患者,男,58岁,2014年起发现外周血白细胞偏高,WBC 15×109/L。2014年12月行骨髓穿刺涂片示“慢性粒-单核细胞白血病?”,外周血单核细胞2.1×109/L(9.0%),幼粒细胞易见;染色体核型46,XY[20];骨髓流式细胞术示“CD34+细胞占0.7%,疑为原始细胞,表达CD10、CD13、CD15、CD16、CD33、CD64”。未予治疗。2017年3月血常规:WBC 48.63×109/L,HGB 55 g/L,PLT 140×109/L;脾脏肿大(229 mm×77 mm);骨髓穿刺涂片:骨髓增生明显活跃,粒红比升高,粒巨二系增生活跃,伴成熟障碍,红系尚增生,粒红二系可见发育异常;外周血涂片示“幼粒细胞易见,可见幼红细胞,单核细胞1.9×109/L(7.0%)”;骨髓活检病理示“造血细胞粒系、巨核系增生活跃,以粒系为明显(晚幼粒较多),网状++”。急性髓系白血病/骨髓增生异常综合征(AML/MDS)相关基因突变筛查:KRAS c.G179A(p.G60D)1.76%,ASXL1 c.2128delG(p.G710fs)47.11%,SRSF2 c.C284T(p.P95L)69.35%,NRAS4 c.G34T(p.G12C)1.78%,CBL c.G1259A(p.R420Q)45.01%,SETBP1 c.G2602A(p.D868N)46.69%。诊断为“慢性粒-单核细胞白血病”。口服羟基脲降白细胞后给予地西他滨20 mg·m−2·d−1×5 d,3个疗程后复查血常规“WBC 2.23×109/L,HGB 41 g/L,PLT 117×109/L”。2017年9月骨髓涂片示“骨髓增生活跃,原始细胞0.035,粒红比升高,粒系增生活跃,核稍左移,AKP积分升高,红系增生减低,巨系增生,血小板散在或成簇可见”;外周血单核细胞23%;骨髓活检病理示“小淋巴样细胞高度增生,弥漫浸润,符合骨髓增生性疾病,网状+++”。因地西他滨治疗后贫血无改善且伴有骨髓纤维化,2017年10月起口服芦可替尼30 mg/d(患者及家属知情同意)。2018年4月复查,血常规示“WBC 23.36×109/L,HGB 128 g/L,PLT 244×109/L”;骨髓涂片示“骨髓增生活跃,粒巨二系增生活跃,红系增生低下,粒巨二系可见轻度发育异常,血小板散在或成簇可见,原始细胞0.015”。外周血涂片原始细胞1.0%,单核细胞26.0%。AML/MDS相关基因突变复查示“SRSF2 c.C284T(p.P95L)37.84%,ASXL1 c.2128delG(p.G710fs)46.68%,SETBP1 c.G2602A(p.D868N)48.83%”。随访至今患者仍口服芦可替尼治疗中,疾病稳定,未出现血液学和非血液学不良反应。
