MORTALITY AND ITS PREDICTORS IN PATIENTS WITH COVID-19 WITH PREEXISTING INTERSTITIAL LUNG DISEASE

PURPOSE: The data on the impact of COVID 19 on Interstitial Lung Disease (ILD) is scarse. There is limited data from India till date to assess the impact of COVID-19 in patients with preexisting ILD. We undertook this study to assess the clinical outcome of ILD patients admitted in our hospital with COVID 19.

METHODS: In this retrospective observational study, records of RT-PCR confirmed COVID 19 patients with pre-existing ILD who were admitted to the hospital in the period from 1st March to 30th November were obtained from the hospital database. The clinical outcomes of the patients were recorded. Univariate analysis was performed to find relation between risk factors for mortality and those with statistical significance(p<0.05) were subjected to multiple logistic regression to determine independent risk factors.

RESULTS: In our study of 16 patients, the overall mortality was 43.75%. There was no effect of age, gender, comorbidities, type of ILD, CT thorax findings on diagnosis, use of corticosteroids and antifibrotics in the past on mortality. On univariate analysis, significant parameters associated with mortality were FVC%, PaO2/FiO2, D-dimer, ferritin and IL-6. However, on multivariate analysis, the only significant parameters were IL-6, p=0.0246, OR= 12.65(6.9-18.4) and D-dimer, p =0.0477, OR=83.3 (24.2-144.4).

CONCLUSIONS: COVID-19 in patients with preexisting ILD has a comparatively higher mortality. D-dimer and IL-6 are significant predictors of mortality in patients of ILD infected with COVID-19.

CLINICAL IMPLICATIONS: Covid has high mortality among patients of pre-existing ILD. Increased D-Dimer and IL-6 in this population indicate high mortality and aggressive treatment is warrented
