We would like to share ideas on the publication “Case report: Pituitary apoplexy after COVID-19 vaccination.1” Piñar-Gutiérrez et al. mentioned that “we suggest that this case should be taken into account for the evaluation of postvaccinal headache and the possibility of VITT, evaluating campimetric compromise and the possibility of hormonal deficits, both in patients with known pituitary adenomas and in cases without prior diagnosis.1” We concur that the present vaccine recipient has a pituitary problem. It might or might not be an adverse effect of COVID-19 vaccination. If it is a direct effect from vaccination, the pathogenesis is still unknown. Piñar-Gutiérrez et al. already mentioned for abnormal immune response and VITT. Nevertheless, we should also recognize for possibility of other concurrent incidence. A headache symptom might be a presentation of other disorder. A medical problem might cooccur in a vaccine recipient. For example, dengue might be a concurrent medical disorder and it might lead to pituitary problem.2
