体复发或疾病进展率为32.5%,CR后复发率为21.6%,与既往文献报道相近[2][3]

根据目前研究报道,利妥昔单抗维持治疗在DLBCL患者中的作用及适合人群存在争议。Jaeger等[8]在NHL13研究中将初次获得CR的662例DLBCL患者和21例滤泡性淋巴瘤3B级患者随机分为利妥昔单抗维持组和对照组,发现维持治疗无法改善患者的总体生存,但可显著提高男性患者的无事件生存(EFS)及PFS率,其中IPI低危男性预后最佳。Huang等[9]回顾性分析207例15~60岁的DLBCL患者,发现利妥昔单抗维持可提高总体患者的PFS及IPI≥3分患者的OS率。Jung等[10]对41例初次治疗后获得CR和部分缓解(PR)的老年DLBCL进行利妥昔单抗巩固治疗,发现PFS有所改善。然而,Habermann等[11]在GALGB 9793研究中,对318例老年DLBCL患者进行分析,并未发现维持治疗能够延长患者的无失败生存(FFS)。复发DLBCL中,Gisselbrecht等[12]在CORAL研究中,将首次复发后行自体造血干细胞移植获得成功的242例患者随机分为利妥昔单抗维持治疗组和对照组,观察两组间患者的EFS差异并无统计学意义。本研究结果显示,2个疗程利妥昔单抗追加治疗未能延长整体DLBCL患者的PFS和OS时间,与NHL13[8]或GALGB 9793[11]研究结果类似。

几项大规模的随机对照研究中利妥昔单抗维持的单次剂量均为375 mg/m2,但维持频度和持续时间均不统一:NHL13[8]研究中利妥昔单抗间隔时间为2个月,维持12次;GALGB 9793[11]研究中间隔时间为6个月,维持4次;本研究为追加2个疗程,间隔时间为21 d。患者分布比例比较:NHL13[8]研究中维持组和对照组患者的中位发病年龄分别为57(19~87)岁和58 (19~88)岁,IPI低危患者各占47.6%和48.0%;GALGB 9793[11]研究中R-CHOP治疗组患者的中位发病年龄69 (60~92)岁,IPI低危患者占12.0%,;本研究中患者的中位年龄为55 (15~89)岁,追加组和对照组中IPI低危患者各占53.0%和58.0%。

DLBCL患者预后差异大,亟需有效的预后模型以适应个体化分层治疗。IPI是基于年龄、Ann Arbor分期、ECOG评分、结外累及器官数目及LDH水平的预后模型,是目前最常用的预后体系[13]。2007年提出的R-IPI重新划分分数等级,被认为更有助于R-CHOP治疗的患者危险分层[14]。2013年提出的NCCN-IPI模型基于利妥昔单抗时代的DLBCL患者,细化了年龄和LDH水平,并重新定义骨髓、肝脏、消化道或肺脏等高危结外器官累及[15]

本研究我们验证了利妥昔单抗时代IPI、R-IPI及NCCN-IPI在DLBCL中的预后作用。经过一线治疗后获得CR的患者无论维持治疗与否,这三种预后模型均具备良好的预后指导意义。此外,我们发现R-IPI低危和NCCN-IPI低危患者可获益于利妥昔单抗的追加治疗。参考NHL13等研究中对IPI≤1分及>1分的亚组分析可以发现,IPI低危患者更倾向于从维持治疗中获益(IPI≤1:HR=0.65, 95%CI=0.39~1.10,P=0.111;IPI>1:HR=0.87,95% CI=0.58~1.30,P=0.494)。即随着危险组别升高,患者从维持治疗中的获益有下降趋势。尽管本研究为追加2个疗程治疗,但我们的观察结果与之相符,并且相比之下疗程数减少,治疗时间缩短,治疗总体费用降低,患者接受能力较强。结果提示目前通用的临床预后评分仍适用于筛选出可获益于利妥昔单抗追加治疗的群体,6次标准R-CHOP21治疗后获得初次CR的DLBCL患者,继续2个疗程利妥昔单抗追加治疗虽未明显改善总体预后,但可延长R-IPI及NCCN-IPI低危患者的PFS,为患者争取到更长时间的治疗缓解期,提高生存质量,争取更前沿的靶向治疗方法的机会。对于R-IPI及NCCN-IPI分层中的高危患者,其预后差,这一部分患者很有可能由于本身疾病原因,需要其他巩固方案或更密集的利妥昔单抗来改善预后,减少患者复发。

本研究亦针对不同细胞来源的DLBCL进行亚组分析。Hans分型是区分GCB和non-GCB的DLBCL的经典方法[16]。追加治疗组和对照组未在GCB和non-GCB患者中显示出差异有统计学意义。Saito等[17]报道利妥昔单抗联合化疗可显著提高non-GCB患者的预后(2年OS率为80%对40%,P<0.01;2年EFS率为80%对33%,P<0.01)。在分子生物学层面上,non-GCB亚型存在B细胞受体/NF-κB通路异常激活,利妥昔单抗靶向该通路[18][19]。需要更进一步的分子生物标志研究来进一步阐明利妥昔单抗在DLBCL各分子亚型患者中的疗效。

综上,本研究我们利用DLBCL患者初治前的临床资料,筛选可获益于利妥昔单抗追加2个疗程的亚组,以期提高患者长期预后,利于临床实践中DLBCL患者的个体化、差异化分层治疗。
