In the original article [1] there was a mistake in the published version of Tables 2 and 3; information about The Netherlands was missing in both Tables 2 and 3. The corrected Table 2 and Table 3 appear below. The authors apologize for any inconvenience caused and state that the scientific conclusions are unaffected. The original article has been updated.
