A 17-year-old girl developed transfusion associated circulatory overload while receiving immune-globulin for multisystem inflammatory syndrome in children.
The girl, who had a three weeks prior history of COVID-19 admitted with fever and dehydration. She was then diagnosed with multisystem inflammatory syndrome in children and placed on IV immune-globulin infusion [dosage not stated]. However, she developed grunting within 20 minutes of immune-globulin infusion initiation.
The girl received haemotherapy, and had also received intravenous fluids 48h prior. Post haemotherapy (HT), workup for suspected transfusion reaction revealed elevated blood pressure and temperature with chest X-ray showing increased interstitial markings, indicating transfusion associated circulatory overload. The NT-proBNP level pre/post HT were 17121 pg/ml and 19824 pg/ml, respectively. Her symptoms improved on diuretics.
