MYASTHENIA GRAVIS MANIFESTATION AND MEDIASTINAL TUMOR FINDING DURING COVID-19

INTRODUCTION: Most patients who develop COVID-19 have mild disease and should continue the current best practice standard of care for MG/LEMS. Viral and bacterial infections are established triggers for a myasthenic crisis in patients with preexisting myasthenia gravis or in those who didn't know thry have it. COVID infection can predispose to many autoimmune and neurological conditions, thymomatous myasthenia gravis being one of them

CASE PRESENTATION: Our patient presented with new onset myasthenia gravis and thymoma following SARS CoV19 infection a M 33Y MODERATE COVID 6 MONTHS AGO NEUROMUSCULAR SIMPTOMS INITIALLY TREATED LIKE GUILLAN BARRE SYNDROME, POSTERIORLY OPRESS CHEST PAIN, CT FINDING ANTERIOR MEDIASTINAL MASS, TRUCUT THYMOMA WAS DIAGNOSES AND THE COMPLETE APPROACH WITH ANTIBODIES WAS DONE TO GET THE DECISON TO RESECT THE TUMOR.

DISCUSSION: Thymoma associated with myasthenia gravis requires a multimodality management with surgery as the gold standard treatment, resect thymic fat, ensures superior outcomes with respect to myasthenia gravis The cross reactivity between antibodies against covid19 virus and AChR might play a role in activation of proinflammatory cascade, cytokines and chemokines along with B and T cell depletion and increased TNF and interleukins loss immunological tolerance aiding the disease process, post covid19 myasthenia gravis may noted at admission, but it may have a delayed presentation after months of the viral prodrome

CONCLUSIONS: COVID infection is associated with pathology of the peripheral nervous resulted in exacerbation of various autoimmune and neurological disorders. Our patient presented with a thymoma following covid19 infection with anti acetyl choline receptor antibody positive myasthenia gravis. Surgical management is 1st treatment option
