Some surgeons are reluctant to perform digital percutaneous needle fasciotomy (PCNF) due to anatomical risks. While training surgeons, we advocate using a needle with a double 90-degree step (Fig 1). By orientating the double 90-degree step and meticulously aligning the bevel with the step, it ensures the trainee surgeon is constantly aware of the orientation and depth of the bevel before advancing further. The modified shape, with a fixed known length created in the distal portion of the needle, allows depth to be more easily assessed, thus reducing anxiety of the supervisor during training.
