Implant Abutment Junction: Demystified

Introduction: Implant abutment connection is crucial synapse between the implant and abutment and responsible for transmitting of occlusal forces from prosthesis down to the implant fixture. The effect of implant abutment connection to the success of implant therapy was very less understood.

Summary: The original external hex system (Branemark system) was used for many years, however various complications like screw failure and abmormal crestal bone loss were observed. Even single piece implants which had no such connection, were not successful in all cases. With the advent of time , platform switch concept came into existence which revolutionised the implant abutment junction systems. Various internal hex systems were also introduced each having its own advantages and disadvantages. The stress load performance , mechanical strength , microgaps, effect on biological width and crestal bone loss was different for various systems. Web based searches were made in various search engines like Pub Med , Ebsco , Scopus about the available implant abutment connection systems.

Conclusion: Each implant company advocated its own attachment system to be superior from others citing various test results. Hence , it is confusing for a clinician to choose the type of attachment. This paper discusses on various implant abutment junctions and newer concepts pertaining to it.
