In the July 2017 supplement of Medical Care, there was an error in Table 1 of the article by Kilbourne et al. In the fifth row under the first column titled “QUERI Program,” the name of the program “De-implementation QUERI (Puget Sound)” should be “Safety and Quality QUERI (Puget Sound)”.
